IEP Meetings and Documentation: Attend IEP meetings for students assigned to the classroom. Write updates for each student's annual IEP in coordination with the state standards, school district expectations, and enter all IEP updates (Present Levels, BIP, ITP, and Goals) into the dedicated IEP documentation system prior to the meetings. Maintain documentation on IEP goals, student academic and social growth, prepare progress reports. Ensure documentation is accessible and provided upon request in a timely manner. Ensure confidentiality of records according to district and FERPA regulations. Collaboration: Work collaboratively with other staff and outside agencies to effectively educate each student. Assistive Technology: Support implementation and use of assistive technology including, but not limited to, augmentative communication devices. Instructional Planning: Plan instruction and design learning experiences based on data. Conduct summative assessments with each student in accordance with IEP goal reporting timelines. Administer, transfer, and interpret educational assessment data for supporting educational decision making and goal development. Professional Development: Participate in ongoing professional development activities as directed by the district including teacher and all staff trainings. This is a Part-Time Position with flexible work hours. 2nd shift availability - (12pm - 8:30pm) is required for this position ESSENTIAL DUTIES: Provide mobile response to a family identified crisis within one hour. Engage youth and families in de-escalating the crisis and assess for risk. Facilitate hospitalization for clients as necessary. Assist with the completion of the CANS assessment and help develop a safety plan and a MRSS plan. Provide ongoing stabilization services with the youth and families for up to six weeks. CRCC offers services in Cuyahoga, Ashtabula, Geauga and Lake counties such as legal advocacy, case management, and therapy services. We also do education and outreach work, to educate the community on sexual violence and human trafficking prevention. The Advocate is primarily responsible for providing trauma informed crisis intervention and ongoing support, and advocacy to survivors mainly in the judicial and civil systems, but also the medical and academic systems. The Advocate provides a broad range of services to empower a survivor including but not limited to educating and facilitating the survivor's decision making, serving as a liaison between agencies, accompanying/staying with the survivor during court appointments, and conducting safety planning. The Advocate will also assist with providing training for law enforcement, prosecutors, judges, and other community professionals. POSITION SUMMARY: The SAY Counselor/Social Worker provides prevention and early intervention services to middle and high school students in designated SAY School Districts. SAY services are designed to help prevent and reduce the use of alcohol and other drugs and decrease the incidence of other social and mental health problems such as substance use/abuse, teen violence, teen depression/anxiety and suicide. RESPONSIBILITIES INCLUDE: The SAY worker will work collaboratively with school principals, school counselors, teachers and other school personnel regarding the social, behavioral, emotional and environmental issues that impact student success. Students can be referred to the SAY worker regarding many teen related issues including but not limited to, alcohol/drugs/vaping, and issues/conflicts with peers, family, or school. The SAY worker will provide prevention strategies to help the student make healthy decisions and refer the student to a higher level of care if needed. The SAY worker will contact parents when necessary and will work with the parent to obtain a release of information so the SAY worker can communicate important information to necessary faculty and staff. The SAY worker will provide pertinent prevention education in the classroom as well as small group sessions for students that will help them learn positive coping tools, active listening skills, effective message sending, self-awareness, decision-making, communication strategies and conflict resolution. The SAY worker will attend parent programs in their school to provide education as well as serve as a resource to schoolwide programs on topics of concern and interest to parents/guardians and school personnel. When needed, the SAY worker will participate in broad, community wide education programs that promote positive adolescent health. The SAY worker will participate in professional trainings at Bellefaire that supports the goals of the program. The SAY worker will maintain timely completion of documentation in accordance with applicable licensing and accreditation regulations and standards. OTHER DUTIES: Attend scheduled staff meetings, supervision, and on-going training. All required trainings, certifications and licensure must be kept current in accordance with applicable licensing and accreditation regulations and standards. Maintain high standards of ethical and professional conduct and adhere to Agency policies and procedures. Other duties as assigned by management. QUALIFICATIONS: Education: Minimum Master's Degree in Social Work or related field. Licensure: Valid Ohio LSW or LPC required.
