Central Health Management is seeking a qualified, licensed Nurse Practitioner with experience in primary care for a part-time role in the Virginia Beach/Hampton Roads area. Our practice focuses on providing quality care and mitigating hospital readmission through best practice care management. The role involves some travel in the area and is compensated accordingly. Clinicians of Nurse Practitioners Services of Texas provide care to nursing home residents enrolled in an Institutional Special Needs Plan called Texas Independence Health Plan. Provides primary care and acute care management for patients in 4 nursing homes in collaboration with an RN Care Coordinator. The emphasis is on proactive and preemptive care through general health management, increased availability of on-site clinicians, and early identification of changes in condition to avoid unnecessary hospitalizations and ED visits.
