Minimum Job Requirements
Bachelor's Degree in Child Life, Child Development, Early Childhood Education, or related field
CCLS - Certified ChildLife Specialist required within 1 year of hire
American Heart Association AED - maintain active and in good standing throughout employment
Completion of Child Life internship following the Association of Child Life Professionals current standards (if candidate has validated Certification from the ACLP, then this requirement will be waived)
Knowledge, Skills, and Abilities
General skills to resolve problems requiring the use of child developmental theory, professional child life principles and experience- based knowledge.

Job Description: Quality Team - Senior Human Services Program Specialist
This is a highly responsible position tasked with supporting the Office of Economic Self Sufficiency Program (ESS) statewide Operations. This position is tasked with assisting with special projects, implementation activities, and special case reviews for the ESS team. The incumbent for this position will have extensive ESS Policy and operational knowledge, specifically in TANF, SNAP, Medicaid determinations along with knowledge of how to run and operate a Disaster Supplemental Nutritional Assistance Program (DSNAP) should a disaster occur in the state. disabilities do not permit them access to other prevocational services. The service will be
reflected in youth's plan of care and must be directed to teaching skills rather than explicit
employment objectives. In addition, Prevocational Services assist with facilitating appropriate

Respite
This service focuses on short-term assistance and/or relief for children/youth with disabilities
(developmental, physical and/or behavioral), and family/caregivers which can be provided in a
planned mode or delivered in a crisis environment. Respite workers supervise the child/youth
and engage the child in activities that support his/her and or caregiver/family's constructive
interests and abilities.
Planned respite services provide planned short-term relief for family/caregivers that are needed to enhance the family/caregiver's ability to support the child/youth's functional, mental health/substance use disorder and/or health care issues. The service is direct care for the child/youth by staff trained to support the child/youth's needs while providing relief from caregiver activities for the family/caregiver. This may occur in short-term increments of time (usually during the day) or on an overnight or longer-term increment. Planned Respite Services support the plan of care goals. Planned Respite activities include providing supervision and recreational activities that match the child/youth's developmental stage and continue to maintain the participant's health and safety.
Crisis Respite is a short-term care and intervention strategy for children/youth and their families as a result of a child's mental health/substance use crisis event, medical crisis or trauma that creates a risk for an escalation of symptoms without supports and/or loss of functioning. It may be used when acutely challenging emotional or medical crisis occur which the child/youth is unable to manage without intensive assistance and support. The need for Crisis Respite may be identified as a result of crisis intervention or may come from referrals from the emergency room, the community, LDSS/LGU/SPOA, school, self-referrals, Care Manager referrals, or as part of a step-down plan from an inpatient setting.
Supported Employment
Supported Employment provides ongoing supports to participants who, because of their disabilities, need intensive on-going support to obtain and maintain and individual job in competitive or customized employment, or self-employment, in an integrated work setting in the general workforce for which an individual is compensated at or above the minimum wage, but not less than the customary wage and level of benefits paid by the employer for the same or similar work performed by individuals without disabilities. The outcome of this service is sustained paid employment at or above the minimum wage in an integrated setting in the general workforce, in a job that meets personal and career goals.
Caregiver/Family Advocacy Supports and Services

What you will do: As a
Youth Care Worker - Days
, you will provide 24-hour security and supervision to always ensure the safety of our Youth. You will monitor and document behaviors, monitor and join the Youth during Sports and Recreation time and assist and mentor the youth while they learn and develop within the program.
To be considered you should: Possess a High School Diploma or GED, have one-year experience working with at-risk youth in a direct care position OR 60 hours of college education and possess a Driver's License.

PRIMARY RESPONSIBILITIES AS A MEMBER OF OUR TEAM TO LEARN AND EXECUTE
PT Business Management
* Build and maintain an active client base
* Manage and maintain an accurate schedule, inclusive of client appointments, meetings, and open availability within our internal scheduling systems
* Market and sell personal training programs to members and/or clients through phone calls, emails, and in-person interactions
* Maintain knowledge of all club services, programs, and products
Service and Hospitality
* Execute fitness assessments, guided workouts, and other complimentary services for members
* Interact with members to enhance their workouts and overall club experience
* Create and coach personalized programs for both in-person and virtual clients
* Maintain an organized and safe fitness floor during shifts and sessions
Professional Development
* Attend Continuing Education classes to elevate knowledge and qualify for promotions
* Attend and participate in any required one-on-one or team meetings with management
Qualifications
* Current Personal Training certification or willingness to obtain one through certification reimbursement program.
* Eligibility to begin a higher Tier & pay rate based on previous experience and education (Degree in the field)
* Current CPR/AED certification
* Certification opportunities available for non-certified applicants

As a Health Advocate at Rise Health Services, you will serve as a key point of contact for our members - particularly those under value-based care or care-management programs. Your role is to engage members via inbound and outbound calls to help coordinate preventive care, routine screenings, medication adherence, appointments, and overall health maintenance. Through effective communication, guidance, and follow-up, you will help improve health outcomes, close care gaps, and support the organization's quality and payer performance goals.
Key Responsibilities:
Member Outreach & Engagement
Make outbound calls to members to schedule preventive care (screenings, wellness visits, immunizations), follow-up appointments, and medication refills.
Handle inbound calls from members seeking assistance, guidance, or support regarding their benefits, coverage, or care coordination.
Educate members about their health benefits, preventive care recommendations, and available resources.
Care Gap Closure & Quality Support
Assist members in closing care gaps tied to quality and value-based metrics (e.g., preventive screenings, chronic-care management, immunizations).
Track and follow up on member care plans, ensuring services are scheduled and completed.
Document all member interactions, outcomes, follow-up needs, and care coordination steps accurately in the system.
Coordination with Providers & Internal Teams
Communicate with providers' offices, clinics, pharmacies, or other partners to facilitate appointments, referrals, or service access as needed.
Escalate complex issues to clinical or care-management teams when additional intervention or support is required.
Member Advocacy & Support
Help overcome barriers to care - logistical, behavioral, educational, or coverage-related.
Offer empathetic, member-centered support; build trust and rapport while ensuring professionalism.
Reporting & Metrics Tracking
Maintain call logs, outreach records, outcomes, and relevant data within CRM/EHR or care-management systems.
Assist in producing reports or dashboards tracking outreach results, care-gap closure rates, preventive care compliance, and quality-related metrics.
Support periodic audit or quality reporting requirements as needed.
Compliance & Confidentiality
Handle protected health information (PHI) in compliance with HIPAA and organizational privacy policies.
Follow company guidelines for documentation, data security, and member privacy.
Requirements:
Preferred Qualifications & Skills
High school diploma or GED required; Associate's or Bachelor's degree in Healthcare Administration, Public Health, Social Work, or related field preferred.
1-3 years (or more) experience in call center, member services, care coordination, or related healthcare outreach roles - ideally within value-based care or managed care environments.
Excellent verbal and written communication skills; strong active listening and customer service orientation.
High proficiency in computer use - comfortable with EHR/CRM systems, web-based applications, telephony systems, and data entry.
Empathy, professionalism, patience, and ability to work sensitively with potentially vulnerable or elderly populations.
Strong organizational skills, reliability, and the ability to manage multiple tasks and follow-ups.
Ability to work from a remote workspace: stable wired internet, quiet and private work area, reliable phone/ headset setup (if remote).
