Schedule: Monday - Friday, 8:30am-5pm Location: 3925 N I10 Service Rd W STE 201c Metairie, LA 70002 Position Summary: The LPN is responsible for assessing the medical needs of potential and current clients, ensuring that services provided are appropriate and meet their healthcare requirements. Primary Duties include but are not limited to: Develop and update comprehensive care plans. Provide staff training, ensure accurate documentation, and support medication management. Conduct in-home health assessments, administer IM and SC injections, and address medical concerns. Supervise caregivers for proper medication administration, vital sign documentation, and protocol compliance. Manage client medications, coordinate refills, and liaise with pharmacists. Lead caregiver training and evaluate competency in essential medical tasks. Maintain accurate health records and electronic documentation. Communicate effectively with families, caregivers, and team members regarding client needs and updates. Work under RN supervision while operating independently in the field. Experience, Skills and Qualifications: Licensure: Valid State LPN License. Education: High school diploma or GED required. Experience: Minimum 1 year in home care, healthcare, or senior-related industry (preferred). Technical Skills: Proficient with software tools such as Adobe, Outlook, Teams, Word; ability to learn new EMR systems. Communication: Excellent written and verbal communication skills.
