How You'll Make a Difference (Essential Functions)
The CNA Homecare (HC) is responsible for patient and family satisfaction by assisting with activities of daily living, providing light homemaker help, following the plan of care, communicating changes in condition, and documenting care. The CNA in home care (IHC) provides patient and family centered care consistent with Agrace philosophy, goals, standards, regulations and policies.
Key Responsibilities
Assist Patients with Activities of Daily Living
Assist with ambulation, changing position, and moving to/from sitting and standing.
Help with bathroom functions like toileting and incontinence care.
Help bathe, shower, dress, provide oral care, and groom.
Assist with eating and drinking.
Perform post mortem care.
Obtain vital signs. Assist with treatments.
Anticipate and respond to patient needs.
Provide homemaker assistance
Perform light housekeeping such as changing linens, tidying room, doing laundry, and removing trash.
Order and deliver patient supplies.
Help prepare simple meals.
Communication
Read, understand, and follow the plan of care. Document cares and changes in patient condition in electronic health record.
Document and report significant change in condition or unusual incidents to appropriate Interdisciplinary Team Member such as client abuse/neglect, suicidal intent, harmful coping mechanisms or violence, etc.
Reinforce patient, family, and facility partner education.
Miscellaneous
Participate in meetings to plan and evaluate patient/family services.
Precept new staff and students.
Delegate tasks to volunteers as appropriate
You Bring (Qualifications)
High school diploma or GED, or equivalent combination of education and work experience
Graduation from an accredited CertifiedNursingAssistant program
Current, unrestricted NursingAssistantCertification in state of employment
One-year personal care experience, hospice, or home health, preferred
Valid driver's license, auto insurance, and access to a reliable vehicle

Job Summary
The NursingAssistant (NA) serves as caregiver to the resident during the scheduled work period in long term care. Provides resident-centered nursing care and daily living assistance to assigned resident under the supervision of a registered nurse (RN). Knowledge of and delivers age-appropriate care related to the physical and psychological needs of the resident as per care plan. Considered a member of the nursing team and is expected to know, and will be held accountable for, following infection prevention and control policies and Personal Protective Equipment use. Provides assistance with basic health care needs including daily living activities that may include, but are not limited to, bathing, toileting, grooming, dressing/undressing, obtaining and recording vital signs, and providing psychosocial support and other personal care to assigned resident. Assists the resident in transferring, repositioning, and walking using correct and appropriate transfer techniques and equipment and also provides range of motion and passive exercises. Completes tasks related to food preparation and dining, while creating a positive dining experience for the resident. Assists residents with meals and snacks, provides water and supplements as appropriate.
The NA aids providers and nursing staff members with procedures, if needed. Documents resident interactions as needed and the outcomes related to resident care, behavior, activity, and dietary. Communicates resident's changing condition and care related concerns/responses to the charge nurse. Collects non-invasive body fluid specimens and gathers vital signs. Provides housekeeping and laundry services to assist residents with keeping living areas clean and operational. Qualifications
High school graduate or general equivalency diploma (GED) preferred, but not required.
Minimum age of 16. Prior clinical or nursingassistant experience in long-term care preferred.
Required CertifiedNursingAssistant (CNA) certification with the State Board of Nursing, or state certified CNA within four months of date into the position by being enrolled in a training program or in the process of obtaining certification through Endorsement. CNA training and certification required and provided by facility.
Satisfactorily completes yearly department specific competency testing and maintains a record of continuing education credits for each year. A minimum of 12 hours continuing education are required annually. Maintains all department specific required certifications.
When applicable, for applicable states, certified and registered with the Unlicensed Assistive Person (UAP) Registry.
Dependent on facility and required responsibilities, this role may classify as a Sanford Category II Driver requiring a motor

*Weekend rotation, every 3rd. Holiday rotation.
Provides clerical support for assigned nursing units and performs NursingAssistant duties under the supervision of the Registered Nurse. Schedules diagnostic tests and therapists for patients and acts as a receptionist and resource person on the unit. May be assigned one role per shift or may be required to perform both roles during assigned shift. High School diploma or Certificate of General Educational Development (GED) or High School Equivalency Diploma (HSED) required.
