Key Responsibilities:
Conduct individual and group psychotherapy sessions for patients in a subacute or skilled nursing facility.
Assess, diagnose, and treat a wide range of emotional and behavioral health conditions, including depression, anxiety, adjustment disorders, and cognitive decline.

Requirements:
Active and unrestricted license to practice as a Licensed Professional Counselor in State applying for.

Ability to provide in-person services at assigned facilities, up to 45 minutes.
Ability to walk, stand, and move between patient rooms and offices within the facility throughout the day.

Preferred Qualifications:

The LifeSet Specialist is responsible for providing services to address the needs of young people 17-21 who have transitioned out of care and need individualized skill development and support to transition successfully into adulthood, specifically obtaining and maintaining safe and suitable housing, continuing their education, learning independent living skills, obtaining and maintaining employment, building a support system, and remaining free from legal involvement.
RESPONSIBILITIES:
The LifeSet Specialist provides individual services to 8-10 young adults in the home and community setting to assist the young adult with transition to adulthood, meeting a minimum of one time per week face to face with the young person and providing additional support via phone, text, or in person as needed. When indicated, a LifeSet Specialist may see a young person daily to ensure safety and stability.
Additionally, the LifeSet Specialist is directly responsible for all interventions associated with a specific young adult and their family. This includes case management and collaboration with external key stakeholders and referral sources, such as probation, child welfare workers, therapists, etc. The Specialist conducts ongoing assessment of the young person's needs from a strength-focused, solution-based perspective and develops service plans on a monthly basis.
The Specialist also documents all service contacts with the young adult within the timelines required by the organization. The Specialist is expected to attend all meetings related to the young adult and/or family with the purpose of gathering information specific to intervention development and advocating in the best interest of the young adult's long-term stability. The Specialist is on call 24/7 (rotating on-call at night and on weekends) and expected to respond in person at any time when it is determined to be necessary by the Team Supervisor, Clinical Consultant, and Program Director.
In addition, the specialist is expected to attend and actively participate in individual and team supervision sessions and trainings and to follow up and/or implement feedback and recommendations received from the Team Supervisor, Program Director, and Clinical Consultant. Trauma Victim Advocates are crisis responders embedded in the hospital 24/7 to support patients and their family members in the immediate aftermath of violence and assist victims and co-victims during the process of physical and emotional recovery. Trauma Victim Advocates also perform a variety of functions in the hospital's emergency department, including\: offering comfort to patients in the trauma bay, giving families regular updates about their loved ones' conditions, providing grief support to families of homicide victims, and linking patients and their families to crime victim services. COMHAR's Community Residential Rehabilitation Services (CRRS), located in the Kensington section of North Philadelphia, are a homelike environment providing clients with the maximum possible opportunity for independence and self-determination within a structured living situation. CRRS provides a range of educational activities, both inside and outside the residence, designed to improve daily living skills. Counselors assist clients with identifying their goals and working with individuals or as groups to assist them with attaining identified life, social or recreational skills, which are consistently addressed and reinforced. Emphasis is placed on establishing positive interpersonal relationships with other clients and counselors through weekly community meetings promoting healthy social interaction, and many clients partake in COMHAR day services to further their social and emotional development. .
Our CRRS is now looking for Full-Time Residential Counselor II who will be responsible for the supervision and required case management of CRRS clients while in residence at the site. Be knowledgeable of current CRRS regulations and follow them in the program.
Residential Counselor Responsibilities:
Responsible for communication on and between shifts of client information and supervisory directives
Deliver quality and consistent individual and group activities, i.e., counseling sessions, hours meetings, supervising residents' chores, monitoring medications, activities of daily living(shopping, meal preparation, cooking, personal hygiene, laundry), social and leisure skills.
Ensures safety and security of the facility including COMHAR property, valuables, medication storage, client funds, tokens/transpasses, petty cash, client rents, and crisis management.
Maintains involvement in the development and review of residential service plans, psychosocial evaluations and annual reviews.
Maintains required or requested timely and accurate chart documentation such as progress notes, residential service plans, house logs, incident reports, medication checklist, chore/meal schedules, etc.
Ensure compliance to all internal and external regulations, policies, and procedures, e.g., confidentiality, client rights, call-out procedures, health and safety, HIPAA, compliance, etc.
Promote continuity of program activity through the maintenance of proper channels of communication through a team approach to comprehensive services, i.e., supervision and staff meeting attendance as requested.
Participates in all mandatory training at hire and annually, e.g., confidentiality, fire safety, infection control, crisis prevention/intervention, suicide prevention, cultural awareness, disaster training, driver training, medication monitoring, safety and emergency procedures
Residential Counselor Physical Demands:
While performing the duties of this job, the employee is regularly required to talk or hear others and accurately interpret speech and sounds. Frequently required to stand; walk; climb and descend stairs. Required to use hands and arms to reach, hold and carry objects. Required to sit; stand, maintain balance; occasionally lift and/or move up to 50 pounds. SOCIAL SERVICE COORDINATOR (F/T)
The Social Service Coordinator oversees social services at Bethesda Project's Supportive Housing Program and is responsible for training and supporting Case Managers in cooperation with other program supervisors. This role meets regularly with their respective Director to establish strategic priorities across the program with the goal of providing quality services to assist residents or guests while maintaining residential stability or moving on to housing, while achieving individual goals.
Reports to: Director of Housing
Major Responsibilities:
Work collaboratively with other Social Service Coordinators to ensure continuity of social service provisions across Bethesda Project, while monitoring and reporting on key performance indicators with Directors.
Assist Program Coordinators to interview applicants in order to select best qualified candidates for Case Managers.
Provide new hire and ongoing training and clinical support for Case Managers.
Work collaboratively Program Coordinators to ensure appropriate and comprehensive onboarding and training for new Case Managers; maintain and monitor Social Service Procedures for programs and ensure Case Managers are properly trained; including conducting onsite visits to perform case file and medication audits on a monthly basis.
Provide clinical support to site teams during guest or resident mental health crises, as needed; organize and facilitate team debrief sessions following mental health crises that result in initiating an involuntary commitment.
Provide interim case management support to guests or residents during staffing transitions to ensure continuity of care.
Serve as a Field Instructor to Social Work students, as applicable.
Skill/Educational Requirements:
Master's degree, MSW or equivalent required
Previous experience in social services, including management and supervisory experience
Strong interpersonal and organizational communication skills
Team oriented disposition
Prior Supervisory experience required, in addition to demonstrated leadership qualities
Proficient use of Microsoft Office
Physical Requirements:
Requires repetitive movement of hands and fingers for typing and/or writing. Ability to communicate and exchange information. Frequent standing, sitting, walking and climbing stairs, occasional stooping, kneeling or crouching and lifting up to 35lbs. Ability to reach with hands and arms and perform audio and visual assessments, as required.
