ESSENTIAL DUTIES AND RESPONSIBILITIES: include the following. Other duties may be assigned.
Will be the primary point of contact for the training of new Residential Aide staff.
Assists in maintaining the unit in an organized, clean, and efficient manner.
Transports patients to and from appointments, support groups, employment searches, and other related activities.
Maintains the security of the assigned unit in accordance with the CTP policy and procedures.
Responsible for immediately reporting child abuse or neglect to the unit supervisor and area director when applicable.
Monitors patients and their activities. Accurately documents the activities of each patient during the assigned shift.
Responsible for reporting all patient infractions, concerns, or problems to the primary counselor or supervisor.
Serves as liaison between shift residential aides and unit supervisor.
Operation oversight of random urinalysis testing of patients in programs.
Ensuring UA results are documented in the electronic health record (EHR) and sent to the lab for chemical testing as indicated.
Operational oversight of patient appointments and transportation needs on each shift.
Ensuring shift reports are passed verbally and by email to the incoming shift.
Ensuring that patient medications are accurately entered into the electronic health record (EHR).

QUALIFICATIONS:
Education:
Requires a High School Diploma or GED.
QMAP certification is preferred but is not required.
CAT certification is preferred but is not required.
Experience:
Must be able to demonstrate a willingness to learn the dynamics of substance misuse and its effects. Prior experience working in a substance misuse setting or a human service agency is preferred.
Other:
Able to communicate effectively. Must be able to follow directions.
CPR and First-Aid certification (initial training and renewals paid for by the agency).
Valid driver's license and insurability.
Basic computer skills are required.

k. (Change) Parents & Children with Special Needs and Health Challenges class

for the classes will target CYS parents and providers and could be taught in an on-

i. (CHANGE) Kids Chat Program. A ninety-minute session for children and
parents that is based off the evidenced based program of Nurturing Parents,
Community based education for Military Families. The kids chat program will be offered
in the evenings, and weekends at the request of units, to provide support to parents with
a deployed spouse. Separate areas will be set up for parents and for children. Child
discussion areas will be by developmental age groups. Child discussion groups will
have a kinetic activity, and discussion will be focused on thriving while a parent is
deployed. Parent discussion groups will be facilitator led focusing on nurturing
strategies for suddenly single parents due to deployment. We anticipate two classes
each month not to exceed 24 classes in a 12-month period. Supplies to facilitate the
discussion about deployment topics, handouts and resources required for the classes
will be government furnished.
j. (CHANGE) Parents Support Group for Waiting Families. A ninety-minute
session held once a month six times a year that aims to provide support to waiting families, where the family has stayed at Fort Carson while their Soldier spouse has
taken an unaccompanied tour of duty. The Waiting Families support group will be for
parents and their children and will be structured as a play group. Nurturing parents
topics will be introduced in each monthly session to provide parenting tips and
resources for families based on the Nurturing Parents, Community based education for
Military Families. Supplies to facilitate the discussion about deployment topics,
handouts and resources required for the classes will be government furnished.
k. (Change) Parents Children with Special Needs and Health Challenges class
will be offered three times a year and will be coordinated through Child, Youth and
School Services. The curriculum will be based off the nurturing parenting evidenced
based program. Three programs will run throughout a one-year time frame. Audiences
for the classes will target CYS parents and providers and could be taught in an on-
premise CYS facility as appropriate for the sign-up. One class will be offered in
coordination with the FAP Manager during a weekend or evening time frame for parents
with children with special needs. Supplies and workbooks will be government furnished.
Instructors will offer sign-up and assessments to attendees. Contractor will provide
follow-up to parents enrolled through the duration of the class to encourage participation
and attendance.

We are adding to our team and are looking for full-time and part-time QMAPs to meet the needs of our Assisted Living Residents. Various shifts available, weekends and holidays required as needed in rotation. $3000 Sign-on Bonus! The individual in this position ensures residents receive optimal care according to their needs and that medications are dispensed appropriately, timely, and accurately. Provides care and assistance to residents in a compassionate, quality customer service environment that preserves dignity. Assist and coordinate residents with various activities, including them in daily tasks and recreational opportunities, providing guidance, ensuring resident's safety, and promoting an engaging and fulfilling experience for all individuals under your care.

Essential Functions

* Sets up medications and passes them to residents following established policies and procedures.
* Performs narcotic counts at the beginning and end of each shift.
* Orders medications from pharmacy following community policies.
* Checks MAR's for error following community policy.
* Calls residents families to bring in medication as needed if applicable.
* Pulls expired medication or discontinued medications, prepares for drug destruction.
* Administers treatments, takes vital signs, and weighs residents.
* Documents/charts on residents' changes and unusual occurrences.
* Documents medical changes and orders on MAR per Dr. orders.
* Ensures lab work, faxes, physician orders, and medication orders are completed and filed according to community policies and procedures.
* Schedules doctor appointments as needed.
* Admits new residents per community policy.
* Assists residents with showers, grooming, dressing, and toileting as needed.
* Assists residents with activities of daily living as described on individual care plans.
* Assists with basic cleaning and maintenance issues when needed.
* Cleans up after incidents in a timely, efficient manner while maintaining the dignity of the resident.
* Assists residents who have fallen providing safety, security, and compassion.
* Escorts residents to and from the dining room as needed.
* Picks up laundry from resident's apartment as needed and transport to laundry room.
* Assists in serving meals and beverages and taking meal orders as needed.
* Answers call lights with patience and compassion following company guidelines.
* Listens to residents with empathy and assurance. Follows up with appropriate personnel regarding resident concerns.
* Makes rounds on resident to ensure their safety and provide assistance as needed.
* Calls Health Services Director, doctors, and families to inform of any change in condition, incidents, medications, etc. as needed.
* Attends meetings/in-services/training sessions as required by Health Services Director and/or Executive Director.
* Maintains confidentiality of residents' personal information in and out of the community and protects and supports residents' rights.
* Communicates with residents, families, visitors, and employees in a positive manner using excellent communication skills.
* Provides tours to potential residents and families that show how the resident's individual needs can be met and promotes the concept of care in the community.
* Maintains a professional appearance and demeanor that encourages a positive, nurturing environment for the residents, families, vendors, and guests.
* Ensures excellent customer service by maintaining open and honest communication, answering phone promptly and professionally, and ensuring residents, families, physicians, and other employee's needs are met.
* Ensures adequate daily coverage for meal service by communicating with the Culinary Services Director and/or Host/Hostess, to include taking resident orders for meals, providing drink refills according to residents' / guests' requests, assists with the cleaning, resetting and preparation of the dining room in-between meal times.
* Performs all other duties as assigned or required.
