DUTIES & ESSENTIAL JOB FUNCTIONS

* Collaborates with Care Transition team and Health Advocates for high-risk patients for timely follow-up appointments and confirms prior to discharge that complex patients are appropriately linked to community services.
* Coordinates patient care activities with other members of the healthcare team, the patient, the patient's representatives, and community partners and makes referrals as appropriate.
* Effectively intervenes in suspected abuse/neglect cases and in complex or high-risk situations as requested; is competent to identify and intervene with high-risk behaviors, responding to traumas.
* Identifies and mobilizes patient and family strengths to optimize the use of healthcare and community resources; in coordination with patient and family wishes, guides/assists in securing needed post-discharge services which may require negotiating for services covered but not readily available; provides consultation and education to team members regarding patient/family (psychosocial and discharge planning) issues and community resources.
* Identifies potential problems, prevents and or resolves variances to the care management plan; assesses and coordinates family and community resources to meet identified needs to support the discharge plan.
* Intervene with patients and patient representatives regarding emotional, behavioral, and financial barriers to current illness and/or disability.
* Leads patient-centered conferences to meet the needs and desires of the patients.
* Maintains patient records including patient assessments, plans, interventions, patient/family involvement, outside agency communications and interdisciplinary contacts.
* Participates in discharge planning activities; effectively identifies and intervenes with high-risk discharge planning issues with psychosocial complexity; whether referred by other healthcare providers or identified through assessment. Assists Care Management Nurse with discharge planning efforts as requested; obtains or coordinates referrals for post-discharge service needs, if required; mobilizes resources to affect the rapid and timely movement of the patient through the system to achieve targeted discharge times established by AHS.
* Performs psychosocial assessment interviews with patients and/or families and records this assessment in the patient's medical record. Assesses patient's level of functioning, environment, appropriateness and adequacy of support system related to illness and ability to cope; reassesses the patient's condition when changes occur and revises the care plan when appropriate. Performs rapid assessments and develops crisis management plans for referral, evaluation and admission.
* Provides patient advocacy, including primary responsibility for initiating processes regarding capacity determinations, grief counseling, and conservatorship/guardianship; takes an advocacy leadership role regarding adoption/surrogacy cases.
* Refers and assists patients/families in applying for appropriate financial programs (CCS, SDI, SSI, SSD, private pensions) and legal instruments as needed.
* Screen for any barriers to care such as substance abuse, neglect, financial limitations or housing.
* Serves as a resource and provides counseling and treatment related to palliative care or end-of-life planning.
QUALIFICATIONS
Required Education: Master's degree in social work/welfare issued by a school accredited by the Counsel of Social Work Education.
Required Experience: Two years of Social work or Case Management experience in an acute setting or protective services.
Preferred Licenses/Certifications: Active certification in Case Management (ACM or CCMC), Current and valid license as a Clinical SocialWorker issued by the State of California Board of Behavior Science Examiners. Bilingual preferred.
PAY RANGE
$53.13 per hour
The pay range for this position reflects the base pay scale for the role at Alameda Health System. Final compensation will be determined based on several factors, including but not limited to a candidate's experience, education, skills, licensure and certifications, departmental equity, applicable collective bargaining agreements, and the operational needs of the organization. 2. Interprets procedures to interested lay persons; and participates in commitment hearings, giving factual and corroborative testimony regarding socio-psychiatric history and present behavior of the patient; contact public social and welfare agencies to obtain and provide information; answers correspondence regarding community facilities, commitment procedures, and mental health matters; and makes referrals when warranted.
3. May provide supervision to personnel assigned to program and participates in the administration of services and facilities.
4. Meets with patients for regular counseling interviews; guides the patient in understanding his illness or addiction and contributing personal, social and economic factors; assists patients in developing realistic plans; advises patients of community resources; and makes referrals and other arrangements as necessary.
5. Prepares reports, case summaries, and correspondence as required; maintains case histories and work records including required statistics.
6. Provides rehabilitation counseling and casework services to patients and consultation to clinic staff members; interviews patients and relatives to gather social and psychiatric data; prepares case histories for use in diagnoses and participates in developing treatment plans for patients.
7. Works with relatives of patients to elicit their cooperation in treatment programs or commitment proceedings; works with community agencies as required to assist patients in their treatment plans and in coping with environmental needs such as housing, medical care, and employment.
**MINIMUM QUALIFICATIONS:**
Education:Possession of a Master's Degree from a recognized college or university in rehabilitation counseling, clinical or educational psychology, or in a closely related field.
Minimum Experience:Two years of experience in the class of Rehabilitation Counselor I with AHS; OR two years of experience in the field of alcoholic or drug abuse rehabilitation or mental treatment.

