In this position, you will:
Help people supported achieve/exceed their personal goals
Provide structure and emotional support
Assist in resolving behavioral issues
Help people supported with personal hygiene needs
Complete detailed, accurate documentation
Transport people supported to and from medical appointments and recreation activities using Agency vehicles
We require:
Clean, valid NYS Driver's license
High School diploma or equivalent
Some experience working with the intellectually and developmentally disabled population preferred; but will train. Cooking and light housekeeping may be required.
Prior experience as Caregiver, Care Assistant, Personal Assistant, Resident Assistant, Direct Care Worker, Nursing Assistant, Home Care Aide or Companion is a plus
Salary ranges from 18.00-20.00
Some program/locations may have a differential in addition to the hourly rate.
ACLD has over 40 locations throughout Nassau and Suffolk counties. We have day, afternoon, evening and overnight shifts available including weekend only shifts.

Per Visit Pay Range: $40- $73.
The range is based on the regular visit rate and initial per visit rate.
Under supervision of the DOPS, conducts medically-based assessments (initial assessments and reassessments) of patients in their home.
ESSENTIAL FUNCTIONS/RESPONSIBILITIES
Complies with all agency policies and procedures.
Evaluates individuals' health status, strengths, care needs, and preferences and guides the development of individualized long term care service plans.
Ensures that the individuals with long term care needs receive the right care, within the right setting, and at the right time.
Responsible for insuring that all confidential and potentially sensitive information is processed, maintained, and utilized according to procedures in strict confidential manner.

MINIMUM QUALIFICATIONS
Must possess a current license and be registered to practice in NYS as a professional nurse.
Must possess a valid NYS driver's license.
Must possess an operational motor vehicle.

Responsibilities:
Provide evidence-based interventions to help clients manage their mental health symptoms and achieve their treatment goals, using a range of therapeutic modalities and techniques.
Collaborate with community agencies, healthcare providers, and other stakeholders to provide coordinated, client-centered care.
Conduct comprehensive assessments of clients' mental health needs and develop individualized treatment plans in collaboration with the client, their families, and other healthcare providers.
Provide crisis intervention services as needed to clients in the community.
Monitor and document client progress, adjusting treatment plans as necessary to ensure continued progress towards treatment goals.

Qualifications:
Master's degree in Social Work or a related field, with a focus on mental health.

Spanish Speaking a plus

In this position, you will:

We require:

· Must obtain and maintain AMAP (within six months of hire) and SCIP certifications
· Prior experience as Caregiver, Care Assistant, Personal Assistant, Resident Assistant, Direct Care Worker, Nursing Assistant, Home Care Aide or Companion is a plus
ACLD has over 50 locations throughout Nassau and Suffolk counties. We have day, afternoon, evening and overnight shifts available including weekend only shifts.
