As a School Social Worker, you are primarily responsible for helping students in Grades K- 12 succeed academically, socially, and emotionally, through individual and group counseling and by collaborating with educators, parents, and other professionals to create a safe, healthy, and supportive learning environment that strengthens connections between home and school.
‖ Responsibilities Include:
Providing individual and group counseling sessions to address interpersonal or family issues that interfere with school performance, including crisis intervention consultation and/or family therapy as needed.
Maintaining all required paperwork including but not limited to communication logs and meticulous notes regarding all services provided to students.
Conducting interviews with students, family members, and school personnel to gather information required to assess student social, emotional and behavioral adjustment to school and the community.
Maintaining regular contact with students' parents/guardians to inform of student's progress, areas of difficulties, and any changes or situations in home life that may affect school performance.
Developing and updating Individualized Education Plans (IEP) and Behavior Intervention Plans (BIP) as needed in adherence to the company, school, state and federal regulations and procedure.
Attending and participating in IEP team meeting, facilitating at the discretion of the school districts.
Actively participating in other team meetings when necessary to address specific student and parent concerns.
Collaborating with education and social services professionals (for example wrap- around, probation officers, court systems) to develop appropriate agency services for students and their families.
Providing case management functions for students, which include but are not limited to, coordination of parent meetings, emergency transportation requests, and coordination with therapists outside the school setting.
Collaborating and consulting with teachers, parents/guardians, district personnel, and service providers to find effective solutions to learning and behavior problems; always establishing and maintaining effective public relations and projecting a positive company image.
Ensuring the use of positive social skill teaching methods by conducting monthly direct observation of classroom staff's implementation of IEP goals and/or Behavior Intervention Plans/ Treatment Plans.
Providing guidance, consultation, and assistance to teachers in the development of classroom schedules, maintenance of student data systems and facilitation of student transition plans to assure appropriate schedules and services based on each student's Individual Education Plan (IEP) and/or other applicable goals.
Assessing student progress through consistent review of classroom data collection and recording systems and providing applicable training and guidance to classroom staff accordingly in addressing student behavioral needs.
Conducting crisis assessments and contacting the appropriate authorities if needed to ensure that emergency intervention is provided to students.
Keeping up to date with research-based practices and developments in subject area.
Responding constructively to formal and informal feedback.
Performing other duties as assigned.
‖ Qualifications Required:
Master's degree or higher in social work, educational counseling, therapy or a closely related field of study.
Licensed currently or in the process of obtaining an IL professional educator license (PEL) school support personnel endorsement.
Licensed currently or in the process of obtaining a licensed clinical social worker (LCSW) credential.
Ability to obtain and maintain certification in company approved crisis management (PCM) training.
Prior experience and/or knowledge in special education services and compliance, particularly for students with severe learning disabilities, emotional disturbance and/or behavioral disorders.
Well-versed in applied behavior analysis (ABA) including positive behavior interventions and supports (PBIS), functional behavior analysis (FBA) and behavior intervention plans (BIP's).
Proven success in developing and implementing effective intervention plans for students with diverse needs, preferably in an alternative, special education and/or behavioral health program setting.
Proficiency in providing motivation and having critical conversations with students that help move them toward realistic goals.
Ability to think and act quickly and calmly in an emergency and make independent decisions.

