How you will change lives
As a Social Worker at US Renal Care, you will be an integral part of a cross-functional team, working to help patients living with kidney disease achieve maximum social functioning and psychological adjustment to dialysis treatment and rehabilitation.
What you will be doing
Advocate & Support. You will be part of an interdisciplinary team working to ensure patients receive the best care, including conducting all required patient assessment and care planning activities such as assessing new patient psychosocial needs and completing the KDQOL in accordance with company policy and all state/CMS regulations. You will identify and counsel psychosocial issues and provide patient and family education. As an advocate for your patients' needs, you will coordinate communities of support for patients and their families, identify social agencies and other resources (e.g., financial/funding), provide information and referrals, coordinate transient arrangements, and represent your patient as needed with appropriate local, state, and federal agencies.
Teamwork. As part of the interdisciplinary clinic team, promote teamwork, educate staff, and provide training around patient psychosocial care. You will participate in all required continuing education and staff meetings. You will collaborate with the Medical Director and physicians and maintain positive relationships with area hospitals, agencies, vendors, and the community.
Safety & Quality. You will help with clinical and operational processes to improve patient health and minimize missed treatments and hospitalizations, achieving target goals for patient outcomes. You will also participate in monthly Quality Assessment and Performance Improvement (QAPI) activities and ensure compliance with federal, state, and local laws and regulations.
What we're looking for
Master's Degree in Social Work accredited by the Council of Social Work Education (CSWE).
Current licensure (in good standing) in applicable state is required unless employed in the state of AZ, PA or Guam. Must meet any practice requirement(s) for the applicable state.
Demonstrated working knowledge of the English language and ability to communicate verbally and in writing.
Basic computer skills, including Microsoft Office (Word, Excel, Outlook).
Proficiency in all USRC clinical applications required within 90 days of hire.
Preferred
Previous experience in providing social services to dialysis patients preferred.
Other Requirements
Must meet any practice requirement(s) for the applicable state.
Additional license requirements may be applicable depending upon state.

Essential Functions:

The YCS is responsible for guiding and mentoring students, assisting with emergency response procedures, and collaborating with all members of the Shrub Oak staff. Our Crisis Intervention Aide (CIA) works directly with the Coordinator of Behavior Intervention and Supports, School Psychologist, Classroom Teachers, Residential Supervisors and all other staff to ensure the highest quality of care is implemented for each student.
Essential Duties & Responsibilities
Observe and Gather information pertaining to Functional Behavior Assessments and Behavior Intervention Plans, as requested by supervisor.
Train staff on the implementation of Behavior Intervention Plans as requested by supervisor.
Provide hands on modeling of Behavior Intervention Plan procedures and appropriate behavior intervention techniques.
Collect and manage data collection as requested by supervisor.
Ensure data collection sheets are available for staff.
Implement de-escalation techniques in accordance with ABA basic principles and SCIP-R guidelines, including the use of physical intervention when faced with behaviors that pose a risk to the safety of the individual or others.
Assist with morning transition to school to help ensure safe transport of students.
Implement discrete trials according to students learning needs, collect DTT data and report to classroom teacher.
Provide flexibility to assist where school needs may require.
Communicate with supervisor regarding student/resident behaviors, to include reporting any noted changes in behavior and any potential environmental changes/setting events that may impact behavior.
Interact and engage with classroom or residential unit activities that will benefit students/residents.
Maintain SCIP-R certification as required.

Acts as a bridge between youth and family, and foster care treatment teams through telephone, face-to-face contacts, family conferences, comprehensive treatment reviews, etc. Attend treatment review meetings.
Assists with reunification of youth with family, community, school by implementing permanent supports and resources including but not limited to identifying and working with a mentor, setting up after school recreational and job opportunities and developing a relapse prevention program.
Job Requirements
Minimum of a Bachelor's degree in Human Services, Social Work, Psychology, or a related field is required.
A Master's degree (MSW, MHC, MFT, or related) is strongly preferred and may be required for clinical or supervisory roles.
Required Preferred 1 years of direct experience working with at-risk youth and families, case management, child welfare, trauma-informed care preferably in a home-based, or community setting.
Must be able to work a flexible schedule that includes variable hours: days, evenings, weekends, holidays, and occasional on-call responsibilities.
Must possess a valid driver's license with a driving record acceptable to agency standards.
Access to a reliable vehicle is required, as frequent travel is necessary.
Ability to plan and coordinate transportation for home visits, appointments, and court-related obligations in an efficient and timely manner.

The Crisis Counselor reports to the Director of Public Safety and Health Services and shares responsibility for the direct mental health services for students, and as a training resource for faculty and staff. The Crisis Counselor works to provide crisis intervention, develops, administers, coordinates, evaluates, and promotes mental health and wellness services; The Crisis Counselor maintains frequent contact with students; college faculty and staff; various college personnel; and representatives and service providers from the community and governmental agencies.
Duties and Responsibilities:
Provide mental health services, including brief individual and group counseling and mental health assessment and referral services to students with a wide range of mental health needs to support their academic success and retention.
Provide triage and crisis intervention to students and individuals in need, and act as a resource to faculty and staff. Provide crisis intervention, evaluation and referral to community resources, as well as collaborates with community psychiatric evaluation teams, as needed.
Follow standard protocols for psychological counseling and adhere to ethical principles as stipulated by the New York State Mental Health Practitioners (Office of the Professions) and/or American Psychological Association and in accordance with laws and regulations pertaining to behavioral health care in the State of New York.
Assess the mental health needs of students and develop an appropriate plan of brief intervention with the student(s).
Serve as a resource to faculty, staff, and administration on mental health and wellness issues that may influence student behavior, including but not limited to, motivation, learning process issues, serious mental illness, suicide, violence, stalking, and threats towards others, anger management, conflict resolution, and social/communication skills.
Educate students, staff and faculty about the maintenance of mental health and wellness with a focus on stress and anxiety reduction.
Collaborate with the campus community to design innovative programs that promote student mental wellness and support student retention and success. These programs are prevention focused and may include anger management, substance abuse harm reduction, domestic violence awareness, relationship skills development, and suicide prevention.
Ensure students have knowledge of and equal access to campus-based mental health services through a variety of venues, such as student orientation, ongoing outreach, marketing, and small and large group education.
Coordinate and maintain collaborative relationships with local mental health agencies.
Participate in appropriate campus committees and coordinate with other campus committees and groups as needed. Serve as a resource for the campus on issues of mental health and to the applicable committees and/or teams, as needed.
Review, research, and modify mental health protocols/policies on an annual basis to assure compliance with laws, regulations, and accepted professional standards of practice; gather data to determine campus trends and needs; recommend protocol/policy modifications in response to identified needs.
Monitor and assist in maintaining the print and electronic/online resources (College website and social media platforms). Assist in ensuring accuracy of print and online publications related to area of responsibility.
Maintain confidential student information according to FERPA and HIPAA and maintain complete mental health treatment records.

Minimum Qualifications:
Master's degree from an accredited institution in clinical psychology, counseling psychology, social work, marriage and family therapy, or marriage, family and child counseling, or equivalent.
Possess a valid license in the state of New York as a Marriage and Family Therapist (LMFT), Licensed Social Worker (LCSW), Licensed Professional Clinical Counselor (LPCC), Licensed Mental Health Counselor (LMHC), and/or Licensed Clinical Psychologist.
Meet all New York requirements to provide supervision to qualified individuals seeking licensure with New York State Mental Health Practitioners (Office of the Professions) and/or the American Psychological Association.
Two years minimum recent experience as a licensed mental health provider.
