We are currently hiring for a full-time Facilities Manager for our location in downtown Albuquerque.
The Facilities Manager is responsible for maintaining the safe and efficient operation of all physical
plant equipment and building systems including capital projects, landscaping, security, fire system,
janitorial, electrical, plumbing and HVAC system. This position also provides oversight of the
maintenance and custodial personnel. Oversees all vehicle fleet operations including driver safety,
driver training, vehicle maintenance, and driver adherence to policy. This position is responsible
for overseeing the completion of all approved facilities capital needs projects.
This position is part of an after-hours emergency team that responds to emergencies such as fire, burglary, significant vandalism, electrical outage, weather, etc.
Duties and Responsibilities:
Monitor day-to-day operations of the Facilities Department including the work, activities,
and scheduling of the custodial and maintenance team.
Provide coaching and professional development to custodial and maintenance team including
conducting regular performance reviews and team meetings.
Oversee facilities and fleet operations and maintenance to ensure all physical aspects of the
AHCH facilities and fleet are functionally correctly and maintained with high quality.
In collaboration with the Chief Operations Officer, verify that all buildings and grounds meet
all health and safety standards and comply with relevant codes and regulations.
Source, negotiate, and manage contracts with third-party vendors for services such as
cleaning, security, landscaping, and maintenance.
Develop and manage budgets for facility operations, projects, and maintenance, ensuring
cost-effectiveness and tracking of expenditures.
Plan and oversee renovations, installations, and refurbishments, from minor upgrades to
major construction projects.
Develop long-term strategies for facility improvement, energy efficiency, and the physical
space's overall usability and sustainability.
Serve as a key member, advisor, leader, and facilitator of campus vigilance and safety efforts
to ensure clients, team members, partners, and visitors are safe while on campus under the
direct oversight and collaboration of the Chief Operations Officer.
Effectively communicate with internal teams, third-party vendors, and upper management.
Serve on a rotated scheduled for after hours response to after-hours emergencies (fire,
burglar, electrical outage, weather, etc.)
Proactively identify issues, make quick decisions, and address unexpected challenges.
Develop and deliver facilities and safety-related presentations to AHCH team members,
volunteers, contractors, and other AHCH affiliates.
Will attend Manager/Coordinators meetings.
Will serve on AHCH Safety Committee with potential for facilitation.
Assist with special projects as needed.
Minimum Qualifications:
Ability to thrive in a complex non-profit healthcare setting.
Ability to work with peers to lead change, high performance, and innovation.
Must have ability to effectively lead capital projects and meet contractual deadlines.
Proficient in Microsoft Office Suite with specific emphasis on the use of Excel
Ability to work in an integrated manner with all levels of organizational staff.
Plumbing, electrical, and general building repair and maintenance experience.
High School Diploma or equivalent.
5 years of experience in facilities maintenance.
2 years of leadership/supervision of a maintenance and/or custodial team.
Preferred Qualifications:
Associates Degree in General Contracting.
Federal procurement experience.
Knowledge of construction principles.
HVAC, Plumbing, Electrical, or other trade certification.
Experience with general duty clause/OSHA compliance.
Knowledge of local/State building codes

Position Details:
Reporting to the Director of Clinical & Quality Outcomes, the Senior Manager of Social Services provides leadership, operational oversight, and strategic direction for the organization's Ryan White, Housing, and Outreach programs. • Provide day-to-day leadership and supervision for Case Management, Ryan White, HOPWA, and Outreach teams.

