The RegisteredNurse (RN) participates in the coordination of medical, behavioral health/addiction treatment services and other supports in accordance with the person-centered treatment plan and works collaboratively with the BHCP team at their respective agency to manage or co-manage complex behavioral health cases. As a member of the multidisciplinary team, the RN provides consultation, education, and support to team members and provider network regarding co-occurring medical issues related to behavioral health/substance abuse treatment and management.
Minimum Education Required Bachelor's Degree Responsibilities
Support consortium entity teams by providing supervision, conducting audits, and actively participating as a member of their team to ensure high-quality care coordination and compliance with best practices.
Provide medical consultation to BHCP team members in the assigned consortium entity (CE).
Liaison to and consult with the BHPMW BHCP Medical Director.
Conduct medication reconciliation functions required in the BHCP Model Contract.
Review medical sections of Engaged Enrollee's comprehensive assessments, and/or the entire assessment as assigned.
Participate in BHCP team meetings to ensure effective communication among all disciplines.
Participate in all required orientation and training and assist with training other staff on the CP Team.
Learn all BHCP policies, procedures, protocols and plans and deliver services and consultation in compliance with them. Assist with training other staff on the CP Team.
Develop knowledge about all focus populations and assist CP Team members in doing so.
Learn evidence based practices identified by the BHC, and deliver CP supports and activities in compliance with them, and assist with training other CP Team members.
Complete all required documentation in a timely manner.
Demonstrate a proactive commitment to maintaining effective communication with staff, Enrollees, Care Team members and other collaterals, and other internal and external stakeholders and customers. Promote strong working relationships and excellent customer service.
Complete required trainings and other professional development activities.
Strictly adhere to HIPPA and confidentiality policies at all time, including when responding to inquiries for Enrollee information
Attend and actively participate in supervision and staff meetings.
Maintain current certifications and licenses
Ensure that all clients are treated with dignity and respect in accordance with BHPMW's Human Rights policy.
Perform all duties in accordance with the agency's policies and procedures.

Qualifications
RegisteredNurse licensed in the Commonwealth of Massachusetts with minimum of 4 years of experience working with adults in a community-based mental health, substance use and/or physical health setting
Demonstrated knowledge base and judgment necessary for independent clinical decision making.
Excellent verbal and written communications skills. Strong interpersonal and customer relations skills required.
Excellent organizational, time management and problem solving skills.
Strong computer knowledge, including proficiency in contemporary Windows operating systems and Windows office suites with an emphasis on Word and Excel; ability to learn new systems; experience entering and working with data; and comfort and experience using mobile technologies.
Knowledge regarding psychiatric rehab and understanding of recovery model.
Strongly prefer that a candidate will have a demonstrated understanding of and competence in serving culturally diverse populations.
Must be able to perform each essential duty satisfactorily.
Must hold a valid drivers' license. Must have access to an operational and insured vehicle and be willing to use it to transport members.
Must have ability to read English and communicate effectively in the primary language of the program to which he/she is assigned.

Position Overview

Paid Bi Weekly

MINIMUM QUALIFICATIONS:
* Must be willing to obtain MDS Certification within first year of employment.
* Must possess a current Florida licensure as a Registered Professional Nurse.
* Must possess a Bachelor's degree from an accredited college or university with a major in nursing or a related field and two (2) years of nursing experience - OR - have six (6) years of professional nursing experience - OR - possess an Associate's degree and have four (4) years of professional nursing experience.
* Availability to travel 75% includes overnight required.
* Successful completion of the due diligence process to include, but not limited to, a Level II Background check and drug screen.
PREFERRED QUALIFICATIONS:
* At least one (1) year of experience working in a long-term care facility with a geriatric population.
* Proficiency with Microsoft Office Suite, especially Outlook, Word, and Excel.
* Basic experience and familiarity with MDS and Care Plans.
* Infection Control Preventionist Certification.
Position Description:
Serve as a liaison between nursing staff and executive leadership of the FDVA Homes Program.
Ensure safe, effective, and evidence-based nursing care.
Monitor resident outcomes and quality measures.
Coordinates and oversees all functions of the FDVA Homes Program's MDS Assessment process to assure compliance with Federal and State requirements. Performs audits to assure prompt completion and transmission of the required MDS Assessment.
Coordinates with Billing to assure accurate RAI-MDS and billing process.
Develops and provides educational programs to the Interdisciplinary Team members.
Prepare for and participate in regulatory surveys and inspections.
Assist in identifying and correcting deficient practices.
Ensure staff competency and licensure compliance.
Review and update nursing policies, procedures, and standards of care.
Oversee infection control and patient safety programs.
Performs Other Duties as Assigned.

Strong analytical and critical-thinking skills.

Position Overview
The Illinois Department of Public Health is seeking a highly motivated and experienced TB nurse consultant to join the TB program and represent the department in the southern region of the state. This individual will have TB case management experience and be willing to support TB programming within the region including travel for education and monitoring. In addition this position will be responsible for the implementation of the Training & Education Network Program of TB Nurses throughout the state.

Essential Functions
* Plans, directs, and coordinates the implementation of program activities in the southern tuberculosis (TB) region.
* Functions as the program liaison to health providers documented as being out of compliance with recommended diagnostic, therapeutic, or patient management procedures.
* Monitors local agencies for adherence to IDPH promulgated standards for local health departments.
* Plans, coordinates, and implements training programs for professional and non-professional staff involved in TB control and management.
* Provides technical supervision to professional staff within local health agencies in complying with program guidelines.
* Performs other duties as required or assigned which are reasonable within the scope of the duties enumerated above.
Minimum Qualifications
* Requires knowledge, skill and mental development equivalent to completion of four (4) years of college.
* Requires three (3) years of progressively responsible administrative experience in nursing.
* Requires licensure as a RegisteredNurse in the State of Illinois.
Specialized Skills
* Requires three (3) years of experience in TB case management.
Preferred Qualifications
* Three (3) years of experience in TB case management.
* Two (2) years of experience working with public health officials.
* Three (3) years of experience working with Microsoft Office products.
* Two (2) years of experience working with infectious disease outbreak/contact investigations.
* One (1) year of experience working with electronic health records.