**Additional Information**
Note: The Per diem/SAN therapist covers the staff therapists when off on leave, vacation or out sick. The SAN therapist covers groups, 3 to 4 per day, for the SMI patients of our PHP/IOP and Wellness programs. Aside from the groups, the SAN therapist would be responsible for writing group notes in Epic, attending the daily Roda meeting to report on patient issues and attending staff meetings.

Screens emotional stability and gives pregnancy test results and provides pregnancy counseling.
Provides educational instruction on contraceptive methods in accordance to Health Education Protocol for Family Planning.
Assesses patient's educational needs and tailors counselors session accordingly.
Reviews tobacco use assessments provided to Family Planning patients.
Conducts teen risk assessment for adolescent patients.
Makes necessary referrals to on-site professional counselors and off-site services as needed.
Performs eligibility screening for all Family Planning patients.
Completes State eligibility forms on Family Planning patients.
Collects and reports statistical information.
Attends workshops, conferences, and/or meetings, which Clinic Supervisor/Manager consider necessary for fulfillment of duties.

CPSP Case Manager Responsibilities and Functions:
Provide CPSP orientations via one (1) hour face to face session.
Provide Prenatal Combined Assessment/Reassessment Tool and Combined postpartum assessment per CPSP guidelines - minimum of three (3) one hour face-to-face assessments per client.
Based on assessment tool responses, provide one to one nutrition, psychosocial counseling and health education to clients.
Develop individual care plan and conduct appropriate follow-up for clients, in person and via phone calls.
Assist in the development/revision of assessment tool protocol.
Consult with psychosocial and nutrition consultants as appropriate.
Report/document client progress needs in medical charts.
Ensure/assist clients with practical arrangements for meeting medical, diagnostic, specialty consults, nutrition, psychosocial and other recommendations.
Assure that perinatology and obstetrical diagnostic reports are received and shared with provider.
Per provider order, contacts patient regarding follow up visits or other medical issues.
Assist patient with hospital pre-registration.
Assist patient with form completion (disability, FMLA, etc.)
Follow-up on Medi-Cal eligibility issues.
Track patient data, including required UDS reporting, and complete reports as required.
Attend Comprehensive Prenatal Health Worker trainings.
Collects and reports statistical information.
Attends department staff meeting including case conference meetings.
Assists with CPSP audits.
Position Requirements and Qualifications:
Ability to work with adults and youth in a variety of settings.
Basic knowledge of reproduction, contraception and related topics.
CHW Certification and counseling skills.
Bilingual in English/Spanish.
One year prenatal care experience.
Excellent verbal communication skills.
Written/oral fluency in Spanish.
Strong written and organizational skills.

Expectations:
This position works directly with patients and families who have complex social situations or extensive psychosocial/mental health needs by assessing the situation, providing necessary intervention and making appropriate referrals or recommendations. It is responsible for supporting the infrastructure necessary to develop and maintain a therapeutic environment for patients/families and staff members. It also provides expertise to staff on legal and ethical issues, mandatory reporting, and psychosocial factors related to the health status of patients. This position requires flexibility of hours.
EDUCATION:
Master's Degree from a graduate school of Social Work accredited by the Council of social Work Education.
EXPERIENCE/TRAINING:
Three to five years of recent relevant experience is preferred.

* A preliminary case service plan shall be developed with the parent(s) or primary caretaker(s) within three working day of admission to the PICU which shall include, but not be limited to, assessment of the following: significant family stress factors, environmental factors, mental health factors, and any other psychosocial factors, and how these factors in the family will be addressed.

Pay Range: The estimated base rate for this position is $75,719.70 - $115,475.40.
Additional individual compensation may be available for this role through differentials, extra shift incentives, bonuses, etc. Base pay is only a portion of the total rewards package, and a comprehensive benefits program is available for qualifying positions.
