We're seeking an exceptional Clinical Lead Care Manager (CLCM) to join our team. Under the direction of the ECM Clinical Manager, the CLCM serves as the client's primary point of contact and works with all their providers such as doctors, specialists, pharmacists, social services providers, and others to make sure everyone is in agreement about the client's needs and care. The CLCM manages client cases, coordinates health care benefits, provides education and facilitates member access to care in a timely and cost-effective manner. The CLCM collaborates and communicates with the client's caregivers/family support persons, other providers, and others in the Care Team to promote wellness, recovery, independence, resilience, and member empowerment, while ensuring access to appropriate services and maximizing member benefit.
This is a hybrid position that requires traveling throughout the Sacramento County area up to 5 days per week. Candidates wishing to be considered must reside within 25-miles of the assigned territory due to frequency of travel.

What you'll do
Hybrid (in-person and remote) care management duties as described below:
Assess member needs in the areas of physical health, mental health, SUD, oral health, palliative care, memory care, trauma-informed care, social supports, housing, and referral and linkage to community-based services and supports
Oversees the development of the client care plans and goal settings
Offer services where the member resides, seeks care, or finds most easily accessible, including office-based, telehealth, or field-based services
Connect clients to other social services and supports that are needed

Low-barrier services are designed to reach disconnected LGBTQ+ transitional-aged youth (18-24 years old) as the program is rooted in trauma-informed care and harm reduction models.
Housing Advocates work to reduce the traumatic impacts from being unhoused by creating a safe, inclusive and stable short-term housing environment where their basic needs are met. In partnership with their Case Manager, other program staff, and community partners, we are working as a team to support the client's capacity to achieve and maintain long-term housing stability, strengthen life skills and self-reliance, upon entry into our program.
ESSENTIAL FUNCTIONS:

Implement and improve recruiting strategies to increase student participation.

Contribute to new curriculum topics and adaptations to the needs of individual school sites.
Collect and encourage student voice and advocacy in program implementation and lesson topics.
Train and place volunteers to support mentoring activities.
Program reporting and administration
Complete internal direct assistance and incentive financial request according to program guidelines and client need.
Review, request, and reconcile supportive services.
Ensure timely and accurate compliance to all reporting requirements including database management, case noting, and file maintenance; and data reports as requested.
Collect, aggregate, and analyze performance data for initial program reports.
Conduct quality assurance checks on casefiles and in ETO.
Create new program forms and create manuals to capture best practices for program services.

Victim Advocate II is the journey-level class in this series. Victim Advocate II is the experienced level classification. General Qualifications
Criminal History/Background: The County may access criminal history information on candidates who have accepted a conditional offer of appointment for this class consistent with the provisions of Board of Supervisors Resolution No. 82-602, Personnel Policies and Procedures B-5, and applicable federal and state laws. The County shall not consider for employment any candidate who has been convicted of a felony or misdemeanor that relates to or impacts the candidate's ability to perform the job duties of this class unless it is determined that mitigating circumstances exist. For purposes of accessing criminal history information, the candidate will be fingerprinted. A subsequent arrest notification may be obtained.
The County may also conduct a background check on the candidate prior to appointment to a position within this class. The background check may include personal and professional reference checks, credit history checks where authorized by Labor Code section 1024.5, Social Security Number verification, professional license/registration verification, military service information and driving history. Information obtained in the course of this background check will be considered by the appointing authority in the selection process. Primary Job Duties
Daily facilitation and coordination of BYLP programs and youth engagement in executing the plan of the day in accordance with program policies and procedures.
Participate in the development and implementation of program service plans.
Assist youth with meeting program goals and personal development.
Serve as a positive role model, mentor and advocate for youth.
Maintain appropriate relationships throughout all interactions.
Attend All Hands meetings and trainings as required.
Assist youth in identifying interests and skills within community service opportunities and activities, as indicated.
Serve as an advocate for assigned youth. Meet with youth weekly and provide advocate notes regarding the youth's perspective and assessment of their respective week.
Assist in maintenance and cleanliness of the work environment.
Perform other duties as assigned.
