The Institute for Family Connections is currently hiring for a Permanency Specialist who will be working with youth and families participating in Hillside's Residential Treatment Services on the Monroe Avenue Campus. The Permanency Specialist is responsible for individualized permanency planning, which includes the delivery of Family Finding services, where extended relatives and fictive kin will be sought and gathered to support youths' legal, emotional and relational permanency. The person in this position will have continuous collaboration with County and community partners, Clinicians, and treatment team members, with the purpose of urgently locating and engaging family members and other significant adults, in order to build a lifetime network of supportive, caring people for each youth and family.
This is a Full Time position - 40 hours per week. Hours will vary throughout the week as they are flexible based on client need.
Essential Job functions
Implement the Family Finding and/or Child-focused Recruitment Model to fidelity with assigned youth.
Facilitate, scribe, and/or coordinate Permanency Roundtables for referred youth.
Engage youth, caregivers, relatives, fictive kin and/or other appropriate adults to increase connections and build a lifetime network of support for each youth and their families.
Support teams in determining the type of permanency intervention needed for youth.
Educate youth, families, providers, and strategic partners (i.e. DSS, OCFS) about the positive impact of Family Finding and/or Wendy's Wonderful Kids permanency services.
Participate and provide expertise in Service Plan Reviews, Treatment Conferences, and Benchmark team meetings as it relates to permanency.
Develop, maintain, and update all required documentation in a timely manner utilizing effective written and oral communication.

Institute for family connections Additionally:
Assess and address the permanency needs of children/youth, and/or partnering with families who want to provide permanency for children/youth.
Increase supports for youth and families enrolled in services within the Institute for Family Connections (IFC). For youth, this entails creating a lifetime network of support by locating and engaging family members and other significant adults to provide on-going support for the youth to achieve positive and successful permanency outcomes.
Create a strength-based permanency plan that includes home time, identifying and coordinating potential permanency resources and other natural supports, recognizing gaps and needs, assessing results and providing on-going communication with the youth's treatment team.
Active participation in agency Permanency Roundtables and serve as a Permanency Consultant to multidisciplinary treatment teams.
Education & Experience
Associate's degree in human services or a related field with minimum 2 of years relevant experience; OR
Bachelor's degree in social work, psychology, or human services required; OR
Bachelor's degree in any field with minimum of 2 years of relevant experience required
SPECIAL REQUIREMENTS
Unrestricted, valid NYS driver's license for minimum of 1 year with a clean driving record and minimum insurance coverage that meets agency standards.

The Transition Advocate will provide both advocacy and transition services to Rochester City School District (RCSD) students referred to/recruited into CCA's Transition Advocacy Program in Rochester. The Transition Advocate will work with students to both advocacy services for students and families who are referred to a RCSD process to address allegations that the student violated the Code of conduct, including but not limited to Superintendent meetings, and mediations with school personnel. Additionally, the Transition Advocate will provide transition services to students returning to
mainstream school from long-term absence. These absences may be due to a long-term suspension from school, a return from juvenile detention, or a return from jail, prison or juvenile placement.
Duties and Responsibilities:
Inform and ensure students and parents/guardians facing a superintendent's disciplinary hearing are aware of their rights and responsibilities as outlined in Education Law 3214 and RCSD's Code of Conduct
Provide support to students who have an Individual Education Plan (IEP) or a 504 plan
Participate in a Manifestation Determination Review as part of the disciplinary proceeding
Address underlying issues regarding a student's misbehavior in accordance with the RCSD Code of Conduct
Assist parents, students, hearing officers, and district staff (administrators, principals and teachers) in considering restorative interventions and supports that encourage educational success
Provide services for the following; informal hearings, informal conferences, short-term suspensions, restorative meetings, local MDR/CSE meetings, transition meetings and family or criminal court proceedings related to school suspensions
Support the transition for students who are returning to mainstream school following an absence due to a long-term suspension, a return from juvenile detention, or a return from jail, prison or juvenile placement
Conduct youth assessment through one-on-one meetings with the student, family members and school staff to review pertinent records to develop an individualized return to school plan.
Collaborate with school district staff to advocate for and support student success and to address any emerging or ongoing concerns
Follow-up to engage students for up to 6 months following their transition back to mainstream school, track progress and provide referrals to the school district and other community supports to ensure a successful transition to mainstream school
Participate in ongoing professional development and training opportunities
Maintain accurate record keeping via case files and designated computer database
Perform other duties as assigned by supervisor
Qualifications:
Bachelor's Degree preferred
Educational background in social work, education, policy, criminal justice or related field with excellent oral/written communication, and advocacy skills
A skillful motivational interviewer, with facility in cognitive behavior interventions, prior experience working with and a commitment to at-risk youth including justice system involved youth
Demonstrated cultural competency

Job Summary:
The Student Behavioral Assistant for the distance learning center plays a crucial role in supporting the academic success and positive behavioral development of students within the school district. When it is determined by the High School Principal that two Student Behavioral Assistants are not needed in the distance learning center on any given day, one person will be reassigned for the day to provide behavioral and academic support to students outside of the distance learning center.
Responsibilities:
* Supervision and Coordination:
* Supervises and coordinate the activities within the distance learning center to ensure a smooth and efficient operation.
* Collaborates with online teachers and building administration to create a schedule that accommodates the needs of students seeking assistance.
* Behavioral Support:
* Implements and enforces behavioral expectations within the distance learning center.
* Addresses and resolves behavioral issues promptly, applying appropriate interventions to maintain a positive and focused learning atmosphere.
* Student Engagement:
* Facilitates and promote student engagement in the distance learning center.
* Encourages active participation and collaboration among students to enhance the overall learning experience.
* Assessment and Progress Monitoring:
