Occasional weekend duties required. The RN will collaborate with the provider and healthcare center team so that completion of the appropriate measures are seamless. The QCMRN will understand the needed measures and work with both the Quality Care Management Program leadership, HCC leadership and providers in providing standard and creative ways for patient engagement that both empowers the patient and completes their gaps in care for stronger engagement with their healthcare and healthcare providers. Retention of patients to stay in care and to actively participate in their healthcare is priority for the QCMRN. The program and position center on closing gaps in care, removing barriers to receiving, completing preventative measures, adhering the the HEDIS and HAB measures and collaboratively executing transitions of care and/or chronic care management with the client, medical provider and HCC staff.
