The Case Manager facilitates optimal outcomes for all parties involved through advocacy, objectivity and liaison/collaboration with an emphasis on continuity of care, effective communication, and coordination of appropriate healthcare services.
The Case Manager utilizes a variety of skills to educate, consult, and assist in the management of an identified patient population in order to achieve the highest quality and best possible outcomes for the patient in a cost effective manner. He/she collaborates with a patient care team that includes, but is not limited to physicians, nurses and assistive staff, rehabilitative therapies, respiratory therapy, pharmacy, dietetics, psychosocial support disciplines, infection control, fiscal services, performance improvement, and any other disciplines directly and indirectly involved in patient care.
The Case Manager utilizes a scientific approach in the analysis of patient care, identifying opportunities for the improvement of patient care/outcomes, quality of life, and cost reduction based on current research findings supported in the literature. Further, this individual collaborates actively on a daily basis (Monday-Friday) with the identified case management team in order to facilitate timely achievement of expected patient outcomes. When not on campus, the Case Manager is available by pager, digital phone, and e-mail.
The qualified candidate will possess the following:
A. Education
1. Graduate of an accredited School of Registered Professional Nurses, BSN preferred, MSN encouraged
B. Experience
1. Minimum of five years of experience in the care of assigned patient population
2. Participation in case management and utilization review encouraged
C. Licensure/Certification
1. Current registration to practice as a Registered Nurse in the the applicable state as determined by job location
2. Case Management certification encouraged

* Identifies system issues and suggest interventions to the Supervisor to eliminate duplication in services and documents significant variances. Identifies trends/process barriers and participates in development of plans to ensure a method to move patients effectively through the care continuum.
* Monitors medical necessity of admissions, continued hospitalization, and surgical procedures using criteria approved by the medical staff and evaluates appropriate level of care for patient and notified the Utilization Review staff of necessary changes
* Utilizes principles of quality improvement in every aspect of performance by participating in unit and organizational quality improvement activities to develop strategies to reduce or resolve variations in practice.
* Provides age/developmentally-appropriate patient planning and communication in accordance with Age-Specific Care Guidelines for the specific age groups served.
* Maintains working knowledge of payer and reimbursement practices impacting the plan of care. Demonstrates the ability to guide the patient and family through an evaluation of their options for post discharge care.

Key Responsibilities
* Patient Coordination:
* Serve as the primary point of contact for patients and families throughout the structural heart care process.
* Schedule and facilitate diagnostic testing, consultations, and procedures.
* Provide education regarding procedures, risks, benefits, and recovery expectations.
* Clinical Management:
* Collaborate with multidisciplinary teams including cardiologists, cardiac surgeons, imaging specialists, and anesthesia.
* Review patient charts for appropriateness and readiness for intervention.

* Program Development & Quality:
* Maintain accurate data for national registries (e.g., STS/ACC TVT Registry).

Minimum Job Requirements:

Department: 36963 Wake Forest Baptist Medical Center - Cardiology: Device Clinic Reynolds Status: Full time Benefits Eligible: Yes Hours Per Week: 40 Schedule Details/Additional Information: Weekdays JOB SUMMARY Under general supervision, perform a variety of cardiac arrhythmia monitoring duties to recognize abnormalities. This includes trans-telephonic pacemaker follow-up, scheduling, downloading, analyzing, and distributing cardiac monitoring transmissions. Collaborates with multi-disciplinary team members to carry out age/developmentally appropriate care in accordance with standards of care. Coordinates services throughout the region to ensure delivery of quality care to patients in a team approach.
EDUCATION/EXPERIENCE: Graduate from an accredited School of Nursing. BSN preferred. Minimum two years' nursing experience in cardiac care. Current pacemaker and defibrillator experience.
LICENSURE, CERTIFICATION, and/or REGISTRATION:
Registered Nurse licensure from the State of North Carolina. Advanced Cardiac Life Support (ACLS) required. IBHRE Certified Cardiac Device Specialist (CCDS) preferred.
ESSENTIAL FUNCTIONS:
* Prioritize communication and collaboration of patient care based on diagnostic evaluation of implantable devices and patient symptoms with mid-level practitioners, nurses, and physicians. Facilitates continuity of care using the nursing process, multidisciplinary collaboration, and coordination of appropriate health care services. Interacts with patients during face-to-face encounters or through a variety of telecommunication strategies, often establishing long-term relationships.
* Demonstrate proficiency in operating multiple manufacturers' programs and remote websites. Evaluate specific manufacturers' algorithms and device functionality.
* Provide independent direct patient care by performing interrogations and reprogramming of pacemakers, defibrillators, implantable loop recorders and other devices.
* Manage the device work queue to provide care for remote and on-site patients.
* Develop and maintain a computer database of all pacemaker and defibrillator patients in the region to ensure appropriate management of all studies.
* Conduct specialized ambulatory cardiac diagnostic studies.
* Review and interpret applicable diagnostic information for all device alerts including, but not limited to, echo results (ejection fractions, left atrial size, and valve abnormalities), anticoagulation status, and antiarrhythmic drug therapy. Provide MD with alerts.
* Document patient care and physician orders related to devices within WakeOne. Also enter data into external device database.
* Communicate a teaching plan to the patient and family for each implantable device, would care, and remote monitoring.
* Communicate information and instructions between patient and EP physician.
* Responsible for correct charge capture and review billing.
* Educate and track patients involved in device recalls, assuring they receive the FDA and industry recommendation for care and follow-up. Coordinate care plan with Electrophysiologist.
* Assist with clinical trials as requested and provide accurate documentation of device settings, specific tests, diagnostics, and any other information required for study protocols.
* Travel to all Wake Forest Baptist Health Cardiology EP locations.
SKILLS/QUALIFICATIONS:
Knowledge of cardiac arrhythmias
Proficient computer skills and electronic management of medical records
Good interpersonal and communication skills (verbal, non-verbal, and listening skills)
Ability to provide direct and indirect patient care
Flexibility with work schedule; possible work from home to maintain scheduled patient follow-up during inclement weather
Time management
Knowledge of Cardiac Rhythm Management (CRM) vendors and devices
WORK ENVIRONMENT:

Join our 24-bed observation/medical unit at Atrium Health Cleveland, where you will care for a variety of patients. Our specialty is observation patients (chest pain, kidney stones, minor surgeries), we also take overflow medical/surgical patients on our unit.
