The Physical Therapy Assistant Provides clinical physical therapy treatments for program participants on acute, restorative and maintenance levels as needed. Follows the treatment plan as per Registered Physical Therapist assessment and documented care plan.
QUALIFICATIONS:
A graduate from a physical therapist assistant curriculum approved by the American Physical Therapy Association.
Currently licensed as a Physical Therapist Assistant in the state of Colorado.
Minimum of one-year experience working with the frail or elderly population required.
Minimum of one year experience as a Physical Therapy Assistant in a skilled nursing facility is preferred.
Good working knowledge of physical, psychosocial and behavioral needs of the elderly population.
Able to understand medical terminology and how it applies to physical therapy, to communicate with participants, staff, IDT and physicians, and how to prepare meaningful reports.
Ability to communicate with the participants to establish favorable attitude to motivate the participants to desired cooperation and effort in his/her rehabilitation development.
Must have a valid driver's license, proof of insurance and have means of transportation.
ESSENTIAL FUNCTIONS:
Perform physical therapy treatments and related duties as may be delegated by the physical therapist.
Communicate Participant's changes and progress per treatment plan on a regular basis to Registered Physical Therapist.
Maintain proper documentation according to state, federal PACE regulations.
Maintain proper records according to PACE policy and procedures.
Perform and record, if required, as specified by the planned treatment program, non-evaluative on-going data: Measurements of vital signs, range of motion, strength and linear increments; Types of assistive mobility devices used, and the distances covered; Results of tests and evaluations performed by the registered physical therapist.
Respond to psych-social support needs of the participant and/or the participant's family.
Communicate appropriately and effectively with all personnel and to document according to professional standards.
Keep timely records, meet regularly with the Registered Physical Therapist and attend care conferences as requested.
Participate in patient care conferences and other rehabilitation related meetings as appropriate.
Communicate with Registered Physical Therapist and IDT members regarding patient progress, problems and plans.
Participate in in-service training programs for all staff in the facility.
Instruct participant and family members and nursing staff in restorative and therapeutic maintenance programs.
Abides by regulations pertaining to PACE as established by Federal and State Agencies and the Volunteers of America Health Facilities policies.
Attends required in-services & completes assigned online modules.
Perform other duties as assigned.

The Medication Technician maintains our pharmacy database with up-to-date and accurate information. Responsibilities include ordering, receiving, storage, distribution and discontinuation of participant's medications and maintaining all required documentation for medication and treatment administration.

Responsible for processing of all medication orders for all participants through direct input (75%) or processing of medication orders inputted by staff provider (25%) with respect to providing full, complete, and legal prescriptions to dispensing pharmacy as defined by pharmacy protocol and state/federal regulation.
Responsible for coordination with pharmacy and relevant Senior CommUnity Care departments to maintain participant medication schedules (maintenance of cycle system) through implementation of new medication orders, mid-cycle medication changes, discontinuations, and reconciliations through transition process as prescribed by policy.
Responsible for medication reconciliation through transition periods (LTC/Hospital/ALF discharge) for all participants and facilitating any changes required.
Responsible for communication with both PACE and community-based providers in regard to medication orders, clarifications, and pharmacy recommendations. Also responsible for communication between pharmacy and prescriber.
Responsible for attendance and participation in IDT.
Responsible for coordinating with local pharmacies to obtain urgent medications that are not available through on-site dispensing. Coordination involves transmission of medication order, demographics, and insurance billing information.
Responsible for inventory and maintenance of on-site medication stock as provided through pharmacy. Also responsible for equipment and computer maintenance and reporting functions.
Responsible for obtaining signed prescriptions for required controlled substances and delivery of those signed prescriptions to the dispensing pharmacy.
Responsible for entering all medication orders for new enrollments, setting up pharmacy cycle, delivery system, assistive systems if needed, linking diagnosis information to medications (through H&P), and verifying all information in writing with participant's primary care provider. Also responsible for coordinating medications for dis-enrolling participants to provide needed medications for a period of twenty-eight (28) days after leaving program.
Responsible for communication with participant and/or family as needed or required.
Alternative Medication Delivery Systems: Responsible for filling alternative medication delivery systems (MRB, Med-E-Lert, etc.) in accordance with medication orders and organizational policies/ procedures.
Responsible for in-house monitoring of participant INR values, dose adjustment, and dispensing of Coumadin for participants, under the direction and supervision of Medication Manager.
Responsible for making medication changes to currently filled delivery systems in accordance with medication orders and organizational policies/ procedures.
Responsible for coordinating delivery of medications from pharmacy to participant either through direct carrier shipping or delivery through or from PACE center.
Responsible for restocking and inventory control of ATM machines at the direction of contracted pharmacy. Also responsible for conducting periodic inventory audit as needed or required.
