* JOB GOALS:
To support the delivery of School District 91's written, taught, tested and reported curriculum and contribute to maximizing student achievement.
To work closely with special education students on a regular basis to provide them with the support they need to gain full benefits from the District's special education program.
* ESSENTIAL DUTIES AND RESPONSIBILITIES:
* Helps the teacher provide a well-organized, smoothly functioning class environment in which students can take full advantage of the instructional program and available resource materials.
* Prepares for classroom activities under the supervision of the Teacher.
* Works with small groups of students to reinforce materials initially introduced by the teacher.
* Assists individual children in need of special attention and children who need consistent behavior management techniques.
* Performs clerical duties relating to their assignment.
* Guides independent assignments established by the teacher.
* Assists teacher with non-instructional classroom duties.
* Checks notebooks, corrects papers, collects data, and supervises testing and make-up work.
* Assists in duties relating to the supervision of students as assigned by the Building Principal.
* Helps prepare class displays and bulletin boards.
* Constructs supplementary materials for use in the classroom under the teacher's supervision.
* Strives to improve and maintain job skills through inservice training programs and other means.
* Maintains high standards of ethical behavior and confidentiality.
* Has regular and predictable attendance.
* OTHER DUTIES AND RESPONSIBILITIES:
* Performs all other duties as assigned.
* TERMS OF EMPLOYMENT:
This is a limited full-time position scheduled for 28.75 hours per week during the school year. Wages will be in accordance with the Classified Salary Schedule as a Grade 5 - $16.39 or $17.19 per hour depending on experience. Benefits will be in accordance to Board Policy.
This is an "at will" position in that the employer or employee may terminate employment at any time or for any reason consistent with applicable state or federal law. The "employment at will" policy cannot be changed verbally or in writing, unless the change is specifically authorized in writing and expressly approved by the Board of Trustees of District 91.
* EVALUATION:
Performance of this position will be evaluated annually by the Building Administrator in accordance with provisions of the Board's policy on the evaluation of classified personnel.

Required Skills, Knowledge, and Abilities: The Mid Level Provider's scope of practice will be consistent with all applicable state laws, regulations, and scope of practice for Mid Level Providers is further defined in the Standardized Policies and Protocols. The Mid-Level Providers will utilize their individual assessment and health care management skills under the guidance of and according to the policies of the Physician Medical Directors.
ESSENTIAL FUNCTIONS
Diagnose and Manage minor illnesses or stable chronic conditions. Able to evaluate, diagnose and treat patient's physical and/or psychosocial health status. Instruct, educate, and counsel patients on health status, test results, disease process, and plan of care. Monitor the efficacy of therapeutic interventions. Provide timely dictation and documentation with respect to charting. Utilize individual assessment and health care management skills in conjunction with Clinic Scope of Practice guidelines and the collegial Medical Director/Mid-Level relationship to meet the health care needs of the patients at the clinic. Follow Clinic Performance Expectations policy. Perform basic radiologic and lab procedures. Perform minor surgical procedures. When the provider enters the room, they check to see if the patient has stamped their card and provide assistance if necessary. The Medical provider checks the batch for correctness and signs the cover sheet. The cards are stored for Quality Assurance. Completes other duties as assigned.
