Advocates is seeking enthusiastic, driven LPNs to offer medical support, care coordination and rehabilitation options to those in crisis.
The LicensedPracticalNurse is responsible for the provision of direct care and nursing care to meet identified individuals health maintenance needs. He/she works as part of a multi-disciplinary team to support people at the program around all domains of living and to support the achievement of goals identified by individuals in Respite.
Minimum Education Required High School Diploma/GED Responsibilities
Assists with rehabilitation of residents and health maintenance in accordance with the Individual Action Plan.
Uphold in all areas of work the philosophy of the Advocates Way and the Intentional Care Standards while serving people at the program.
Contribute to assessment, individual risk management/safety planning, discharge planning, and care coordination.
Assists the Nurse Manager and individuals staying at the program with administration of medications and therapeutic treatments according to site policies and procedures as well as the person's abilities.
Assists with the education of residents and staff in areas such as medication management, checking pulse, temperature, blood pressure and blood glucose levels.
Implements interventions and documents progress according to program policies and procedures.
Reports changes in resident's status or unusual activity/occurrence to the Nurse Manager or Clinician On-Call in a timely and appropriate manner.
Maintains documentation/record keeping in accordance with agency and program guidelines.
Advocates for residents and coordinate with other healthcare providers.
Utilizes a person centered rehabilitation and recovery perspective to modify the approach to each individual person.
Ensure an environment that promotes safety, recovery, and treatment.
Attends and actively participates in supervision, staff, team, and family meetings
Attend trainings as assigned; maintain necessary certifications (CPR, First Aid, SOLVE, MAP).
Ensure that individuals we support are treated with dignity and respect in accordance with Advocates' Human Rights Policy.
Perform all duties in accordance with the agency's policies and procedures.
Qualifications
Graduate of an approved nursing school with current licensure in the Commonwealth of Massachusetts as a LicensedPracticalNurse and 1 year experience, experience with people with diagnosed mental health issues who live in the community preferred.
Must be able to perform each essential duty satisfactorily.
Ability to communicate effectively verbally and in writing and ability to use good judgment.
Must have basic computer knowledge.
High energy level, superior interpersonal skills and ability to function in a team atmosphere.
Must hold a valid drivers' license, access to an operational and insured vehicle and be willing to use it to transport individuals.
Demonstrated understanding of and competence in serving culturally diverse populations.

Position Summary
Under the direction of the Director of Nursing or designee, the incumbent will provide direct nursing care by assessing, planning, organizing, documenting, administering, and evaluating nursing care services for intellectually and developmentally disabled adults living in Residences provided by The Arc Rockland. Care delivery will be done in compliance with rules and regulations of the NYS OPWDD and The Arc Rockland within the scope of the practice and assessed as such. The Registered Nurse directs and guides the activities of other nursing-related personnel while maintaining standards of professional practice and the established policies and procedures of the Clinical Services and Residential Services Departments.
Role's Responsibilities
• Performs clinical/physical assessments and triage.
• Implements plan of care in collaboration with other disciplines. Evaluates plans of care using interdisciplinary approach and updates plans as needed.
• Arranges medical services for supported individuals.
• Provides care direction as needed to LPN and DSP staff based on the individual's plan of nursing care.
• Supports the interdisciplinary approach to individual care as well as the team process and participates on the team as directed by the Director of Nursing or designee.
• Ensures that all individuals' medical records are in compliance with all applicable regulations.
• Provides restorative and rehabilitative services as necessary, i.e., bowel and bladder training, ostomy care and delivery of oxygen therapy.
• Performs on-going surveillance and nursing intervention to rescue chronically ill individuals in the agency's care from the development of negative side effects and secondary results of treatment.
• Takes immediate action, upon discovery of a medical issue, including notifying the • Director of Nursing or designee and Residential Services management.
• Gives medications and treatments as indicated.
• Takes and records vital signs.
• Provides oversight to AMAP certified staff, which includes trainings and in-servicing pertaining to regulations/OPWDD requirements.
• Participates in and/or directs the rendering of first aid and emergency care as needed or directed.
• Maintains nursing office(s) with supplies as needed, including, but not limited to, OTC medications and durable medical equipment.
• Maintains nursing office/nursingpractice in compliance with OSHA guidelines.
• Utilizes principles of infection control when caring for supported individuals who attend community programs.

Role description
Under supervision and according to physicians' or registered nurses' instructions, prepares, administers and records ordered medications and injections; obtains patient histories; observes, reports and records physiological and psychosocial signs; takes vital signs; maintains medical records; maintains a safe and clean environment, and performs related duties as required.
* Administers nursing care to patients according to physician's and Registered Nurse's instructions and approved nursing and departmental policies and procedures; as a team member, participates in planning and maintaining nursing care plans; assists physician in the performance of diagnostic and therapeutic procedures and treatments; prepares, administers and records ordered medications and injections.
* Provides direct patient care in order to make patient clean and comfortable; may serve and feed patients; may assist patient with elimination functions.
* Screens patients by obtaining histories; observes, reports and records physiological and psychological signs, symptoms and behaviors; takes vital signs; obtains specimens and may perform some required nursing diagnostic tests, such as guiac stools and sugar and acetone tests, etc.
* Keeps accurate records of administration of medications and treatments; makes reports of patient's progress towards goals; maintains medical records in order to meet legal requirements and provide reference for follow-up of patients' treatment and care.
* Maintains a safe, secure and clean environment; maintains adequate supplies and equipment in assigned area.
* Instructs and counsels patients relative to physical self care and emotional needs concerning test and specimen collections.
* Understands human relationships between patients, families and other health care personnel.
* Recognizes and understands the effect of social and economic problems as well as cultural backgrounds and religious affiliations of patients and their families.
* When working in specialized areas, and after training in special procedures pertaining to such areas, performs specialized procedures as approved by nursing service policy.
