The PT - Physical Therapist provides direct rehab care in physical therapy to patients upon physician referral in accordance with all applicable laws, regulations, and Life Care standards. Education, Experience, and Licensure Requirements Graduate of an accredited program in physical therapy, BSPT, MSPT, or DPT (2003 or later must have MSPT or DPT) Currently registered/licensed in applicable State. Must maintain an active license in good standing throughout employment. One (1) year experience in post acute care or related setting preferred CPR certification upon hire or obtain during orientation. CPR certification must remain current during employment. Specific Job Requirements Participates in community health matters/projects as appropriate Proficient in Microsoft Word, Excel, and e mail Liaisons with patients, families, support departments, etc., to adequately plan for patient needs Must demonstrate good body mechanics at all times Make independent decisions when circumstances warrant such action Knowledgeable of practices and procedures as well as the laws, regulations, and guidelines governing functions in the post acute care facility Implement and interpret the programs, goals, objectives, policies, and procedures of the department Perform proficiently in all competency areas including but not limited to: patient rights, and safety and sanitation Maintains professional working relationships with all associates, vendors, etc. Maintains confidentiality of all proprietary and/or confidential information Understand and follow company policies including harassment and compliance procedures Displays integrity and professionalism by adhering to Life Care's Code of Conduct and completes mandatory Code of Conduct and other appropriate compliance training Essential Functions Follow physical therapy treatment plans for patients Establish, assess, and modify realistic, measurable, timely, and functional goals Oversee and evaluate care given by PTAs, Rehab Aides, and PT and PTA students Chart appropriately and timely Utilize therapy software appropriately and accurately Exhibit excellent customer service and a positive attitude towards patients Assist in the evacuation of patients Demonstrate dependable, regular attendance Concentrate and use reasoning skills and good judgment Communicate and function productively on an interdisciplinary team Sit, stand, bend, lift, push, pull, stoop, walk, reach, and move intermittently during working hours Read, write, speak, and understand the English language An Equal Opportunity Employer

Working under the supervision of the PACE Center Manager, the PACE Social Worker is independent in the application of social work knowledge and skills. He/she is responsible for providing direct social work care management services to the participants of the North East Medical Services PACE program and works collaboratively with the PACE Interdisciplinary Team (IDT) to manage long-term care needs. ESSENTIAL JOB FUNCTIONS: Conducts psychosocial assessments to determine PACE Participants' care needs, preferences, and goals. Participates as an active member of the IDT for Participant care planning. Documents accurately and thoroughly in the Participants' electronic health record in accordance with industry and departmental standards including time requirements. Communicates effectively and openly with other Interdisciplinary Team members on participant status and needs. Job Details Adelante Healthcare Roosevelt - Phoenix, AZ Exempt HS Diploma from accredited school Health CareDescription This position requires excellence in performing multiple psychotherapies including, but not limited to, acceptance and commitment therapy, cognitive behavioral, mindfulness, behavioral activation, dialectical behavioral, interpersonal, supportive, motivational interviewing, graded exposure and ritual prevention, solution-focused, desensitization, couples, family, grief, and trauma therapies. Eye movement desensitization and reprocessing is also preferred. Such therapies will be provided in both one-to-one and group settings. The therapist will be responsible to complete initial intakes for the behavioral health program. The therapist will make a clinical decision on the care that is recommended following the intake. The patient may be referred to see the Psychiatrist, may remain with the therapist for Psychotherapy or may see both providers, based on the patients needs and their acceptance of treatment options. The therapist will work collaboratively with the behavioral health team (psychiatrist, case manager and behavioral health medical assistant) directly and with primary care providers indirectly. Treatment plans will be developed, maintained and adjusted over the course of treatment for each patient. The therapist will monitor patients in a patient registry and aid in making recommendations for a portion of these patients by record review without actually seeing the patients. The therapist will contact patients at predefined intervals for those patients who are missing appointments, meeting treatment plan goals, or as otherwise required. The therapist will coordinate care and ensure patients obtain referrals to outside sources or back to their primary care team after completing treatment plan goals. POSITION REPONSIBILITIES/ESSENTIAL Master's degree in the Behavioral Health field (Psychology, Social Work) with two (2) years of directly related experience is desired LPC Computer literacy is required Bilingual (English/Spanish) is preferred Certification or certification eligibility as an LCSW, LPC, Psy.D., LISAC, LMFT, or Ph.D. is required. Excellent working knowledge of behavioral medicine and evidence-based treatments for medical and mental health conditions. Ability to work through brief patient contacts as well as to make quick and accurate clinical assessments of mental and behavioral conditions. Should be comfortable with the pace of primary care, working with an interdisciplinary team, and have strong communication skills.
