ACMH Hospital company history timeline

1952

Audubon County Memorial Hospital was established as a 29-bed acute care county owned hospital in 1952 to serve the residents of Audubon County and surrounding areas.

1973

ACMH, Inc. is a 501 (c) (3) not-for-profit corporation founded in 1973.

1979

ACMH has operated housing programs with a rehabilitative focus which are licensed by the New York State Office of Mental Health (OMH) since 1979.

1993

Since 1993, ACMH has also operated permanent supportive housing programs which provide both the financial subsidy and case management support that many persons with serious mental illness need to access and maintain housing in the community.

1998

In 1998, to meet the needs of the Audubon County residents, a major inpatient addition to the facility opened for service.

2000

In the year 2000 the hospital was designated by the state of Iowa as a “necessary provider” making it eligible to become a critical access hospital.

2001

The hospital converted to a 17-bed critical access hospital in December 2001.

2002

Beginning in 2002, ACMH began providing targeted case management services to the community at large, independent of ACMH operated housing.

2005

In 2005, a two-bay ambulance garage complete with decontamination showers were built.

2011

In 2011, ACMH purchased the medical practice of Doctor James Cunningham which became Audubon Family Healthcare Clinic.

2013

A renovated outpatient clinic opened in the spring of 2013 which allowed residents of Audubon County to access quality care from top specialists around the state.

In 2013, ACMH began providing care management for NYS Medicaid Health Homes, as well as to non-Medicaid recipients under a contract with the NYC Department of Health and Mental Hygiene (DOHMH).

2015

In April 2015, ACMH opened a Short-term Crisis Respite & Step-Down service serving up to 3 guests experiencing a psychiatric crisis or transitioning from a hospital stay.

2016

In April 2016, ACMH began providing Home and Community Based Services (HCBS) on behalf of Medicaid Managed Care Organizations.

2017

In May of 2017 ACMH changed its logo and name to Audubon County Memorial Hospital and Clinics.

2018

In April 2018, this service was expanded to 10 beds.
