Key Responsibilities:
Conduct individual and group psychotherapy sessions for patients in a subacute or skilled nursing facility.
Assess, diagnose, and treat a wide range of emotional and behavioral health conditions, including depression, anxiety, adjustment disorders, and cognitive decline.
Develop and implement evidence-based treatment plans tailored to the needs of each patient.
Collaborate with facility staff, interdisciplinary teams, and family members to support patient care.

Requirements:
Active and unrestricted license to practice as a Licensed Marriage and Family Therapist in State applying for.
Effective oral and written communication in English

Strong organizational and documentation skills, with attention to regulatory compliance.
Ability to provide in-person services at assigned facilities, up to 45 minutes.
Ability to walk, stand, and move between patient rooms and offices within the facility throughout the day.

Job Description
The Behavioral Health Advocate will provide counseling and advocacy services regarding all aspects of a participant's life, including addressing the needs of their family. Areas of focus will include mental, social, physical, and behavioral health. The counselor will maintain some flexibility in hours, which may include occasional evenings or weekends.
Duties include but are not limited to:
Assess Behavioral Health needs of clients and their families - using a comprehensive family assessment tool completed by clients at intake.
Help clients stabilize urgent needs and find appropriate providers for long-term services.
Provide short-term counseling/support services to interested clients.
Develop therapeutic goals with clients. Track progress and adjust goals as appropriate.
Provide intensive case management services to EARN and Work Ready participants. Act as the primary Case Manager when caseloads indicate a need for this. Otherwise, relevant case management will occur as needed with clients receiving counseling services.
Conduct small group sessions focusing on needs of participants.
Travel Required: Yes, local travel as needed.
Schedule: Monday - Friday (8:00am - 4:00pm)
External Hiring Range: $42,500 up to $45,050/year
Financial Incentives: After 90 days - financial assistance available for licensing fees, including Supervised Clinical Experience for candidates working towards clinical licensure.
