Support Services Representative Responsibilities:
Processes and communicates authorization and schedule changes to ensure coordination of service and client satisfaction.
Coordinates utilization of Care Partner teams to ensure fill rate expectations met. Researches and resolves payroll and billing questions under direction from the Client Service Manager.
Functions as a liaison for Field Supervisors in communication with payer case management teams. Supports recruiting and onboarding needs of team.
Responsible for scheduling and monitoring/reporting all required client visits by field supervisor. Assists Client Service Manager in the presentation of staff training related to program requirements for newly hired and current office employees.
Complies with applicable legal requirements, policies and procedures including, but not limited to the Compliance Program: Code of Conduct, HIPPAA and Documentation Standards.

What we ask of you:
Prepare for examinations by checking equipment and restocking supplies, reviewing patient schedules, reading physician orders and entering patient data accordingly.
Perform cardiac ultrasound exams and analyze data of 2D, 3D, M-Mode and Doppler data, and assist with administering contrast agents and bubbles for exams.
Obtain quality images by positioning transducer, monitoring display screen and listening to signals, adjusting beam strength and focus.
Perform exercise and pharmacologic stress echocardiograms and assist cardiologist with transesphageal echocardiograms.
Prepare patients and reduce patient anxieties by explaining procedures and answering questions.
Provide training and support to echo students during clinical internships.
Responsible for off premise on-call hours on nights and weekends.
All other duties as assigned.
What we require of you:
Graduate from a school of Ultrasound and/or Radiologic Technology, along with completion of a clinical rotation in Radiology and/or Untrasound prior to graduation.
RDMS certified or ARDMS Registry Eligible.
CPR/BLS certification required through ARC or AHA.
At least 1 year of experience performing adult echo exams/ultrasounds. Stress echo experience preferred.
Familiarity and compliance with matters of law, reglulations and internal policies affecting the employment relationship.
Comfort with ultrasound equipment and imaging software, Echo/Bicycle Beds, EMR software.

Access use and ability to disclose personal health information (“PHI”) necessary to fulfill the above duties and responsibilities and as defined by each organization.
Licensure, Minimum Education & Experience Requirements:
Licensed, registered or certified as a physical therapist Assistant by the state(s) in which he or she will provide services, as applicable per the state requirements.
Graduate of 2 year college-level program approved by the APTA or CAPTE.
In the last five (5) years, two (2) years of experience as a PTA that is relevant to the patient population assigned

What You'll Do
As the Client Service Coordinator in home care, you will be responsible for monitoring and supporting the daily activities of services being provided to clients in accordance with state regulations and company policy, while ensuring quality coordination of service for our clients and Care Partners.
Other Client Service Coordinator responsibilities include:
Functions as primary support for daily EVV and payroll functions
Processes and communicates authorization and schedule changes to ensure coordination of service and client satisfaction
Coordinates utilization of Care Partner teams to ensure fill rate expectations met
Researches and resolves payroll and billing questions under direction from the Client Service Manager.
Functions as liaison for Field Supervisors in communicating with payer Case Management teams
Supports recruiting and onboarding needs of team
Responsible for scheduling and monitoring / reporting all required client visits by field supervisor
Assists Client Service Manager in the presentation of staff training related to program requirements for newly hired and current office employees

Where applicable, reviews Eligibility Alerts and Responses for Medicare Clients.
Requests, H&P, Discharge Summary, and other documents needed for ICD 10 coding by the CPOCC team, if not received with the referral information.

Be the Best Home Health LVN/LPN You Can Be
If you meet these qualifications, we want to meet you!
One year of clinical experience and one year of home health experience
Ability to provide nursing visits according to the treatment as prescribed by the physician and provide and maintain paperwork to maintain accurate and complete medical records
Required Certifications and Licensures:
Graduate of an accredited practical or vocational nursing program
Currently Licensed Practical Nurse or Licensed Vocational Nurse in the state of agency operation
Must possess and maintain valid CPR certification while employed in a clinical role
Must be a licensed driver who can travel to all business locations

If you meet these qualifications, we want to meet you!
At least one year experience as a Home Health Aide and successful completion of competency evaluation
Ability to perform tasks involving physical activity, including heavy lifting, extensive bending, and standing
Can document observations and provision of care according to policies and procedures
Required Certifications and Licensures:
Current Home Health Aide/Nurse Aide Certificate meeting HCFA requirements, if required by the state where employed
Must possess and maintain valid CPR certification while employed in a clinical role
Must be a licensed driver who can travel to all business locations
