Care Integration Managers will manage, and support the transition of complex, chronically ill clients between BAYADA Home Health and Hospice practices, using data to proactively advocate for and coordinate the utilization of the most appropriate benefit at the right time. Qualifications of the Care Integration Manager: Four (4) year college degree. Be a licensed Registered Nurse or Social Worker.
