Overview
Memorial Hospital Belleville is an acute care hospital offering medical and surgical services plus critical and emergency care including Children's at Memorial for pediatric emergency care. It provides patients a full complement of diagnostic and treatment services as well as heart and vascular care. Memorial, offering medical and surgical services plus critical care, is an accredited Chest Pain Center with PCI by the Society of Cardiovascular Patient Care and is designated as an Acute Stroke Ready Hospital by the Illinois Department of Public Health. In addition, Memorial Belleville recently was accredited by the American College of Radiology as a designated Lung Cancer Screening Center. Since 2008, it has been designated as a Magnet -recognized organization for nursing excellence by the American Nurses Credentialing Center.
Memorial Hospital Shiloh, a 94-bed, all-private suite hospital was recognized with Magnet status in 2018 and provides emergency care, labor & delivery, nursery, medical and surgical services plus critical care.

Role Purpose
Performs diagnostic imaging procedures in at least one advanced imaging modality (defined as Computed Tomography (CT), Magnetic Resonance Imaging (MRI), Interventional, Mammography, Bone Density, Cardiac Catheterization) on adults and/or pediatric patients for the purpose of diagnosis and/or treatment, and quality control.

What You'll Do:
Maintains and updates reports and registries for the Patient Care Health Home (PCHH) team. Reviews registry data and ensures proper compliance with established workflows and procedures.
Completes audits for invasive procedures and in-house labs. Audits other PCHH data and initiatives. Tracks all audits, providing documentation of audits as requested.

Manages various work queues daily to ensure timely processing and completion of referrals.

Sets up appointments where applicable and coordinates follow up between referral source and patient.

About the Position
We have an opening for a Supportive Housing Associate at our Rogue Ridge Apartments in Ashland, Oregon. The Supportive Housing Associate motivates adults with serious mental illness to participate in a variety of skills training and other treatment activities. Primary responsibilities include facilitating access to community resources and assisting tenants with health care arrangements, housing applications, move-in arrangements, medication administration, money management, symptom monitoring, and housekeeping. The Supportive Housing Associate encourages socialization, self-help, communication, and gives daily support for problem solving. This Supportive Housing Associate position requires that you demonstrate empathy and active listening. In addition, you will receive crisis de-escalation training, which requires performing various physical, hands-on maneuvers and techniques.
If you have experience working as a Qualified Mental Health Associate (QMHA), Direct Support Professional (DSP), Residential Assistant (RA), Residential Counselor, Skills Trainer, or Caregiving for a client with behavioral health needs, then the Supportive Housing Associate position may be a great fit for you!
Vocational Training Program
For applicants starting as a QMHA-R, ColumbiaCare offers a Vocational Training Program to accelerate the training, on-the-job experiential learning, and testing preparation for qualified employees to achieve the 1000 supervised hours required to complete their certification as a QMHA-I. Through this program, this certification can be achieved in as little as 6 months. This is an optional program.
Work Schedule: Thursday and Friday, 8:00am - 12:00am (16 hour shift, sleep onsite) (Part Time, Day & Overnight). An additional $75 bonus will be included on the nights that you sleep on-site.
What You'll Make
$22.00 - $23.75 per hour DOE/Credentials. An additional $2.50 per hour night shift differential will apply during the work hours of 10:00pm - 6:00am.

Rogue Ridge Apartments is a brand-new, 28-unit, permanent Supportive Housing program in Ashland, Oregon. Rogue Ridge serves adults who are houseless and living with a mental illness. The program is in a beautiful three-story apartment complex with elevators, ADA units, and a community/activity room for residents to enjoy. Supportive services include on-site staff who provide peer support, skills training, tenancy assistance, community navigation, and more; 24/7 access to housing support; and tenancy services and support around move in and out, rent collection, maintenance, and repairs.
ColumbiaCare Services is a qualifying employer for the Public Service Loan Forgiveness (PSLF) Program.
What You'll Need
Must be eligible for Qualified Mental Health Associate (QMHA) registration. Eligibility requires 1 out of 3 Criteria below:
Bachelor's degree in Behavioral Science: Psychology, Counseling, Social Work, Human Development, or related field;
OR minimum three (3) years of verifiable behavioral health experience working with individuals with behavioral health needs in a supervised setting;
OR equivalent combination of behavioral science education in addition to satisfactory supplemental behavioral health experience and training
If not already registered as a QMHA through the Mental Health & Addiction Certification Board of Oregon (MHACBO), we will assist you in registering within 30 days of hire.

