SUMMARY
Under general supervision, is responsible for providing medical treatment, first aid, emergency care and medications, under the direction of a Registered Nurse and in accordance with ADAPT Community Network policies, the regulations and requirements of applicable regulatory agencies and the standards of the profession.
ESSENTIAL DUTIES AND RESPONSIBILITIES
The duties and responsibilities of the Licensed Practical Nurse - Residential will include but are not limited to the following:
Serves as a member of interdisciplinary team and is responsible for assisting Registered Nurse in coordinating following through on provisions of health care services for assigned residences.
Keeps progress notes under supervision of registered nurse for each client and summarizes for quarterly, semi-annual and annual reviews.
Provides health care to individuals receiving services as instructed by registered nurse. Provides direct hygiene and health instruction to individuals receiving services and families as required.
Handles control of communicable diseases and infections through identification and assessment, reporting any finding to registered nurse and consulting physician, implementing appropriate protective and preventive measures and monitoring infection control practices.
Provides first aid to persons in need of immediate care, with referral to registered nurse or outside emergency services as necessary.
Assists physicians in medical examinations as needed.
Maintains a tickler for all appointments. Schedules appointments and follows up on physicians' recommendations.
Keeps up to date and accurate records of examinations, immunizations, illnesses, accidents and other pertinent data via progress notes.
Administers medications when appropriate. Supervises storage of medications and assists in oversight and ordering of medication.
Assists registered nurse in maintaining AMAP re-certification files.
Obtains consent for surgery of residents in absence of registered nurse.
Refers all consults and diagnostic reports to registered nurse for review.
Attends and transports clients to appointments as necessary.
Attends team meetings as scheduled.
Attends to clients' medical conditions.
Carries beeper and is on-call, responds to questions regarding physician and RN treatment plans.
Keeps abreast of changes in Nursing field.
Performs other related duties as requested.
QUALIFICATIONS

Ability to read, write, speak and understand English.
Good oral and written communication skills.

Knowledge of equipment typically used by persons receiving services, including wheelchairs and adaptive devices.
Must be willing to work at various program sites.
EDUCATION and/or EXPERIENCE
Graduate of an accredited program of professional nursing.
At least one year of experience as a Licensed Practical Nurse in the delivery of community-based health services to individuals with developmental disabilities.
License and current registration as a Licensed Practical Nurse in the State of New York.

DEPARTMENT: Primary Care
Status: Unpaid (For Credit or Experience only)
The Nurse Practitioner provides continuous and comprehensive health care which entails the coordination of all services necessary for health promotion, maintenance, rehabilitation and the prevention of disease and disability.
ESSENTIAL FUNCTIONS:
Perform initial medical assessments including baseline history specific to HIV illness
Provide health promotion and disease education specific to the needs of the AIDS client
Provide and facilitate health maintenance activities
Provide health education including but not limited to education about HIV, opportunistic infections, prophylaxes, and medication adherence
Assess clients' needs and provide counseling about medical needs
Provide well care, sick call, monthly physical examinations, facilitation and monitoring of hospitalizations and post-hospital follow-up care
Diagnose, manage and treat common illnesses and acute conditions
Work collaboratively with the clients' primary care providers and other specialties to ensure the client's medical and psychiatric needs are met
Facilitate the Comprehensive Care Planning process
Present client cases to the inter-disciplinary team for conference and care planning
Prepare reports and statistical forms
Supervise staff nurses
Prepare lesson plans for structured group activities
Initiate and participate in research related to the health care of AIDS clients, as assigned
Participate in continuing education programs appropriate to practice
Participate in morning rounds
Perform all related duties, as assigned, or unrelated duties, as requested, during emergencies
Assumes joint responsibility for ADHC services as need
