Woodfords has an opening for a full-time Adult Case Manager within our Adult Case Management program in Cumberland County. This position is based out of our Westbrook office.
Woodfords' Adult Case Management Services are designed to provide support to adults with special needs and their families Case managers help connect individuals to resources and services in their own communities Case managers work with adult consumers to identify their interests, strengths and vision for their desired lives, and to develop plans to help them achieve their goals


Schedule:
Case Managers typically work Monday through Friday between 8am and 4pm, but do work some evenings to meet with consumers, as needed. Case Managers work full-time, and are salaried, with some work-from-home and remote schedule flexibility.


Salary:
$38,000.00 to $42,000.00 annually, commensurate with education and experience Adult Case Managers have the opportunity to earn additional financial incentives of $350 or more monthly

Responsibilities of the Adult Case Manager:
Meet regularly with consumers in a variety of settings to assess consumer functioning, progress, quality of life, health and safety issues, and to address concerns in this area Seek collateral contact with family and community providers to assess consumer needs and progress Identify and report any suspected or observed abuse and/or neglect of an adult Assist adults with maintaining a stable living arrangement and supporting their abilities to function in the community, vocationally, socially, financially and emotionally Provide triage to consumers with emergency situations and provides information to consumers on emergency preparedness Possess working knowledge and understanding of MaineCare Sections 13:12, Section 21 and Section 29 Attend regularly scheduled meetings with DHHS Maintain up to date action notes, V-6 assessments, BMS-99 and other requested EIS requirements as determined by DHHS Complete all Person Centered Plan responsibilities including and not limited to service planning and related documentation. The case manager will coordinate and monitor the plan and services throughout the year and make necessary changes if needed. Initiate and maintain positive and cooperative relationships with other community supports and provider systems


Qualifications of the Adult Case Manager:
Bachelor's Degree from an accredited four-year institution of higher learning, with a specialization in psychology, behavior health, social work, special education, counseling, rehabilitation, nursing, or a closely related field At least one (1) year experience in the field of developmental disabilities A valid State of Maine driver's license and have use of an insured vehicle Good organization, communication, decision-making, and interpersonal skills Computer skills and attention to detail when writing

JOB SUMMARY:

Adult Blended Case Managers are responsible for assisting individuals to make full use of Behavioral Health Services and natural community supports which will help individuals achieve a more independent and fulfilling life. Services provided will include developing a partnership with the individual to assess needs, obtain services, and resolve crises. Through this partnership the individual and Adult Blended Case Manager will work together to plan, coordinate, monitor, and advocate for the services and supports needed to achieve the individual’s goals.

SPECIFIC DUTIES AS AN ADULT BLENDED CASE MANAGER:

  1. Develop partnerships with assigned individuals to assess needs, obtain services and resolve crises
  2. Assist individuals with the development of a Personal Goal Plan
  3. Assist individuals with identifying a goal and defining the steps needed to achieve the goal
  4. Complete all assessment and planning documentation including Adult Psychosocial Assessment; Strengths Assessment; Treatment Plans and other required documentation
  5. Ensure all required documentation is completed in a timely, comprehensive and legible manner and is in compliance with PATH policies and procedures
  6. Provide 24 hour/7 days a week crisis coverage as assigned
  7. Provide weekend staffing as assigned
  8. Assist in linking individuals with their chosen services and resources in the community such as needed psychiatric and medical treatment, residential services, financial entitlements, social/recreational activities and general social services
  9. Monitor those supports and services for appropriateness and effectiveness
  10. Provide informal support and network building to help individuals increase their support system outside of the Behavioral Health System

Position Responsibilities : The Case Manager is an active member of a treatment team that works with adults diagnosed with psychiatric disabilities. Qualifications: Bachelor degree in Social Work, Psychology, or other mental health related field with one (1) year experience providing direct services to patients in a mental health setting preferred.
