Job Description
Performs the duties required to provide safe, efficient care and services to patients. Functions as a member of the health care team providing assistance in planning and implementation of patient-based care based upon sound nursing judgment, clinical preparation, knowledge, skills and experience.
POSTION QUALIFICATION REQUIREMENTS
Education: Graduate of an accredited school of practical nursing.
Experience: Requires (4) four years' experience working in a community health setting; Experience coordinating clinic services.

FUNCTIONS OF THE JOB
Essential Functions
Serves as a nursing source for clinical staff and may serve as the key clinical contact in the absence of the Health Center Assistant Director by providing support to clinical staff scheduling and competency oversight of entry level staff.
Tracks, reviews, and monitors follow-up of diagnostic test results, under the direction of the provider. Provides related education, counseling, ongoing review, and monitoring regarding diagnostic tests. Assists in the management of patient care between visits.
Performs specialized procedures, examinations and/or tests as ordered.
In collaboration with physician, provides extensive health education, counseling and advice to patient and families on topics such as life style changes, diabetic controls, etc. Effectively assesses patient educational needs, prioritizes patient's learning needs, identifies barriers to learning, develops teaching plan, and uses various educational tools to activate the non-compliant patient.
Assists Health Center Assistant Director with patient flow. Acts as patient advocate to ensure needs are met.
Obtains and documents reason for visit, patient vital signs, including temperature, blood pressure, weight, respiration, pulse and BMI.
The following defined protocol independently assesses patient/callers' health status based upon presenting symptoms. Determines the urgency for care and directs patient/caller to most appropriate level of care, or an acceptable alternative venue for care of patients with acute problems should access not be readily available at the clinic. Seeks provider advice appropriately.
Use computer knowledge to assess patient information, follow-up appointments, e-mail, etc.
Performs other duties as assigned.
Marginal Functions
Monitors clinic supply system and inventory and orders supplies as indicated.
Monitors completion of medical records for clinic session and patient confidentiality.
Monitors crash cart supplies for outdated medications and supplies in medication room.

FUNCTIONS OF THE JOB
Essential Functions
* Performs patient intake (i.e., blood pressures, temperatures, pulse, respirations, weights and measurements) on patients for medical examination.
* Perform tests (i.e., pulse oximetry, vision tests, hearing screens and EKGs).
* Gives medications/immunizations as ordered per route (oral, intramuscular, subcutaneous, and subdermal).
* Notifies Clinic Nurse Coordinator and Provider of abnormal vital signs, blood glucose, or EKG results.
* Prepares, maintains exam rooms assuring that they are cleaned and stocked; performs sterilization of instruments.
* Assists urgent care patients by monitoring vital signs and assisting with transportation to another department or facility.
* Responsible for general office procedures (i.e. nebulizer treatments).
* Perform other duties as assigned.

* Assist with primary care appointments.
* Assist with orientation and training of new Nurses, Medical Assistants, students, and volunteers.
