CORE JOB RESPONSIBILITIES:
Tuesday - Saturday start time between 12am-2am
3rd shift at least one weekend day.
Load and secure product from the Medline Facility into a truck.
Safely operate a Class A vehicle between the Medline Facility and multiple customer destinations on a delivery route. Abide by Medline's safety regulations, including wearing safety equipment where needed, performing safety checks, etc.
Unload and deliver product at our customer facilities, ensuring timely deliveries.
Provide in-person support to the customer - addressing their questions and needs and providing appropriate documentation upon delivery.
Communicate with dispatching team regarding necessary adjustments to delivery route or schedule in order to meet customer needs and maintain safe operations.
Maintain a travel and cargo log in accordance with Federal and State regulations and company policy.

GLFHC is currently seeking a Mobile Health Unit Driver / Patient Service Rep. The function of the Mobile Health Unit (MHU) Driver / Patient Service Rep is to safely operate and drive GLFHC MHU to and from assigned destinations within the Merrimack Valley as determined by the Mobile Health Unit Manager. This position also provides an array of onsite and community (mobile) based harm reduction and screening services including but not limited to street and venue based outreach, prevention counseling, overdose education and prevention, Narcan enrollment and distribution and Syringe Services to priority population members defined by the Massachusetts Department of Public Health Office of HIV/AIDS (MDPH, OHA), as those at high risk for acquiring or transmitting HIV, sexually transmitted infections (STI's) and/or viral Hepatitis (MSM), active substance users, injection drug users (IDUs), people living with HIV/AIDS and/or Hepatitis C, sex workers and other high risk populations.
Operate company vehicles in a safe and professional manner complying with federal, state and local laws.
Load, unload, prepare, inspect and operate pickup truck with 32ft trailer attached.
Safely follow all local and state driving laws and road regulations, including but not limited to, driving within posted speed limits.
Perform maintenance of MHU, including replenishment of supplies, refilling gas, checking tire air pressure, and cleaning the unit on a regular basis.
Complete activity, travel, and mileage logs and reports on a daily basis.
Schedules unit for preventative and maintenance in accordance with agency prescribed schedules.
Arranges transportation to detox facilities.
Conducts onsite and community based education sessions on overdose prevention.
Conducts individual and group overdose prevention counseling and trainings session to IDU, community members and services providers.
Recruits people who inject drugs (PWID).
Follows established GLFHC and MDPH protocols including but not limited to safe handling, storing and disposal of syringes.
Educates clients on safer drug use and injecting, the transmission of infection and how to minimize the risk of infection.
Qualifications
Comprehensive knowledge, familiarity and comfort with marginalized populations such as (active substance users, injection drug users (IDUs), people living with HIV/AIDS and/or Hepatitis C, sex workers and other high risk populations) and the Latino MSM community and understand the most effective ways to conduct outreach and bring them into services.
Must have valid driver's license and reliable transportation. Vehicle must be insured.
Satisfactory completion of the background check including Motor Vehicle Report (MVR).

SECONDARY FUNCTIONS:
* Assesses patients' barrier's to women's preventative health services via an assessment tool that includes screening for non-medical service needs provided by the program.
* Educates patients on the available services' standard educational materials.
* Tracks patient follow-ups to monitor progress in completing needed services.
* Collects data to evaluate the primary outcomes of patient navigation, including patient adherence to service recommendations and patients lost to follow-up.
* Keep meticulous records including number of patients served, referrals, outcomes and any other activity conducted.
* Establish and maintain contact with patients to provide referrals and conduct follow-ups.
* Ability to work in a variety of settings with culturally-diverse families and communities with the ability to be culturally sensitive and appropriate.
* Continuously expand knowledge and understanding of community needs, resources, services and programs available.
* Maintain confidentiality per HIPAA guidelines in regards to patient information.

MINIMUM QUALIFICATIONS:
