BAYADA Home Health Care is currently seeking an experienced RN or Social Worker to fill the position of a Care Integration Manager to support our hospice teams in Arizona. Care Integration Managers will manage, and support the transition of complex, chronically ill clients between BAYADA Home Health and Hospice practices, using data to proactively advocate for and coordinate the utilization of the most appropriate benefit at the right time. Qualifications of the Care Integration Manager:
Four (4) year college degree.
Be a licensed Registered Nurse or Social Worker.
Five (5) years' experience in nursing, social work, care coordination, and care integration in a relative specialty (home health, hospice, etc.)
Excellent verbal and written communication skills.
Ability to work seamlessly across settings and practice lines in a highly matrixed local environment.
Demonstrated record of strong interpersonal and organizational skills
Competence in basic PC skills required to perform job functions. (Word, Excel, SharePoint, Outlook)
Acceptable pre-employment assessment results.
Ability to read, write and effectively communicate in English.
Responsibilities of the Care Integration Manager:
Develop working knowledge of BAYADA's mission, services, people, organization, policies and procedures.
Develop a yearly care integration plan and set strategic quarterly goals.
Interact with referral sources to facilitate transition of BAYADA clients across the continuum of care and foster positive long-term relationships to represent BAYADA as the preferred home health/hospice provider for complex, chronically ill patients in late life.
In collaboration with the client's physician, educate and engage prospective clients and their families about hospice and prepare them for the transition to end-of-life care.
Promote effective care coordination between BAYADA Home Health and Hospice and communicate clinical status and staffing needs to each service office.
Manage routine Care Integration touchpoints utilizing the Care Integration Dashboard to proactively identify Hospice needs.
Collaborate with Directors in achieving cross-practice referral goals.
Demonstrate solid performance or exceed performance standards in key job dimensions/attributes defined on the Performance Appraisal for Office Staff.
Perform related duties, or as required or requested by supervisor.
Coordinate referral documentation, face-to-face program explanations, and in-services needed for Hospice admission or Home Health referral.
Perform cross-practice educational in-services and updates for Home Health and Hospice field and office staff

Provide home-based physical therapy to patients who have lost mobility, strength, and functionality due to illness or injury
Work as part of a home health team, which may include an RN, LPN, CNA, HHA, OT, and SLP, focused on the patient's plan of care and goals
Act at the direction and under the supervision of the treating Physical Therapist and in accordance with the plan of care

BAYADA Home Health Care is currently seeking an energetic and experienced Licensed Medical Social Worker, MSW, for a per diem opportunity performing home health visits our OF. Arizona Tucson Hospice office this office services clients on a per visit basis in territories throughout Pima County.
Prior home care experience preferred, but not required. As a Medical Social Worker, you will be an integral member of a multi-disciplinary health care team that provides skilled nursing and rehabilitative care to clients, affording them the opportunity to receive the medical care required to remain at home.
Responsibilities include:
* Making home visits to clients in designated geographic territories.
* Assist our team in understanding the social and emotional factors related to our clients' health problems.
* Provide clinical social evaluations and plan appropriate interventions based on evaluation findings.
* Identify appropriate community resource referrals to address the client's and the family's practical and environmental needs.
* Act as an advocate for both the client and the family to navigate the patient through the community system.

Qualifications for the ideal candidate:
* Must be licensed by the Arizona Board of Social Work Examiners at the Master (LMSW) or Independent (LISW) level.
* A minimum of a Masters in Social Work, with at least 2 years of community-based experience.
* Accreditation by the Council on Social Work Education, as demonstrated by school transcript or degree.
* Home care experience a plus.
* Demonstrated ability to read, write, and effectively communicate in English.
* Ability to work independently and manage time effectively.
* Strong interpersonal skills.
* Solid computer skills; prior experience with electronic medical records (EMR) preferred.
* Ability to travel to cases, as assigned.

As a Clinical Manager / Quality Assurance (RN), here's a big-picture view of what you'll do:
Assure ongoing interdisciplinary assessment of the patient.
Ensure our home care clinicians meet and exceed standards for safe, effective, and efficient clinical practice through support and oversight of our home care staff in accordance with all applicable federal and state laws, rules, and regulations.
Supervise paraprofessional and licensed professional team members.

BAYADA Home Health Care is currently seeking an experienced Physical Therapist Assistant, PTA, for a PRN opportunity performing home health visits for our OFFICE. This office services adult and geriatric patients on a per visit basis in territories throughout the North Tucson area.
As a home care Physical Therapist Assistant (PTA) you will assist clients in attaining or maintaining optimal physical well being by providing treatment, tests, client education and appropriate interventions as delegated by a licensed Physical Therapist.
Each Physical Therapist Assistant (PTA) must:
Have current licensure or certification in the state of AZ
Have graduated from a two-year college level program approved by the American Physical Therapy Association, as indicated by school transcript or diploma, or
Have two years of appropriate experience as an Physical Therapist Assistant (PTA) and a satisfactory grade on a proficiency exam conducted, approved or sponsored by the U.S. Public Health Service when initial qualification occurred before December 31, 1977
Have at least one year's work experience under the supervision of a qualified Physical Therapist.

As a Home Health Speech Language Pathologist, here's a big-picture view of what you'll do:
Provide home-based speech therapy to patients having communication, swallowing or cognitive difficulties due to an illness or injury
Work as part of a home health team which may include an RN, LPN, CNA, HHA, PT and OT, focused on the patient's plan of care and established goals
Observe and evaluate a patient's ability to communicate, swallow and process information
Conduct patient assessments, document progress and ensure patient goals are met
Assist with mouth, breathing and swallowing exercises, fluency, articulation and reading
Educate patient and family on plan of care, therapy goals, safety practices and self-care
A few must-haves for Home Health Speech Language Pathologists:
Master's or Doctoral degree in Speech Language Pathology and active SLP license in Arizona
Minimum of one (1) year of speech therapy experience, ideally in home healthcare
CPR Certification (demonstration course required)
Valid Arizona Fingerprint Card
No Covid Vaccination Required
Pass a background check and drug test (we are required to test for THC)
Knowledge of state and federal home health regulations
Good clinical judgement, critical thinking skills, strong communicator, empathy and compassion
