Guardian Scholars provides current and former foster youth with a partial college scholarship, mentoring, and individual support with the goal to increase their chance of academic success, graduation, employment, and ability to become a contributing member of society.
RESPONSIBILITIES:
Under the supervision of the Guardian Scholars Supervisor, support a caseload of approximately 25-35 high need youth requiring daily driving to meet youth where they are in the community.
Implement evaluation and assessment tools to ensure programmatic data and outcomes can be collected and analyzed for outcome management.
Support youth trainings and workshops, as needed.
Ensure Salesforce is up to date with accurate files, notes, and documentation. Salesforce must be updated daily per phone calls, emails, and visits.
Conduct monthly in-person meetings with youth to address challenges, track progress, and provide support, supplemented by additional check-ins via phone, text, or email as needed.
This position requires regular evening and weekend hours to meet participant needs.
Address any youth issues as they arise including career navigation, homelessness, and academic needs.
As part of the team, support youth development program components including but not limited to college and career tours, Women's and Men's Leadership Network, Advocates for Change, care packages, Mentoring Department, internships, Guardian Scholars Resource Fair, and youth training and events.
Assist in grant writing and fundraising activities as it pertains to Guardian Scholars.
Maintain knowledge of current laws, regulations, and resources pertaining to educating foster youth and make referrals.
Assist with youth recruitment to ensure agency participation goals are met.
Provide support to volunteer mentors to ensure their effectiveness with youth and satisfaction with the program.
Plan, coordinate, and attend occasional events to engage and build connections with youth.
Represent Promises2Kids in a professional and friendly manner at all times and develop a full understanding of Promises2Kids' mission and role in the community.

QUALIFICATIONS:
Lived experience in the foster care system preferred.
Bachelor's degree in social work, psychology or related field preferred.
2 - 3 years experience in case management and experience working with at-risk populations.
Proficient in public speaking and giving presentations.
Proficiency in computer skills, including entire MS Office Suite.
Demonstrate professional writing skills.
Excellent interpersonal skills, including the ability to work with a variety of people and represent Promises2Kids in a positive and professional manner to all staff, visitors, donors, and the community.

Program Description: NAMI San Diego Clubhouses operate under the Clubhouse International Model for psychosocial rehabilitation, with the goal of empowering adults living with mental illness to live vocationally meaningful and socially satisfying lives. Our program provides a uniquely supportive and collaborative Clubhouse setting where Members (program participants) and staff work together to operate all aspects of the organization in an atmosphere built on principles of mutual respect and caring.
Primary Responsibilities/Function:
As an integral part of the Clubhouse team, the SSI Advocate provides application assistance and education to individuals living with mental illness who are seeking to apply for SSI/SSDI benefits. The SSI Advocate works in partnership with the San Diego BH SSI contractor Program to track application progress, obtain additional documentations, and summit SSI/SSDI applications within a timely manner. The SSI Advocate actively contributes to Clubhouse programming by attending Clubhouse meetings, social events, and other activities designed to foster unique Clubhouse relationships. The SSI Advocate helps members connect with supportive resources within the community and may be required to provide transportation using company vehicles. To be successful in this position, the SSI Advocate must have excellent organizational and time management skills and be able to effectively communicate with all stake holders. This position is located at East Corner Clubhouse.
Duties and Responsibilities:
Provide Supplemental Security Income (SSI) application assistance and advocacy services as a component of the Clubhouse program including support with the initial applications, reconsideration, and appeals.
Create and maintain an SSI/SSDI profile for each individual application, including release of information, application progress, and supportive documentation.
Coordinate and collaborate with SD County BHS SSI Advocacy and Training Contractor program to determine client eligibility, track application progress, obtain additional documentation, and application process.
Manage multiple responsibilities and diverse personalities, while maintaining a positive and energetic attitude throughout the day.
Promote self-sufficiency by identifying and reinforcing member strengths, assessing appropriate support, and assisting members to access community resources.
Support Clubhouse programming as needed, by working side-by-side with members to build meaningful relationships and perform Clubhouse operations.
Participate in Clubhouse social programming, where evening and weekend coverage may occasionally be required.
Complete all training courses in a timely manner as required by NAMI SD/IC in accordance with our program contract, including the SD County BHS approved SSI Advocacy Training.

