The CertifiedNursingAssistant is responsible to provide assistance to the support staff with the daily activities of individuals, provide ongoing support, guidance, role modeling and providing assistance in meeting clients medical and health need.

Provide support in a manner conducive to their safety and comfort, under the direction of an RN and Program Manager.
Perform all duties in accordance with established methods, techniques and recognized standard.
Assist program support staff in completion of all daily/shift tasks, including but not limited to oversight of individuals' medical and health needs: hospitalization, appointments, assessments, discharging, transferring and caring for the residents' personal belongings.
Provide ongoing support, guidance and role modeling for individuals.
Assist individuals with home maintenance and perform cleaning duties as assigned.
Perform Activities of Daily Living (ADLSs) for residents as required.
Coordinate scheduling for residents requiring occupational therapy, physical therapy and speech therapy.
Take and record weights, blood pressures, temperatures, pulse and respiration and give skin care treatments, as permitted by state regulation.
Report on resident status or family concerns to RN and Program Manager.
Ensure all paperwork, reports, trainings are up to date and completed within the Divisions expected timelines.
Ensure that residents and families receive the highest quality of service in a caring and compassionate atmosphere, which recognizes the individual's needs and rights.
Attend and actively participate in all trainings as assigned and maintain certification in CPR, First Aid, crisis management, and MAP
Qualifications
CertifiedNursingAssistantCertificate Required. Experience with the program population is preferred.
Ability to write routine reports and correspondence.
Strongly prefer that a candidate will have a demonstrated understanding of and competence in serving culturally diverse populations.
Ability to read and interpret documents such as policy and procedure manuals.
Strong computer knowledge.
High energy level, superior interpersonal skills and ability to function in a team oriented environment and as well as work independently
Ability to execute a variety of decision-making models.

Encourage family involvement through volunteering and visitation.

Performs personal services for customer's (i.e., reading, writing letters, etc.) as time allows.

Assists with care of dying and expired customers.
Records resident food/fluid intake. Completes meal monitor form as indicated. Reports changes in resident's eating habits to the Staff Nurse.

Job Description
**This recruitment is for multiple positions at all DPH facilities, including Laguna Honda Hospital and Rehabilitation Center and Zuckerberg San Francisco General Hospital and Trauma Center.**
Under direct supervision of a licensed nurse, the CertifiedNursingAssistant (CNA) performs a variety of routine duties involving care of medically or mentally ill acute or sub-acute patients; performs direct patient care; observes and reports changes in the behavior and physical status of patients; provides a clean and safe environment; and performs related duties.
The essential functions include, but are not limited to:
Under direct supervision, and as part of the treatment team, provides routine patient care in accordance with established policies, procedures and standards.
Establishes and maintains a therapeutic and/or medical relationship with patients.
As appropriate, reviews patient's admission documentation and notes pertinent facts regarding diagnoses and treatment history. Documents care according to department standards.
Maintains constant and continuous communication with medical providers and staff.
As directed by licensed staff; reviews and implements treatment plans, and provides one-to-one care when essential to treatment plans.
Observes, records and reports patient medical, social and psychiatric behavior and physical conditions.
Participates in multidisciplinary team meetings and treatment planning by reporting observations and concerns and by asking questions.
Attends and participates in patient groups, case conferences, staff meetings and in-service training programs.
As assigned, completes procedures and specimen collection including phlebotomy.
Adheres to health, safety, and infection control policy and resource utilization.
Manages work time effectively, efficiently and productively.
The CertifiedNursingAssistant may perform other duties as assigned/required.

License and Certification:
Possession of a current CertifiedNursingAssistant (CNA) certificate issued by the California Department of Public Health**

Possession of a Certified Phlebotomy Technician 1 (CPT-1) Certificate, issued by the State of California Department of Health Services, is required for those positions that perform phlebotomy.

Training and Experience Evaluation (Weight: 100%): Candidates can complete the Training & Experience Evaluation now. It will also be emailed to candidates meeting the Minimum Qualifications. This evaluation is designed to measure knowledge, skills and/or abilities in job related areas which may include but not be limited to: Knowledge of treatment tools for medically and mentally ill acute and sub‐acute patients, of the principles of patient care duties, of basic medical terminology; Oral communication ability; Written communication ability; Ability to observe and identify changes in patient's condition, to assist patients in and out of bed, into bath, and in and out of wheel chair, to interact effectively with patients, family members, and clinical staff.
Candidates must achieve a passing score on the Training and Experience Evaluation to continue in the selection process and will be placed on the confidential eligible list in rank order according to their final score.
Additional selection processes may be conducted by the hiring department prior to making final hiring decisions.

Eligible List/Score Report
A confidential eligible list of applicant names that have passed the civil service examination process will be created and used for certification purposes only.