Human Services Worker Youth Justice - Probation Services Hourly Range: $25.11 - $37.65 JOB SUMMARY: The primary function of the Human Service Worker (HSW) is to provide the State of Wisconsin-mandated services to accomplish the mission of the Children, Youth, and Family Services department. The role is responsible for meeting the needs of youth and families while maintaining community safety. HSWs provide both direct and indirect services by performing court intake activities; completing court documentation for upcoming court hearings; and all ongoing supervision of adjudicated delinquent or Juvenile in Need of Protective Services (JIPS) clients (ages 10 to 17).
JOB DUTIES:
Intake of youth referred for adjudication, Department of Corrections (DOC), and JIPS. Follow all applicable policies/procedures (i.e., intake interview with the family, gather assessment information through the Youth Assessment Screening Instrument (YASI), submit referrals and recommendations within timelines).
Attend all court hearings related to the client and provide written and oral reports to Courts regarding the youth's status. Give recommendations for placement/services, DOC extensions, waiver studies, testify at contested hearings if needed, and arrange for court-ordered services/programs.
Meet (in the office, virtually, and in the community) with the client and family/placement to determine the youth's current behavior and the possible need for intervention (e.g., arranging other services, doing UAs, GPS, 72-hour holds, suggesting behavior strategies, sanctions, other court activity, etc.).
Maintain contact with other professionals involved with a client, via phone, virtually, e-mail, or in meetings. Check school attendance, therapy/program participation, and community service. Respond to inquiries from police or other agencies regarding youth's behavior.
Routinely and professionally document all case activity, including case notes; follow guidelines for case plans and reassessments (YASI). Maintain case records and files in an organized and accessible manner.
Meet with the unit supervisor to discuss cases for case planning and/or assistance with case concerns. Follow agency protocol for staffing cases for court recommendations or referring cases back to the court regarding program/service provision for clients.
Based on the youth's behavior or needs, prepare, and submit documents such as capias requests, sanction motions, revisions for programs or out-of-home placements, motions to lift stays, extensions, compliance reports, etc.
Provision of coverage (both partner coverage and daily section coverage monthly), attend unit and section meetings, Synthesis service authorizations, authorization of Dayforce, submission of mileage forms, workgroups, and mandatory training.
Travel outside of Milwaukee County to visit youth in a secure placement (i.e., Lincoln Hills School (LHS), Copper Lake School (CLS), Mendota Juvenile Treatment Center (MJTC)).
Perform other duties as assigned.
Minimum Qualifications:
Must be a current resident of the State of Wisconsin.
Current Wisconsin Driver's License and access to a reliable insured vehicle; both maintained throughout employment.

What are the Job Responsibilities?

JOB PURPOSE AND REPORTING STRUCTURE: Under the direction of the Director of Community Services, the Outreach Worker outreaches and enrolls consumers that are homeless and have suspected mental illness. The Outreach Worker assists consumers to seek psychiatric and/or substance abuse treatment, to secure income, and find housing, in accordance with agency policies and procedures.
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.
Outreach consumers in the community. Outreach consumers that have been referred from other community Organizations. Go out and meet consumers where they are at, which includes parks, meal sites, shelters, clinics, crisis respite house, and others.
Interview consumers to assess whether they are homeless or possible mentally ill. Assess consumer for information on current treatment provider, having a release signed, and sending for records. If the consumer does not have a current provider, refer for psychiatric treatment at Recovery Behavioral Health Clinic.
If found eligible for PATH due to being homeless and having a suspicion of mental illness, concentrate on engaging with consumers. Engaging with homeless consumers can be very difficult but is the key to long-term success. Provide support supportive counseling to consumers, symptom assessment, and symptom management.
Interview consumers to complete assessments and crisis plans. Assessments include mental status exams. Develop plans of care with the consumer. Provide ongoing linkage and follow up, referrals for services, and service coordination.
Assess the need for psychiatric treatment, including substance abuse and trauma history.
Intervene to get to get consumers into treatment for the preceding issues.
Assist and refer consumer's to apply for SSI/SSDI benefits through the SOAR model. Assist consumers to find housing including providing technical assistance for rent or security deposit and applying for housing subsides.
Provide crisis linkage and follow up to include the preceding paragraph as well as assisting with medical treatment, obtaining legal assistance, finding employment, training and education, and development of social support systems. Provide supportive contacts and assist consumers in navigating through the community.
Keep daily consumer logs and enter information on a weekly basis into Wisconsin Servicepoint. Enroll eligible consumers into PATH using Wisconsin Servicepoint. Create consumer Intake and Eligibility Forms for all PATH enrolled consumers.
Transfer consumers where services are no longer appropriate or more services are needed. When appropriate, refer to CARS, Family Care or Adult services. Identify consumers that might be appropriate for Wiser Choice and/or OCHC AODA case management services through the SHARE program and do appropriate referrals.
Attend county staffings for shelter placement and/or to update the team when a consumer is staying at Safe Haven or THP.
Coordinate transportation for consumers to and from appointments, to include medical and mental health.
Attend and actively participate in departmental meetings and meetings with other service agencies.
Coordinate/schedule any necessary medical examinations or psychiatric evaluations through OCHC Clinics or establish collaborative relationship with an organization that will perform any examinations needed to make a disability determination.
Collaborate with and make referrals to other organizations to ensure favorable outcomes for consumers and beneficiaries.
Experience with jobs requiring organizational skills, administrative work, writing and editing.
Ability to comprehend instructions and understand medical/psychiatric terminology to screen claimants for program appropriateness.