• Develop, implement, and monitor program workflows, policies, and service delivery models. Represent assigned areas of oversight within the organization serving on committees and workgroups.
• Maintain strong working knowledge of Ryan White Parts B/C/D requirements, HOPWA project criteria, and case management best practices.
• Lead program evaluation and quality improvement initiatives; analyze data and prepare reports for internal leadership and funders.
Staff Supervision & Development
• Hire, train, coach, and evaluate program staff including case managers, outreach workers, and program managers.
• Provide ongoing professional development, including trauma-informed practices, HIV care standards, housing support services, regulatory accountability, and community resource navigation.
• Foster a collaborative, inclusive, and mission-aligned team environment.
Service Delivery & Care Coordination
• Ensure comprehensive, patient-centered case management that supports medical adherence, access to benefits, housing stability, and supportive services.
• Oversee outreach strategies aimed at priority populations, including persons experiencing homelessness, individuals with HIV, and underserved communities.
• Promote strong coordination with medical, behavioral health, pharmacy, and dental teams to integrate social services into whole-person care.
• Support crisis intervention, client advocacy, and resource linkage as needed.
Compliance & Grant Management
• Ensure compliance with all grant conditions, documentation requirements, client eligibility verification, and reporting timelines.
• Monitor program budgets, assist with grant applications, and support fiscal stewardship.
• Maintain accurate records within electronic health record (EHR) systems and external reporting portals (e.g., CAREWare, E2NM, HMIS/HUD systems).

Community Partnerships & Outreach

Position Details:
Reporting to the Vice President of Quality Assurance & Operations, the Behavioral Health Counselor II (BHC II) functions as a core member of a collaborative care team involving the patient's primary care provider, a psychiatric provider, case manager, nurse, and other clinical staff as necessary and appropriate. The BHC II is responsible for ensuring programmatic and therapeutic fidelity and the maximization of internal and community resources in the delivery of behavioral health services.
The BHC II will model professionalism, meet expected performance levels, demonstrate the values of the organization, and most importantly provide patient-focused services. This role will serve as a patient advocate using good judgment, initiative, critical thinking, and effective communication at all times.
Position responsibilities include but not limited to:
Screen and assess patients for common mental and substance use disorders.
Support and closely coordinate mental health care with the patient's primary care provider and other SCC staff as appropriate.
Provide patient education about common mental and substance use disorders and available treatment options.
Monitor clients (in person or by telephone) for changes in clinical symptoms and treatment side effects or complications.
Support psychotropic medication management prescribed by Psychiatric Practitioner, focusing on treatment adherence, attention to side effects, and effectiveness of treatment.
Provide or facilitate in-clinic or outside referrals as needed.
Document patient progress and treatment recommendations in EMR.
Conduct regularly scheduled caseload consultation with the multidisciplinary team and communicate the resulting treatment recommendations to the patient's PCP. These consultations will primarily focus on patients who are new to treatment or who are not improving as expected with recommendations for changes to treatment if needed.
Facilitate patient engagement and follow-up with behavioral health care.
Track patient follow-up and clinical outcomes. Document encounters in EMR and use the system to identify and reengage patients who may be lost to follow-up.
Facilitate referrals for clinically indicated services outside the primary care clinic (e.g., social services such as housing assistance, vocational rehabilitation, mental health specialty care, substance use treatment).
Complete relapse prevention plans with clients with substance use disorders.
Participate in regular training and technical assistance activities for SCC staff.
Provide chart review or other clinical supervision for BHC I staff as assigned.
Other duties as assigned.

Duties and Responsibilities:
Establish systems for contracts, grants, and agreements that ensure all are maintained, tracked, and that relevant documentation accompanies each contract in both digital and hard copies.

