The agency was originally created in 1989 as the Agency for Health Care Policy and Research in the Department of Health and Human Services. It adopted the name Agency for Healthcare Research and Quality in 1999.

In 2017, a study (link is external) published in JAMA Internal Medicine followed nursing homes that adapted CUSP guidelines to reduce catheter-associated urinary tract infections (CAUTIs). CAUTIs are common in nursing homes and can result in severe illness, leading to large costs.
However, though Americans spent $3.65 trillion on health care in 2018 (link is external), AHRQ received just $333 million in federal funding (link is external). That was less than 0.01% of total federal spending (link is external).