EDUCATION:

SUD Counselor III will perform counseling in individual and group setting, deliver curricula in accordance with program specific requirements and case management, and assist clients in navigating systems of care while providing a supportive treatment environment.
KEY RESPONSIBILITIES
Treatment Setting:
Maintain an assigned caseload of up to 25 clients, depending on the program staffing pattern and business needs, and provide individualized weekly counseling.
Facilitate group counseling sessions and community meetings.
Proactively link clients to both internal and external resources based on their treatment needs.
Teach classes on treatment-related topics to assist clients in meeting their treatment goals.
Collaborates with each caseload client and other available internal and external resources to develop/maintain treatment plans, transition plans, progress notes and appropriate updates in support of the health and recovery needs of the client.
Perform crisis intervention and communicate with the treatment team.
Follow up on clients' treatment progress.
Monitor and document client medication administration.
Ensure secure storage and proper accounting of client medications.
Accompany clients to offsite appointments as needed.
Assign client duties and sign off upon completion.
Motivate clients to fulfill their weekly obligations.
Support intake process and maintain supplies.
Collect, clean, or dispose of biohazard waste.
Disinfect and clean rooms and common areas.
Safety and Security:
Maintain safety and security of clients in the facility.
Perform periodic house runs.
Perform property and room searches.
Respond to medical emergencies including overdoses.
Perform substance use tests such as urinalysis and breathalyzer.
Observe clients and maintain observation logs.
Documentation:
Document group and individual counseling sessions in a timely manner per program and/or contract requirements.
Accurately enter data in electronic systems.
Document client updates and incidents daily.
All documentation must be maintained in accordance with guidelines established by HealthRIGHT 360 to satisfy internal and external evaluating requirements.
Complete discharge paperwork and agency assessments.
Maintain weekly logs of client contacts.
Read and respond to communications in a timely manner.
Administrative and Other Responsibilities:
Attend meetings and actively participate.
Meet residential service delivery goals.
Plan and facilitate celebrations and special events.
Work on weekends and holidays as needed.
Complete assigned training on time.
Maintain full compliance with certification requirements.
Assist with onboarding and training of new employees, interns, ambassadors, volunteers, etc. or as assigned.
QUALIFICATIONS
Education, Certification, or Licensure
High school diploma or equivalent required. Bachelor's degree in Health and Human Services, Social Work, Psychology, or other related field preferred.
Certification as a Substance Use Disorder Counselor from an accredited California agency (CCAPP, CAADE, CADTP).
Valid First Aid and CPR certification or ability to obtain within 30 days of hire.
Valid California Driver's License and access to registered and insured transportation.
Experience
Minimum of one year of experience as registered substance abuse or behavioral disorder counselor or recovery specialist in community mental health setting.
Experience carrying a caseload.

Program Description: NAMI San Diego Clubhouses operate under the Clubhouse International Model for psychosocial rehabilitation, with the goal of empowering adults living with mental illness to live vocationally meaningful and socially satisfying lives. Our program provides a uniquely supportive and collaborative Clubhouse setting where Members (program participants) and staff work together to operate all aspects of the organization in an atmosphere built on principles of mutual respect and caring.
Primary Responsibilities/Function:
As an integral part of the Clubhouse team, the SSI Advocate provides application assistance and education to individuals living with mental illness who are seeking to apply for SSI/SSDI benefits. The SSI Advocate works in partnership with the San Diego BH SSI contractor Program to track application progress, obtain additional documentations, and summit SSI/SSDI applications within a timely manner. The SSI Advocate actively contributes to Clubhouse programming by attending Clubhouse meetings, social events, and other activities designed to foster unique Clubhouse relationships. The SSI Advocate helps members connect with supportive resources within the community and may be required to provide transportation using company vehicles. To be successful in this position, the SSI Advocate must have excellent organizational and time management skills and be able to effectively communicate with all stake holders. This position is located at East Corner Clubhouse.
Duties and Responsibilities:
* Provide Supplemental Security Income (SSI) application assistance and advocacy services as a component of the Clubhouse program including support with the initial applications, reconsideration, and appeals.
* Create and maintain an SSI/SSDI profile for each individual application, including release of information, application progress, and supportive documentation.
* Coordinate and collaborate with SD County BHS SSI Advocacy and Training Contractor program to determine client eligibility, track application progress, obtain additional documentation, and application process.
* Manage multiple responsibilities and diverse personalities, while maintaining a positive and energetic attitude throughout the day.
* Promote self-sufficiency by identifying and reinforcing member strengths, assessing appropriate support, and assisting members to access community resources.
* Support Clubhouse programming as needed, by working side-by-side with members to build meaningful relationships and perform Clubhouse operations.
