Requirements
Patient Care
1. The Hospice Team Nurse provides professional nursingcare to patients by utilizing all elements of nursing process.
2. . Assesses and evaluates patient's status
3. Initiates the plan of care and makes necessary revisions as patient status and needs change.
4. Develops a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions. Includes the patient and the family in the planning process.
Acts as Case Manager when assigned by Clinical Supervisor and assumes responsibility to coordinate patient care for assigned caseload.
Communication
1. The Hospice Team Nurse completes, maintains and submits accurate and relevant clinical notes regarding patient's condition and care given. Records pain/symptom management changes/outcomes as appropriate.
2. Communicates with the physician regarding the patient's needs and reports changes in the patient's condition; obtains/receives physicians' orders as required.
3. Communicates with community health related persons to coordinate the care plan.
POSITION QUALIFICATIONS
1. Graduate of a National League of Nursing accredited school of nursing.
2. Current licensure in State of CA and CPR certification.
3. Registered nurses shall have a minimum of one (1) year of experience as a professional nurse within the last three (3) years; OR have a baccalaureate degree in nursing from a program accredited by the National League of Nursing and a current RN license.
Minimum of two (2) years' experience, at least one of which is in the area of public health, home care, or hospice nursing is preferred.
4. Management experience not required. Responsible for supervising hospice aides.
5. Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; nursing skills per competency checklist.

PRIMARY FUNCTION
The Admissions Registered Nurse is an Interdisciplinary Team member whose primary responsibility is to present ANCHOR HEALTH services and philosophy to patients and families in a manner that results in the admission of appropriate patients.
JOB RESPONSIBILITIES FOR THE ADMISSIONS REGISTERED NURSE
1. Admissions Registered Nurse participates in the information gathering, analysis, education, and form completion aspects of the admissions process
2. Admissions Registered Nurse evaluates patients for hospice appropriateness, e.g. medical records review as requested.
3. Admissions Registered Nurse Obtains agreement for admission of referred patients and families who meet admission criteria.
4. Admissions Registered Nurse analyzes data collected by the admission staff or ANCHOR HEALTH clinicians to facilitate/obtain admission orders
5. Admissions Registered Nurse builds effective referral source relationships to support the high quality and responsiveness expectations associated with ANCHOR HEALTH.

Position Summary:
The Wellness Nurse provides nursingcare and supervision to residents. This includes direct resident care, conferring with other disciplines, referring to community resources and/or utilizing resource materials as needed. This position participates in care conferences and staff meetings, completes paperwork in a timely manner, and communicates with health care providers, support staff, and other community persons as needed or directed. The Wellness Nurse is also responsible for conducting, coordinating, and tracking training for community care staff.
The starting salary for this position ranges from $40.00 to $45.00/hour. Position Responsibilities include:
Demonstrates appropriate physical assessment skills for initial and follow-up appraisal of resident status. Possesses an adequate knowledge of clinical disease entities.
Demonstrates ability to perform nursing treatments and procedures as prescribed for assigned residents. Performs those treatments in an appropriate manner.
