In this role, you will collaborate with a team of mental health clinicians, develop discharge plans, and help connect individuals with critical community-based mental health services upon release from custody. SHIFT INFORMATION:
Generally Monday - Friday, day shift hours
Essential Functions
* Collaborate with a team of mental health clinicians in a supporting role to provide case management and treatment coordination for inmates with mental health needs.
* Assist in the development of discharge plans to ensure inmates have access to appropriate community-based mental health services upon release.
* Participate as an active member of a multi-disciplinary team, addressing the complex needs of inmates with mental health and medical concerns.
* Support the coordination of chronic care services for inmates, ensuring ongoing mental health support.
* Address concerns from inmates and stakeholders, ensuring a supportive and effective system for mental health care.
* Monitor inmates with state commitments and police holds, ensuring proper documentation and coordination of their care and release.
* Perform other duties related to mental health case management as assigned to include reporting statistics/scanning patient paperwork and to work together as a cohesive unit with other members of the Administrative and Case Management team.
Job Requirements
* A bachelor's degree in psychology, social science, criminal justice, sociology, or a related field and/or a mix of relevant education and experience.
* Knowledge of community-based mental health services and local community resources is a plus.
* Experience working with individuals with mental health needs in a correctional or similar environment.

EXAMPLE OF DUTIES:
* Assess and process referrals for child abuse or neglect and conduct assessment and fact-finding interviews to obtain and assess mental, physical, economic, employment, environmental and social status of client/family/caregiver.
* Assess interview information to determine substantiation of referrals and prepare written intake reports.
* Design, organize, implement, and monitor effectiveness of case plans using an array of community resources to resolve issues.
* Facilitate client access to support services and carry a full caseload in child welfare.

The Water Department Night Service Worker responds to emergency calls to the Water Department, assesses the scene of the emergency and determines the appropriate level of response, including calling out crews to perform repairs. As the Night Service Worker, the employee responds to after hours (including weekends and holidays) emergency calls to the Water Department, working independently to assess emergencies and determine the appropriate level of response. The work is performed under the supervision of the Plumbing Supervisor.
The job requires knowledge of methods and techniques of water line installation, maintenance, and repair, operation of power tools and equipment such as dump trucks with trailers, loaders and compactors. The job requires a Class I Drinking Water Distribution Operator Certification.* The job requires the ability to maintain a collaborative and cooperative working relationship with elected and appointed officials, other City employees, other organizations, and the general public.
The work environment is a public street environment that may include exposure to adverse weather conditions, trench excavations, traffic hazards, moving vehicles and heavy equipment at the job site, and hazards involving the use of power tools and equipment. The noise level is generally moderate but with occasional exposure to noisy and/or loud equipment.
The job is an on-call position, responding to emergency situations. Employees designated as immediate
response personnel must live thirty (30) minutes or less from their usual daily work site.
ESSENTIAL DUTIES AND RESPONSIBILITIES
* Responds to emergency calls to the Water Department, assesses the scene of the emergency and determines the appropriate level of response, including calling out crews to perform repairs. State Hospital South (SHS) located in Blackfoot is seeking Clinicians to join our amazing Treatment Team. We provide psychiatric inpatient treatment and skilled nursing care for Idaho's adult citizens with the most serious and persistent mental illnesses. The hospital works in partnership with families and communities to enable clients to return to community living. SHS is a 110-bed Adult Psychiatric Treatment Hospital, and this position will be working on a 30-bed unit within that facility. This position is Monday - Friday, 8:00 am - 5:00 pm with holidays off.
Clinicians are part of the hospital Treatment Team providing evidence-based patient-centered care for adult patients who are committed to the Department of Health and Welfare for care and treatment. You will be working as part of a treatment team that includes a prescriber, other clinicians, nursing, pharmacy, medical, recreational therapy, and others.
This position is not eligible for telework.

EXAMPLE OF DUTIES:
* Perform assessments, evaluations, observations, and interviews to identify strengths and needs.
* Develop treatment plans, recommend services and interventions to the Treatment Team.
* Ongoing case management consultation with members of the Treatment Team, Regional staff, and Families involved with the patient to gather data relevant to assessment, treatment, and discharge planning.
* Perform assessments for patients at risk due to suicidal, homicidal, assaultive, or elopement behaviors and develop a structured intervention immediately to implement in the Treatment Plan.
* Document clinical observations and assessment data in the electronic medical records per hospital policy and procedure
MINIMUM QUALIFICATIONS:
You must possess all the specialty minimum qualifications below to pass the exam for this position. Please make sure your resume or work history supports your meeting the minimum and specialty qualifications for this position. * A Master's degree (or higher) in a direct clinical practice Human Services field is required for this position. (Master's degree in social work, psychology, marriage and family counseling, marriage and family therapy, psychosocial rehabilitation counseling, psychiatric nursing, or very closely related field of study) in accordance with Idaho Codes: 54-3206, 54-3402, 54-3405, and 54-3214.
SPECIALTY MINIMUM QUALIFICATIONS:
* Requires licensure in a professional specialty in one of the following areas: Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW) in accordance with Idaho Code 54-3206, Licensed Professional Counselor (LPC) in accordance with Idaho Code 54-3405, Marriage and Family Therapist (LMFT) in accordance with Idaho Code 54-3405.
