In this HSS Coordinator role, will be an essential element of an Integrated Care Model by relaying the pertinent information about the member needs and advocating for the best possible care available, and ensuring they have the right services to meet their needs.
This is a Field Based position with a Home-Based office.
If you reside within 30-50 miles from Tarrant, Wise, Denton, Parker, Hood, and/or Johnson, Counties in TX, you will have the flexibility to work a Hybrid/Field schedule as you take on some tough challenges.
**Primary Responsibilities:**
+ Assess, plan, and implement care strategies that are individualized by patient and directed toward the most appropriate, least restrictive level of care
+ Identify and initiate referrals for social service programs, including financial, psychosocial, community and state supportive services
+ Manage the care plan throughout the continuum of care as a single point of contact
+ Communicate with all stakeholders the required health-related information to ensure quality coordinated care and services are provided expeditiously to all members
+ Advocate for patients and families as needed to ensure the patient's needs and choices are fully represented and supported by the health care team
+ Visit members in their homes and/or other settings including community centers, hospitals, nursing facilities or providers' offices

**Required Qualifications:**
+ Must meet one of the following:
+ High School Diploma/GED or higher with 5+ years of experience working with ABD/SSI population within a clinical health care role
+ Associate's degree with 3+ years of clinical experience
+ Unrestricted LPN/LVN license in the state of Texas with 3+ years in clinical experience
+ CHW certification with 3+ years of clinical experience
+ Demonstrated familiarity with Microsoft Office, including Word, Excel, and Outlook
+ Driver's License, access reliable transportation and the ability to travel in this 'assigned area' to visit Medicaid members in their homes and/or other settings, including community centers, hospitals, nursing facilities or providers' offices
+ High speed internet in residence
+ Reside within 30-50 miles from Tarrant, Wise, Denton, Parker, Hood, and/or Johnson Counties, TX
**Preferred Qualifications:**
+ Unrestricted LPN/LVN license in the state of Texas
+ Ability to create, edit, save and send documents, spreadsheets and emails

**Primary Responsibilities:**
+ Provides exceptional customer service to all consumers and members of the clinic staff
+ Fills prescription orders and makes them available for verification under direct supervision of the Registered Pharmacist
+ Orders, receives and stores incoming pharmacy supplies
+ Receives and processes wholesaler medication orders
+ Verifies medication stock and enters data in computer to maintain inventory records
+ Works with the Pharmacist to asset in the pharmacy functions and keeping the pharmacy in compliance with all federal and state requirements
+ Performs various clerical duties relating to the department
+ Communicates with strong professional verbal and written communication skills
+ Other duties as assigned
