The Pharmacy Technician II - Rx Solutions Center (RSC) is responsible for handling member, pharmacy, and provider calls related to pharmacy clinical campaigns. This role will require collaboration across different areas within CareSource to ensure best in class service is delivered to our members.
Essential Functions:
Performs technical and clerical duties that do not require the professional clinical judgement of a pharmacist (i.e., confirms member identity, updates demographic information, updates provider information, faxes providers, etc.)
Receive and respond to incoming calls. Take appropriate action based off protocols & use critical thinking skills to escalate issues to appropriate parties, when needed
Ensure each member has a positive experience by providing accurate information and a high level of customer service

Education and Experience:
High School Diploma or equivalent years relevant work experience is required

About this Role
As a Pharmacy Technician, you will provide virtual (primarily telephonic) support for Medicaid patients. You will partner with Waymark clinical pharmacists to help patients access medications, address barriers to medication non-adherence, access relevant durable medical equipment (DME), and elevate prescription needs to providers. You will report to the Clinical Pharmacist Lead in Illinous state.
You will interact with patients through telehealth modalities (SMS and phone) as part of a broader team that includes community health workers who see patients in person and support patients in addressing social determinants of health, therapists who provide mental health counseling, and care coordinators who work closely with care providers and community-based organizations to support wrap-around services. You will also work closely with the Waymark clinical pharmacist to provide interventions in primary care that help patients overcome challenges gaining access to, and appropriately taking, their medications.
Key Responsibilities
Perform telephonic and SMS outreach to patients and providers/pharmacies to address medication access issues, medication non-adherence or other drug utilization activities to improve patient and quality outcomes. Outreach may also include addressing medication-related and disease control-related HEDIS gaps.
Coordinate patient medication needs with a multidisciplinary team including community health workers (CHW's), care coordinators, and therapists.
Coordinate with external healthcare providers and Waymark care team to provide accurate and timely information on medications.
Facilitate medication access including answering, resolving and triaging inbound inquiries in collaboration with the Clinical Pharmacist Lead.
Facilitate timely responses to internal Waymark requests for pharmacy team assistance by obtaining and documenting medication information and verifying eligibility, formulary, benefits and allergy information as requested by the Waymark Pharmacist Lead.
Communicate with patients and healthcare providers to assist in completing medication prior authorizations, navigate formulary changes, manage side effects, and identify therapeutic substitutions in conjunction with Waymark and primary care teams.
Provide accurate and timely documentation to ensure productivity and quality metrics are met.
Actively participate in innovation with an evolving startup environment by identifying trends, resolving problems, recommending improvements, and implementing process improvement changes, applying observations and recommendations to resolve operational issues, increasing productivity and quality, and achieving excellent customer experience.
Minimum Qualifications
Current Illinois State Pharmacy Technician License in good standing.
PTCB CPhT Certification.
2 or more years experience working in outpatient/primary care/clinic-based, or managed care settings.
Experience with and commitment to improving care for underserved populations.
Ability to collaborate and communicate effectively in a multidisciplinary team setting.
Strong interpersonal communication skills, comfortable with proactively engaging patients telephonically, comfortable reaching out to on-site clinicians, payors, and pharmacies telephonically.

Qualifications

The Position:
The Therapy Care Specialist will work with SoleoRx TCMC model and utilize national and local resources to facilitate high quality care of a patient with specialty pharmacy needs. The Therapy Care Specialist will work under the supervision of a Pharmacist to oversee logistics of a patient's dispense and prescription maintenance, assessment data capture, and serves as the point person to coordinate patient care across all layers of the patient care continuum while on service with Soleo Health. Responsibilities include:
Act as single point of contact for specialty patients and as liaison with Soleo branch personnel to coordinate timely medication dispense and delivery
Works under supervision of specialty pharmacist and/or branch pharmacist to capture assessment data, ascertain patients supply needs, and schedule new and refill medication deliveries
Run daily and weekly pharmacy reports as needed
Answer phones and warm transfer call to appropriate location, monitor incoming faxes, and monitor TCMC refill queues
Send orders to physicians for signature
Work with intake/reimbursement staff to maintain active patient insurance coverage and actively pursue authorizations and re-authorizations with the auth/billing staff at the branch level
Documentation of all patient interaction and care coordination in the designated system of record for clear and consistent communication
Complete therapy specific training as required