3. Monitors inventory and brings related issues forward for prompt resolution. Assists with cost analysis models.
4. Assists with basic personnel oversight including participation in selection, training, developing, and providing coaching under the direction of the Supervisor or appropriate Director. Assists in new employee orientation, training and competency. Actively involved in implementing and ensuring staff completion of competency program requirements. Self-directed and motivated to seek out opportunities to contribute to projects to help the department. Coordinates and is active in at least two special projects per year within the department or SQL/LSA System, along with other miscellaneous department duties.

As our Patient Support Associate, here's what you can expect:
Takes accurate attendance and tracks patient participation at scheduled activities.
Is familiar with patient schedules for all programs.
Knows the campus layout, including rooms, offices, and amenities.
Performs scheduled rounds of the campus.
Documents patient lateness and absences as required.
Safely searches new patient luggage using proper protocols and PPE.
Follows “Items to be Held” policy and maintains confiscated items accurately.
Inputs/searches items in Personal Effects Form and labels items properly.
Provides tours and orientation for new admissions.
Reviews and answers questions about the Patient Handbook.
Effectively addresses behavioral issues, documenting concerns in shift reports.
Communicates important psychiatric and behavioral information to care teams.
Facilitates and documents clinical groups and interventions as needed.
Assists with AV equipment, documentation, and strategic department meetings.
Participates in Crisis Prevention and clinical training sessions.
Maintains safety, security procedures, and supports in emergency drills.
Operates golf carts for patient transport and practices proper radio etiquette.
Follows standard operating procedures and department policies.
Participates in interdisciplinary team functions and ongoing education for clinical roles.

High School diploma or GED required; Associates or Bachelor's Degree in Human Services preferred.
A valid driver's license with clean driving record required
CPR/BLS certification required
Prior experience working in SUD or behavioral health setting strongly preferred
Recovery Coach and/or CPRS certification preferred
Crisis Intervention/de-escalation training/experience preferred
Ability to maintain calm within the patient community during any emergency or drill
Working knowledge and acceptance of multiple pathways to recovery (12-step, Peer Recovery, SMART, etc.)
Demonstrates current, accurate knowledge of trends and issues related to recovery and wellness
Ability to sets clear boundaries with patients
Speaks and acts in an ethical manner
Excellent customer service skills

Open Sky's Support & Stabilization Services provide an array of services to support youth up to the age of 22 years and families that may be experiencing emotional and/or behavioral challenges. Services provided include groups, therapeutic services to help children remain in their home, preparation support for children reunifying with family, permanency support, skill building, increasing social connections, and providing families with community resources to meet basic needs. The goal of the program is to provide families with access to services that are tailored to their individualized strengths and needs which gives youth the greatest chance for safety, permanency, and well-being.
The Youth Support Specialist will be responsible for helping individuals develop the skills needed for self-management, including crisis and prevention planning, social skill enhancement, daily living skill education, stress management, education about mental illness and symptoms and self-reinforcement and contingency management while monitoring their progress toward achieving identified goals.
Other Key Responsibilities:
Assist individuals in identifying individual strengths and interests.
Maintain files, documentation and meet deadlines for S&S Services and Rehab Option and all other licensing guidelines
Transport individuals in a safe manner in matters related to service delivery
Assist in providing continuity of service delivery to youth as needed
Qualifications
Bachelor's degree in social services or related field required or five years of experience working with youth and families required.
Valid driver's license, acceptable driving history and reliable transportation, required.
Bilingual candidates and those with lived experience highly preferred.
Reliability, willingness to learn, and being open to new opportunities.
