POSITION OVERVIEW:
Under the supervision of the Director of Nursing, the RN Manager is responsible for overseeing the assessment, coordination, implementation, and management of the care and services of residents using the standards and guidelines set forth by the Department of Health, NYS Board of Nursing, and Casa Promesa. The RN Manager is as a member of the interdisciplinary team and provides support and information (verbal/written) to leadership about facility operations. The Nursing Supervisor Coordinates facility operations in the absence of facility leadership. Follows all infection control protocols. Follows attendance, dress code, and scheduling guidelines. (9am-5pm M-F).
KEY ESSENTIALS:
Complete admissions assessments and reassessment on all patients.
Support and promote the philosophy, objective, policies and procedures of Casa Promesa.
Responsible for providing leadership and supervision to the nursing staff, housekeeping, and security.
Works closely with medical support staff to ensure resident needs are met.
Will greet admitted patients and prepare them for physical examination and treatment and document admissions for Director of Nursing.
Will maintain medical supplies and equipment and work closely with Unit Clerk to ensure par levels.
Provides education as indicated after assessing the learning needs of the patient to staff members.
Assess the patient's physical and psycho social condition on an ongoing basisthroughout the continuum of care. Must alert Director of Nursing and Medical Director of all changes.
Utilize manual and automated information systems to improve efficiency and effect productivity. Accurately records patient information in the electronic medical records system and patient medical record. Documentation must always be in compliance.
Demonstrate a current working knowledge of equipment, instrumentation and medications.
RN may be directed to give out medications to patients if required.
All nurses will utilize the five rights of medication administration. They are:
Right Patient
Right Time
Right Drug
Right Dose
Right Route
Must prepare Nursing 24-Hour Report.
Adhere to the standards, policies, procedures and goals of the medical center.
Must participate in all trainings as required and provide trainings to LPN's and CNA's on the units.
Act as a liaison in coordinating patient care activities with nurses, physicians and allied health professionals.
Participate in the preparation and facilitation of regulatory agency site visits to include DOH.
In accordance with agency protocol and DOH standards, maintains adequate infection control standards.
Review and report all medical, accident/incidents related to patient care and provides input into actions necessary to resolve problems.
Participate in weekly and on an “as needed basis” case conferencing sessions with interdisciplinary team members to discuss resident issues/concerns.
Manage Petty Cash funds appropriately to meet resident needs.
Must process payroll for his/her unit each week and work closely with the Staff Coordinator to achieve compliance.
Must provide scheduling assistance to find coverage. At times, RN may be mandated to stay on duty.
Must provide annual evaluations to all LPN's and CNA's on the assigned unit.
Must ensure methadone and narcotic logs are up to date daily.
Must conduct COVID-19, Influenza, RSV and any other testing for both staff and residents as directed.
REQUIREMENTS:
Bachelor of Science of Nursing required.
1-3 years of nursing experience preferred.
Current and unrestricted licensure in New York State as a Registered Professional Nurse.
Current and valid, CPR (BLS)/AED Certification.
Current Infection Control certification (approved by
NYS, valid for 4 years, 4.0 hours).
Knowledge of Substance Use, HIV/AIDS, Geriatrics and Mental Illness required.

KEY ESSENTIAL FUNCTIONS

Under supervision of the OASAS Program Nursing Director, the Nurse Supervisor is responsible for coordinating and delivering patient care utilizing the nursing process. Ensures that the findings of the nursing assessment, the comprehensive care plan, and recommendations for nursing follow-up are documented, dated, and signed in the client's medical record.

Prepares admission and discharge forms for residents to and from the hospital in a timely fashion.
Admits new residents and completes admission forms and nurses notes; orients residents to new environment.
Documents pertinent information regarding resident conditions as per policy/procedure.
Utilizes the Electronic Medical Record (EMR) to Maintains accurate documentation of all nursing activities.
The LPN will participate in all mandatory training internal/external.
The LPN will complete incident reports as needed. The incident reports be written clearly, factually, and timely.
The LPN will be able to start IV's after receiving facility training.
Complies with departmental policies regarding attendance, work schedules, and dress code.
Follows all infections control protocols.

