The organization has four acute care hospitals and one specialty hospital that together house over 1,800 hospital beds. Position Information: TheHeart Care Transition Clinic is a specialized medical office for cardiac patients recently discharged from the hospital to receive follow-up medical care. The Heart Care Transition team works with the cardiologist to ensure patients get the follow-up care needed in a timely manner. These clinics focus on patients with a diagnosis of Heart Failure, AFib, and other chronic cardiac conditions to provide follow-up care, medication and diagnostics review and focus on in-depth patient education as a means to assist patients with diet and lifestyle changes as a means to liaise care back to the patient primary cardiologist and mitigate readmissions. This candidate would possibly be rotating between the Heart Care Transition Clinic and rapid diuresis.This position is primarily based out of our Metro Parkway and HealthPark Medical Center, however training and orientation will require floating to all 4 of our acute care facilities. The schedule is Monday - Friday with 8-hour shifts (the practice may consider a 4-day work week with 10-hour shifts for the right candidate. There is no call and no weekends!
