What you'll do as a Certified Home Health Aide (HHA):
Under the direct supervision of an RN Case Manager, you will provide patient care according to the Plan of Care. You will also:
Provide personal care including, baths, back rubs, oral hygiene, shampoos, and changing bed linen
Assist patients with dressing and undressing
Provide skin care to prevent breakdown
Assist the patient with toileting activities
Keep patient's living area clean and orderly, as appropriate
Assist in feeding the patient, if necessary.
Perform range of motion and other simple procedures as an extensional therapy service as ordered (with appropriate completed/demonstrated skills competency).
Assist patient in the self-administration of medication.
Meet safety needs of patients and using equipment safely and properly (foot stools, side rails, etc.).
Report on patient's condition and significant changes to the assigned nurse.

What you'll do as a Speech Therapist/Language Pathologies (ST)
In partnership with the Case Manager you will provide skilled services to assist patients with breathing, speaking, and swallowing functions. You will evaluate and treat speech, language, cognitive and fluency disorders that affect communication. You will serve as an educator, patient advocate and a member of the interdisciplinary care team.

What you'll do as a Medical Social Worker (MSW) Home Health:
Under the general direction of the Case Manager, the Social Worker assesses social and emotional factors related to the patient's illness, medical and nursing requirements in the home. Develops and implements a plan for home social work services, including education of patients, family members and care givers. This position also coordinates with other home health staff and community resources to provide an efficient integration of care.
To be a success as a Medical Social Worker (MSW) Home Health:
Knowledge of local community resources is required.
