RESPONSIBILITIES
The Advocate will perform intakes for new clients upon arrival at the shelter.
Establish and maintain positive relationships with clients.

Assist residents leaving shelter in transition to another living situation (including referrals, furniture, security deposits, applications)
Provide necessary cleaning of resident rooms upon exit; Prepare rooms for new resident arrival

Accompany adult victims/survivors to a hospital or private physician, police and/or court to provide advocacy and/or emotional support upon victim's/survivor's request

ESSENTIAL FUNCTIONS
Monitors the development, implementation and documentation of the individual plan.
Works directly with the CIS in ensuring accountability for the efficient implementation of the individuals plan.
Completes daily ISP goal implementation and tracking, scheduled IP's, quarterlies, progress reports and behavior programs as required by QSR and internal quality assurance procedures.
Establishes effective program strategies to address all individuals' areas of need.
Conducts staff trainings as needed.
Monitors Therap T-logs and GER and updates the critical incident log as needed.
Reviews Therap documentation to support billing compliance.
Assists with the facilitation of referrals, i.e. tours, intake paperwork.
Participates in clinical support team meetings as required.
Writes one-time funding proposals for approval.
Provides support i.e. hands on, guidance, education to staff and individuals as needed.
Covers for the manager as needed.
Covers for Community Inclusion Specialists as needed.

SUMMARY
Family Service League is seeking a part-time Youth Peer Advocate for the Children and Family Treatment and Support Services (CFTSS) program. The Youth Peer Advocate will deliver services to youth enrolled in our High-Fidelity Wrap program for up to 15 hours weekly. The Youth Peer Advocate will work with youth ages 5-21 who are experiencing social, emotional, medical, developmental, substance use and/or behavioral challenges in their home, school, placement and/or community. Travel is required.
RESPONSIBILITIES
The Youth Peer Advocate will meet with clients weekly in their home and community.
Develop skills towards goals identified that are focused primarily on activities that involve helping the youth in the areas of skill building, coaching, engagement, bridging and transition support, self-advocacy, self-efficacy and empowerment, and community connections and natural supports.
The Youth Peer Advocate will attend monthly child and family team meetings.
Work closely with the High-Fidelity Wrap Care Manager and Family Peer Advocate.
Document in an EHR and conduct regular communication.
The Youth Peer Advocate will develop treatment plan goals.
Transport youth as needed.
Work as a team with other CFTSS and care management staff.
Participate in monthly staff and team meetings (in person or via phone).
All other duties as assigned.
QUALIFICATIONS
High School Diploma or equivalent required.
Must be an individual 21 to 29 years old who has self-identified as a person who has first-hand experience with mental health or behavioral challenges, or co-occurring disorders.
Youth Peer Advocate credential required.
Must be able to use a “lived experience” with a disability, mental health condition, involvement in juvenile justice system, special education, substance use disorder and/or foster care to assist in supporting youth in their resiliency, recovery, and wellness.
Prior experience working with children required; children with special needs preferred.
Excellent interpersonal, engagement and verbal and written communication skills required.
Ability to be creative and present ideas and skills in a variety of ways required.
Proficient computer skills, including Microsoft Office required.
Bilingual in Spanish preferred.
Valid and clean NYS Driver's License required. Must own a reliable car that is properly insured, registered, and maintained to drive from worksite to worksite.
PHYSICAL REQUIREMENTS
This position requires sitting for long periods of time and travelling to different client homes, community settings and other FSL offices.

Under the supervision and direction of the department Manager, the Program Services Case Coordinator assumes responsibility for monitoring and managing direct care staff adherence to all documentation requirements associated with individual's programs, activities, and program plan implementation. Ensures all individual' s documentation and case files are in compliance with DDS, CARF and other regulatory agencies. Direct liaison between Easterseals and stakeholders i.e. families, DDS case managers, state agencies, and community resources. Works in collaboration with the management team.
ESSENTIAL FUNCTIONS
Monitors the development, implementation and documentation of the individual plan.
Works directly with the CIS in ensuring accountability for the efficient implementation of the individuals plan.
Completes daily ISP goal implementation and tracking, scheduled IP's, quarterlies, progress reports and behavior programs as required by QSR and internal quality assurance procedures.
Establishes effective program strategies to address all individuals' areas of need.
Conducts staff trainings as needed.
Monitors Therap T-logs and GER and updates the critical incident log as needed.
Reviews Therap documentation to support billing compliance.
Assists with the facilitation of referrals, i.e. tours, intake paperwork.
Participates in clinical support team meetings as required.
Writes one-time funding proposals for approval.
Provides support i.e. hands on, guidance, education to staff and individuals as needed.
Covers for the manager as needed.
Covers for Community Inclusion Specialists as needed.
Performs other related duties as required.
JOB KNOWLEDGE
This position may be authorized to have access to protected Health Information (PHI) to perform specific job duties.
SKILL REQUIREMENTS
Valid driver's license, good driving record as deemed by the agency's liability broker. Will be required to drive company van as needed to transport individuals served.
Certification of CPR & First Aid, PMT, and all other required state trainings within required timeframe.
Must have personal vehicle to attend external meetings, site visits, etc.
Eligible to obtain DDS medication certification based upon program needs.

Strong organizational skills to manage priorities, meet deadlines and plan effectively.