Our ideal candidate is someone with strong critical thinking skills, a client-centered mindset, and the ability to work well with diverse populations. The candidate must be comfortable with traveling to clients in various parts of the city and transporting clients in our company vehicles.
AHCH practices a harm reduction approach in all services and we seek candidates that embrace and uphold this philosophy and provide services within a trauma informed framework. We are a housing first, treat first organization that strives to remove as many barriers to health care and housing as possible.
CSW caseloads can vary from 20-30 depending on client acuity and it is expected that the CSW will have a minimum of 60 encounters per month. Additionally, documentation of encounters must be completed within 72 hours of the visit date.
Duties and Responsibilities:
Assess individual needs of clients
Create individualized service plans with direct input from client in accordance with CCSS
requirements
Work in an integrated manner with other AHCH programs to optimize client care
Refer client to appropriate programs and agencies for coordination of wrap-around
services in the community
Participate in client chart audits for peer review
Provide direct service to clients with 60% of contacts conducted in home
visits/community visits
Perform duties in a trauma informed and client centered manner
Ensure accurate and appropriate documentation of all client contacts in Electronic Health
Record (EHR) within timeframe required by AHCH procedure and billing standards
Attend program-specific meetings and trainings including clinical supervision with
Behavioral Health Clinical Director
Attend organizational meetings and trainings
Minimum Qualifications:
Ability to work in a client centered, trauma informed, and harm reduction manner to assist clients in meeting the goals that they develop
NM driver's license (or the ability to obtain an NM driver's license within 30 days of start date) and clean driving record
A bachelor's degree in a human service field from an accredited university and one year relevant experience with the target population
OR
An associate's degree and a minimum of two years of experience working with the target population
OR
A high school graduation or general educational development (GED) test and a minimum of three years of experience working with the target population
Preferred Qualifications:
CCSS certification
Experience in trauma-informed care and harm reduction models
Bilingual or Spanish speaking

Behavioral/ Mental Health Therapist

We are looking for an engaged and integrated Behavioral Health Therapist to provide individual therapy, family therapy, couples therapy, group therapy, crisis intervention, and behavioral health assessments. This person will focus on substance use disorders and other related disorders in collaboration with AHCH Behavioral Health and Medical Teams. The perfect candidate would have experience in treating a high-needs population with serious mental illness and emotional disturbance; we want someone who loves our population.
Duties and Responsibilities:
Provide individual and/or group therapy to a limited number of behavioral health clients
Administer comprehensive diagnostic assessments
Provide Crisis Intervention services as needed
Provide behavioral health services on the AHCH campus, as well as through targeted and site-based Outreaches
Independent licensures may be asked to provide clinical supervision to practicum students/interns
Integrate with Medical and Psychiatric providers for holistic client care
Collaborate with other departments and serve as a BH resource to staff
Attend meetings as required
Participate in peer review of clinical documentation
Participate in quality assurance and performance improvement activities within agency
Comply with all AHCH policies and procedures
Minimum Qualifications:
Master's Degree in Social Work, Professional Counseling, or Marriage and Family Counseling
NM licensure as LPCC, LCSW, LMHC or LMSW (provisionals welcome to apply)
One-year related experience and knowledge of behavioral health, substance abuse, co-occurring disorders, and trauma
Preferred Qualifications:
Previous work with people experiencing homelessness or other under-served populations

The Housing Navigator position will serve as a systems-integrated resource to housing and social services for those currently residing in shelters, respite, hotels, and/or motels, or in places not meant for human habitation.
Duties and Responsibilities:
· Conduct interviews with clients to determine housing needs
· Administer housing assessments (as needed) for those experiencing homelessness
· Acquire and maintain information regarding available housing resources in Bernalillo County, including but not limited to housing vouchers, affordable housing resources, and landlords with available housing stock
· Develop and maintain tracking system(s) for funder reporting (in conjunction Housing Manager/Housing Coordinator)
· Document all encounters and contacts made on behalf of client into EHR within 72 hours
· May assist participants in their housing search, including applying for apartments, negotiating with potential landlords, and accompanying participants to apartment viewings as needed.
· Connect clients to other social services in the community as needed
· Complete releases/requests for information, intakes, referrals, and other required documentation to ensure compliant and efficient transition between community services
· Work closely with housing voucher, social services, and governmental programs in the community
· Work collaboratively and effectively within an integrated team and establishes positive, supportive relationships with staff and providers
· Perform other duties as assigned
Minimum Qualifications:

We are currently hiring for a full-time Facilities Manager for our location in downtown Albuquerque.
The Facilities Manager is responsible for maintaining the safe and efficient operation of all physical
plant equipment and building systems including capital projects, landscaping, security, fire system,
janitorial, electrical, plumbing and HVAC system. This position also provides oversight of the
maintenance and custodial personnel. Oversees all vehicle fleet operations including driver safety,
driver training, vehicle maintenance, and driver adherence to policy. This position is responsible
for overseeing the completion of all approved facilities capital needs projects.
This position is part of an after-hours emergency team that responds to emergencies such as fire, burglary, significant vandalism, electrical outage, weather, etc.
Duties and Responsibilities:
• Monitor day-to-day operations of the Facilities Department including the work, activities,
and scheduling of the custodial and maintenance team.
• Provide coaching and professional development to custodial and maintenance team including
conducting regular performance reviews and team meetings.
• Oversee facilities and fleet operations and maintenance to ensure all physical aspects of the
AHCH facilities and fleet are functionally correctly and maintained with high quality.
• In collaboration with the Chief Operations Officer, verify that all buildings and grounds meet
all health and safety standards and comply with relevant codes and regulations.
• Source, negotiate, and manage contracts with third-party vendors for services such as
cleaning, security, landscaping, and maintenance.
• Develop and manage budgets for facility operations, projects, and maintenance, ensuring
cost-effectiveness and tracking of expenditures.
• Plan and oversee renovations, installations, and refurbishments, from minor upgrades to
major construction projects.
• Develop long-term strategies for facility improvement, energy efficiency, and the physical
space's overall usability and sustainability.
• Serve as a key member, advisor, leader, and facilitator of campus vigilance and safety efforts
to ensure clients, team members, partners, and visitors are safe while on campus under the
direct oversight and collaboration of the Chief Operations Officer.
• Effectively communicate with internal teams, third-party vendors, and upper management.
• Serve on a rotated scheduled for after hours response to after-hours emergencies (fire,
burglar, electrical outage, weather, etc.)
• Proactively identify issues, make quick decisions, and address unexpected challenges.
• Develop and deliver facilities and safety-related presentations to AHCH team members,
volunteers, contractors, and other AHCH affiliates.
• Will attend Manager/Coordinators meetings.
• Will serve on AHCH Safety Committee with potential for facilitation.
• Assist with special projects as needed.
Minimum Qualifications:
• Ability to thrive in a complex non-profit healthcare setting.

Duties and Responsibilities:

Minimum Qualifications:
Experience in social services, behavioral health or health care setting

The Contract Analyst is responsible for analysis and maintenance of all contracts, grants, and agreements within the Administration department of Albuquerque Health Care for the Homeless.
This is an onsite position. Opportunity for a hybrid schedule is a possibility after 90 days, however, this position will require the applicant to live in New Mexico.
Duties and Responsibilities:
· Establish systems for contracts, grants, and agreements that ensure all are maintained, tracked, and that relevant documentation accompanies each contract in both digital and hard copies.
· Review and analyze all contracts, grants, agreements and provide feedback to AHCH leadership to minimize potential risk, identify favorable contract agreement terms, and ensure compliance with all applicable laws/regulations
· Identify the need for new policies and procedures, amendments, or other modifications to agreements in conjunction with AHCH leadership
· Analyze agreements for acceptable business/legal risks and communicate legal implications of contracts to AHCH leadership
· Cross functional collaborative work with AHCH leadership on vendor contract bids including supplier recommendations and preparation of final contracts
· Keep abreast of changes in laws and regulations regarding healthcare, funders, and other applicable laws.
· Monitor and track contract deadlines and conditions
· Financial analysis and reporting: preparing reports on contract performance
· Leading negotiation of contract terms with external parties (achieving favorable outcomes for AHCH)
· Conducting regular internal audits to ensure deliverables, legal requirements, internal policies, administrative requirements are met.
· Maintenance of AHCH Contract Library (i.e. Policies & Procedures, affiliation agreements, etc.)
· Clerical tasks, data entry

Qualifications:
· Familiarity with programs and processes of primary granting agencies, including federal agencies as well as corporate and private sponsors.
· Bachelor's degree in Business, Health Care Administration, or other relevant degree and one year experience OR five years' experience in contract administration
· Keen attention to detail, with an ability to spot errors and inconsistencies
· Strong analytical and organizational skills