POSITION OVERVIEW:
The Registered Nurse (RN) is accountable for evaluating consumer's needs for nursing care on an ongoing basis. The RN is responsible for the physical health of Intensive Mobile Treatment (IMT) consumers. The functions below outline the responsibilities associated with this position. These functions are considered critical to effective department operations and Acacia's mission. Satisfactory attendance and performance of functions in a competent, cooperative, and timely manner is required.
KEY ESSENTIAL:
• Conduct initial health screenings for IMT consumers.
• Provide referrals as needed to primary and/or specialty medical care.
• Conduct blood draws and administer psychotropic medication treatment, including administration of IV medications.
• Prepare medication to be dispensed, and work closely with the Psychiatric Nurse Practitioner to dispense medication and monitor clients' physical and mental wellbeing.
• Conduct care coordination between other IMT staff members, pharmacy, health homes, and other outside providers • Record patient data in Promesa's NextGen Electronic Health Record system.
• Participate in grant related workshops and trainings.
• Must be able to deliver in-person services after standard hours of operation as needed.
• Perform clinical, compliance, and administrative duties as instructed.
• Enhance patient's health and self- management skills as developing wellness goals in the use of evidence-based practices and maintain record of these activities.
• Participate in daily IMT staff and organizational meetings.
• Practice according to the principles of recovery oriented and trauma informed service-delivery.
REQUIREMENTS:
• Must hold a NYS-issued RN license
• Previous relevant experience in a mental or substance use disorders required, experience with individuals experiencing homelessness a plus.

POSITION OVERVIEW
The oversight of self-administration of medication is an important part of the care that many individuals receive in QVCMH for JCAP, Inc. residential facilities. Patient safety is the number one priority in the delivery of healthcare. The LPN oversees Medication Administration, monitors client health, and collaborates with program staff around client treatment.
KEY ESSENTIAL FUNCTIONS
Facilitate the monitoring of client's self-administration of medication
Appropriately document all medication taken by residents
Monitor clients for any adverse reactions to medications and take appropriate actions, and report as necessary
Accept and document deliveries of medication
Perform daily medication reconciliations to verify counts of all clients' medication
Comply with all medication policies and procedures
Secure, monitor, and distribute controlled medications, in compliance with all agency, city, state, and DEA regulations
Coordinate with other nurses and staff to coordinate patient care and medication
Document and share all non-compliance with medication regiments
Appropriately discard medication as required, do so and documenting in accordance with all policies and regulations
Protect client confidentiality except in cases to coordinate care with staff
Conduct groups with residents the promote medication education and medication compliance
Coordinate processes to improve/evaluate medication administration. Makes recommendations.
Assist with referring clients to appropriate health care providers/specialists.
Coordinates with the client's primary care physicians to inform decisions regarding management and treatment of problems identified and provides care as prescribed by a physician.
Assist with the management of client prescriptions with medical providers and various pharmacies, with focus on Evers Pharmacy.
Provide oversight and monitoring to promote organized and efficient medication tracking, refills and disposal of medication, as needed.
Oversee compliance with rules for documentation of medication administration
Identify when a person's response to a medication or change in condition requires medical review and possible intervention. Notify medical providers of this observation
Consult with Medical Director/Provider if a self-directed individual appears to no longer be capable of taking or understanding their medication
Collaborate and coordinate with 819 provider staff and provide reports as requested
Participate in client clinical review meetings and coordination of care meetings as requested
Monitor and document clients' vital signs and additional vitals as needed to ensure client safety i.e., sugar levels, blood oxygen levels, etc.
Promptly investigate medication errors based on severity index/potential severity in order to improve staff performance and patient care. Work with Medical Director to identify and when required, to report errors to proper authorities.
Participate in meetings with staff and vendors that work to improve policies and practices
Serve as a resource person by consulting with agency staff
Performs other tasks as assigned by Supervisor/s.
