POSITION OVERVIEW Responsible for wide range of case management duties assisting applicants and referents to Greenhope's residential and outpatient services. Will admit new clients and assist medical staff as needed with admission documents. KEY ESSENTIAL FUNCTIONS Primary liaison with internal and external referral sources for residential and outpatient services. Maintains a stream of referrals to ensure residential bed and outpatient slots are covered on a weekly basis. Complete preliminary telephone or face-to-face screening to determine eligibility for services as delineated by contractual requirements via electronic health record system.. Conducts screening interview and complete LOCADTR, WNRA, and other assessment documents. Obtain detailed demographic information on client criminal, family, substance, mental health and other relevant data. Answers all inquiries from potential program clients, returning all calls within 24-48 hours. Maintain a calendar noting work schedule to be shared with managers, receptionist, outreach staff and program managers. Help to develop partnerships and linkages with all referral sources (hospitals, community agencies, crisis system, corrections, family, and other community partners) via phone and letters. Schedule recruitment activities for outreach staff. Input new and updated referral sources in AWARDS Electronic Health Record system. Receives and coordinates all referrals to include notification of insurance providers for prior authorization and services covered. Coordinate with the Benefits department to ensure client financial status via Medicaid and other managed care funding source. May need to review financial information as needed to determine ability to pay for services. Meet with residential clients within discharge phase to refer to Outpatient aftercare services Along with department manager, maintains all program waiting lists, admissions, discharges and updates all lists on an on-going basis. Maintains weekly/monthly contact with referring case managers and other providers. Conduct induction process with residential clients and deliver to primary counselor. Coordinate with department manager to have active list of primary counselor assignments. Distribute email notification of pending admission and actual admission report in a timely manner to include chart number, name, children and any other pertinent documents. As needed, participate in clinical staff meetings and case conferences as necessary to ensure good coordination and continuity of care. Receive regular weekly supervision from Department Manager. Other duties as provided by supervisor REQUIREMENTS The competitive candidate will have a High School Diploma or GED with a minimum of five (5) years' experience in the health and social service field. Must have a CASAC-T or be a qualified health professional. Knowledge of DSM-IV diagnoses, substance abuse, and recovery Knowledge of state and local behavioral health and related services Ability to communicate effectively, both orally and in writing Ability to work independently and with others and on a team Ability to maintain confidentiality of information Ability to work under pressure and handle issues which develop unexpectedly Pass criminal and child abuse background check WHY JOIN US? Develop a housing procurement, financial and self-sufficiency case plan with clients: this shall include intake interview, determine client goals, needs, and eligibility. KEY ESSENTIAL FUNCTIONS Assesses housing needs of client upon referral from other providers. Develops and maintains housing resources and assists clients in finding permanent, affordable housing. Facilitates the prioritization of project-eligible individuals for participation in the project. Procures and places participants in affordable housing and determine participant income resources that could offset subsidy amount. Works in partnership with HUB's housing offices and under the direction of the Housing Services Director, manage the timely payment of subsidy funds for apartment rental, utility payment one-time payments for furnishings. Maintains documentation of client progress in case records according to all contractual & regulatory standards. POSITION OVERVIEW The Entitlements Specialist is responsible for assisting Services for the Underserved consumers with entitlements and ensures that consumer's benefits are maintained for eligibility within the outpatient programs. This individual will also assist consumers through the eligibility process for the appropriate benefits. KEY ESSENTIAL FUNCTIONS Establish linkages and maintain solid working relationships with local Social Security Office and income maintenance centers. Work with staff to determine if clients are in receipt of all entitlements, they may be eligible for. Assist & Support staff in applying for, re-certifying and/or trouble shooting with City, State and Federal entitlement programs. Provide training to staff on various Entitlement Programs. Directly work with clients and staff in difficult to resolve entitlement issues. Conduct workshops/presentations for clients and staff in Entitlements Programs. Prepare and assist consumers with service needs and interview process. Manage crisis intervention services and addresses all emergency situations as appropriate. Participates in professional development activities such as in-service trainings, seminars, webinars, and conferences as needed. REQUIREMENTS Associate/BA Degree preferred. POSITION OVERVIEW: We are seeking Housing Specialists who will be responsible for permanent housing placement for all housing-ready individuals in the shelter. The Housing Specialist will have an assigned caseload and will be responsible for the successful placement of these individuals by NYC Department of Homeless Services (DHS) contractual requirements. The Housing Specialist works with community organizations, government agencies, and property owners to secure living arrangements, and negotiate rents, deposits, and move-in terms on behalf of their clients. The position involves various tasks that increase housing options for people who are trying to transition to permanent housing. Salary for the Housing Specialists is $40,000 to $45,000/Annually based on relevant experience. KEY FUNCTIONS: Assisting individuals in shelters/transitional housing with finding suitable permanent residence. Developing and maintaining liaison with landlords and the community. Helping individuals with application/process for Section 8 and NYCHA. Assisting individuals through the entire housing process from application, meeting prospective landlords, lease signing and moving. Working directly with realtors, landlords and Social Services Department to expedite lease signing process. Inspecting apartments, checking status of Public Assistance Cases. Preparing monthly reports. Conducting initial intakes and initial independent living plans. EDUCATION AND REQUIREMENTS: Associate's Degree required.
