Responsible for informing the community and creating awareness about Borinquen Medical Centers (BMC) services, including: substance use, mental health, HIV prevention, and prevention education on the risks of substance misuse, HIV/AIDS, and STDs. The Behavioral Health Outreach Specialist also conducts outreach in high-risk areas to recruit participants for testing and BMC services, provides needed linkages to service provision for individuals with mental health, substance use and/or co-occurring disorders, and participates in the coordination and facilitation of presentations to educate the community about our services. The Residential Counselor (Milieu Counselor) provides milieu management, guidance, safety, stability, and direction to a client population that is actively intoxicated and/or experiencing withdrawal symptoms. This is a pool (on call) position with no set schedule. Education, Knowledge, Skills & Experience Required: Minimum of High school diploma or GED equivalent Required Preferred Bachelor's degree in a related field. Essential Duties: Assessment and documentation of levels of intoxication, including breath alcohol levels, withdrawal symptoms, vital signs, physical condition, mental status and behavior. Identifies signs of an emergency and reacts appropriately. Ensures Adverse Incident Reporting (AIR) procedures are followed during shift. Monitor milieu to ensure safety and to make sure client needs are met. Including but not limited to cleaning/organization of shared spaces, restocking supplies, ensuring doors and windows remain closed/secure. Facilitate and co-facilitate psychoeducation groups on unit with clients. Effectively responds to client needs and problems. Engages clients in discussions to identify goals and build rapport; provide referrals to supportive services when needed. Serves as Acting Shift Coordinator when coverage is needed (pay differential is provided). Practices OSHA safety standards and disposes of biohazard waste according to OSHA standards. If given approval, must transport clients to and from hospitals, aftercare, or other approved services as directed by the Shift Coordinator. Rocky Mountain Health Care Services (RMHCS) Counselors are responsible for providing assessments and individual/group counseling sessions to RMHCS participants. Counselors conduct assessments of participants' mental health and cognitive functioning and develop appropriate treatment plans. Counselors are an essential member of the interdisciplinary team (IDT) model of care and play an integral role in the overall well-being of our participants. MISSION: Improving lives, optimizing wellness, promoting independence RESPONSIBILITIES AND DUTIES: ESSENTIAL JOB FUNCTIONS: Conducts psychosocial, and behavioral assessments of program participants and writes professional reports that are accurate and timely. Evaluates the needs of each participant and develops a treatment plan appropriate to the participants goals and level of functioning, mental health or cognitive status and care needs. Participates in daily IDT meetings and is prepared to share necessary information to support the teams efforts in integrated care management. Assists in the care coordination between assisted and skilled facilities on the behalf of participants. Assists with the collection of data on health services and participant outcomes, especially as it related to goals, group progress and any research projects. Coordinates ongoing training programs for both internal and external customers on behavioral health issues, as requested. Responds to previously active participants in moments of crisis, even if they are not currently seeking counseling. Keeps thorough and accurate progress notes and submits them within the specified time frames. Other duties as assigned.
