JOB SUMMARY: As a key member of the SkillSpring (f/k/a GCD) department, the Workforce Programs Case Coordinator will engage with current participants in the young adult and high school programs in an effort to prepare for and secure employment. The role of the Workforce Programs Care Coordinator is to collaborate with SkillSpring participants using the care management process including: assessing a person's functional level, their physical, cognitive, social-emotional status and their support system; developing a plan of care that addresses the needs and problems identified and incorporating the services that are needed to enhance the current support system; identifying and arranging for coordinated delivery of those services; monitoring changes in the person's condition, and reassessing the person's needs on a regular basis. This team member will serve in a coaching role and effectively advocate for program participants to have their various needs met with compassion and grace. The Workforce Programs Case Coordinator must possess a strong work ethic, a passion for working with underserved young adults, and be able to build and manage relationships with internal and external partners contributing to the professional success of the SkillSpring Department and its program participants. REPORTS TO: Young Adult Program Manager- Workforce Programs DUTIES AND RESPONSIBILITIES: 1. Ensure the health, safety and well-being of participants in the young adult program, by providing support and supervision of all activities. 2. By training the next generation of high-quality, culturally competent health care professionals, SkillSpring generates excitement and enthusiasm for careers serving older adults. Position Overview: The New Jewish Home's SkillSpring program seeks a dedicated and culturally responsive Training Instructor to lead Certified Nursing Assistant (CNA)-aligned work readiness instruction and exam preparation for our Community Resources for Employment and Development Program (“CRED NYC”) Program. This full-time role supports participants through the early and middle stages of the program-covering foundational workplace skills, healthcare career awareness, eldercare practices, and CNA test review. The Training Instructor will work closely with case managers, Nurse Educators, Lehman College faculty, and employer partners to ensure that participants are prepared for both classroom and clinical training, and for long-term success as Certified Nursing Assistants. Key Responsibilities: • Deliver high-quality, trauma-informed instruction for CRED NYC participants, focusing on healthcare career readiness, eldercare, professional development, and Certified Nursing Assistant (CNA) exam preparation. • Facilitate engaging, interactive lessons aligned with DYCD's CRED NYC curriculum requirements, incorporating cultural humility, communication skills, conflict resolution, and job retention strategies. • Collaborate with SkillSpring's case managers, job developer, mental health staff, and Lehman College instructional team to integrate academic, career, and social-emotional learning across program phases. • Support participants' preparation for CNA credentialing through tutoring, study groups, mock exams, and one-on-one academic coaching. • Track participant attendance, progress, engagement, and outcomes using internal databases and DYCD reporting tools. • Ensure daily participant sign-in/sign-out and contribute to monthly data reviews and continuous improvement. • Contribute to recruitment, enrollment, and retention efforts by fostering a welcoming and inclusive learning environment that reflects participants' cultural backgrounds and lived experiences. • Create and adapt intergenerational curriculum and project-based activities that connect healthcare practice to community service and long-term career planning. • Attend team meetings, professional development sessions, and CRED NYC learning exchanges; stay current with best practices in trauma-informed instruction and adult workforce development. • Collaborate with SkillSpring leadership to refine tools, templates, and instructional materials that improve learner outcomes and program quality. • Uphold all CRED NYC contract requirements, deliverables, and compliance protocols. • Perform other duties as assigned to support participant success and program integration. Qualifications: • Bachelor's Degree in Biology or related field required; strong content knowledge in healthcare preferred. • Minimum of 3-5 years of experience delivering training or instruction to young adults or adults from underserved communities. • Passion for adult education, youth development, and healthcare workforce advancement. • Experience preparing learners for credentialing exams and delivering work readiness curriculum. • Demonstrated ability to build trust with learners and apply trauma-informed, culturally responsive teaching practices. • Skilled in curriculum implementation, data tracking, and continuous quality improvement. • Strong communication and interpersonal skills; ability to foster a safe, inclusive learning environment. • Flexible schedule with availability for some evenings and occasional weekends. • Knowledge of healthcare employment pathways, CNA job functions, and eldercare environments. • Familiarity with NYC workforce systems, DYCD programs, or healthcare sector a plus. Physical Demands: • The role requires normal mental and visual attention, along with manual coordination. • Work is performed in lighted, heated, and ventilated areas, with occasional interruptions to workflow. • Duties involve regular periods of sitting, standing, or walking and the ability to work at a computer for sustained periods. • Ability to lift and/or move up to 25 pounds. • Travel to all TNJH campuses and program sites is required.
