About the Opportunity
A respected home health organization is seeking a dedicated Occupational Therapist to join its compassionate clinical team serving the Mission Valley and greater San Diego area. As an Occupational Therapist in home health, you will assess functional limitations, develop personalized treatment plans, and guide patients and caregivers through rehabilitative techniques designed to enhance safety, mobility, and overall quality of life. You'll work collaboratively with a supportive interdisciplinary team committed to delivering high-quality, patient-centered care.
Key Responsibilities
Conduct evaluations to assess patients' functional abilities, limitations, and home safety needs
Develop and implement individualized occupational therapy treatment plans
Educate patients and caregivers on therapeutic techniques, adaptive strategies, and equipment usage
Document all care accurately and promptly in accordance with regulatory and organizational standards
Partner with nursing, physical therapy, and other disciplines to ensure coordinated, effective care
Travel within the designated territory to provide in-home visits
Required Skills
Graduation from an accredited Occupational Therapy program
Active state licensure as an Occupational Therapist in California
Minimum of 1 year of experience as an Occupational Therapist
Current CPR certification
Valid driver's license, reliable transportation, and automobile insurance

What You'll Do

Qualifications

Job Summary:

is seeking a hand professional to join our clients expert surgical team as an Occupational Therapist, to treat various upper extremity disorders ranging from tendon injuries, sprains, strains, post-operative fractures, and more. We are seeking a therapist who has or is able to acquire their CHT (Certified Hand Therapist certification) and is skilled in custom splinting and other occupational hand related practices.
You will work with our Department CHT Lead and our surgeons to ensure you have what you need for success. You will evaluate, plan, and implement individualized treatment programs in accordance with the occupational therapy principles and collaborate closely with our interdisciplinary treatment team to deliver quality patient care. Primary Responsibilities:
Therapy Services: Responsible for the evaluation, plan of care, treatment, re-evaluation, discharge, and appropriate communications of high quality hand therapy services to patients and customers.
Patient Evaluation & Treatment: Evaluate patients upon physician referral, develop comprehensive and individualized treatment plans, administer procedures and modalities, and plan for discharge using occupational therapy techniques tailored to each patient's age and needs.
Case Management: Serve as a case manager for assigned patients, maintaining accurate records and updating treatment plans with physician approval as necessary.
Patient & Family Education: Provide age-appropriate instructions to patients and their families on therapy procedures, home programs, and the use of assistive devices.
Communication: Collaborate with the rehabilitation team, family, physicians, and third-party payers to discuss patient progress and program changes.
Professional: Seeking positive interaction with staff and clients. Encouragement, team player and staff comradery.
Stay current and up to date with Occupational Therapy research and employ Evidence Based Practice (RCB, clinician expertise, patient perception).
Safety & Cleanliness: Maintain a safe, clean, and organized workspace, reporting any safety issues to supervisors.
Skills:
Experience: Preferred - 1+ years in an acute hospital or outpatient hand clinic; (new graduates may be considered with experience).
Solid experience in application of custom splinting pre and post-op orthopedic
Skills: Strong foundational skills in occupational therapy evaluation, treatment across Pediatric, Adolescent, Adult, and Geriatric age groups.
Interpersonal Skills: Excellent communication and collaboration - Caring, Communication, Collaboration, and Competence.
Physical Effort: Must meet physical demands to lift/push/pull patients and equipment; reasonable accommodations provided if needed.
Requirements:
Licensure & Certification: National registration and state licensure or registration required
Nevada Occupational Therapy license, and current AHA (or equivalent) BLS certification.
Certified Hand Therapist preferred.
CPR certification
Valid state driver's license

DESCRIPTION
Optimize the functionality and skills of patients
Responsible for the evaluation and treatment of residents after a written order received from physician
Screening of facility residents
Completes all necessary documentation
Attend all required meetings and in-services, including but not limited to family meetings, care plans, staffing, utilization review
In conjunction with Director of Rehabilitation, develop programs appropriate to the needs of the facility residents through the continuum of care
Compliant with all infection control procedures and environmental protocol while following OBRA Resident's Rights Guidelines
Provide consultation and clinical supervision to COTAs (may or may not apply)
Maintain valid state license and awareness of issues related to the profession of OT and healthcare environment
Responsible for appropriate scheduling of residents
Facilitates and completes family and facility staff in-services and education
Assist the facility in meeting standards necessary to maintain state and federal certification
Complete all other tasks that are deemed appropriate for the role and assigned by the manager/supervisor
REQUIRED SKILLS
Communication skills both written and verbal
Ability to maintain professionalism in all situations
Time management and ability to manage caseload
Physical Demands: Required to perform a variety of duties relative to the job, some of which may require a moderate to heavy category of physical demand
REQUIRED EXPERIENCE AND QUALIFICATIONS
Minimum Bachelor of Science degree or greater in Occupational Therapy
State licensure
Current certification by NBCOT if required by state
