Position Responsibilities
Provide skilled physical therapy services to home health patients in accordance with physician orders and established care plans
Collaborate with patients, families, and interdisciplinary care teams to support functional independence and safety in the home
Maintain accurate, timely clinical documentation in compliance with regulatory and organizational standards
Participate in team meetings, care coordination, and quality improvement initiatives
Supervise Physical Therapist Assistants (PTAs) as assigned
Qualifications
Active Ohio Physical Therapist license (required)
Minimum of one year of experience as a Physical Therapist (hospital, SNF, ALF, hospice, or home health)
Home health experience preferred; training available for qualified candidates
Strong communication and interpersonal skills
Ability to manage a caseload of approximately 2025 patients
Collaborative mindset with interest in mentorship and interdisciplinary teamwork
You will work closely with an in-house therapy team that includes PTs, PTAs, OTs, COTAs, and Speech Therapists.
