Mission:
The HR Business Partner (HRBP) position is responsible for aligning business objectives with employees and management in designated business units. The position formulates partnerships across the HR function to deliver value-added service to management and employees that reflects the business objectives of the organization. The HRBP maintains an effective level of business literacy about the business unit's financial position, its midrange plans, its culture and its competition.
What You'll Do
As an HR Business Partner, the role involves collaborating closely with business leaders to understand their needs and align HR strategies and solutions with their goals. This includes developing positive working relationships and business partnerships within designated offices and throughout the company. The HR Business Partner provides guidance and support to business partners on HR policies, procedures, and best practices. They work with HR departments to implement, administer, educate, and deliver HR services, programs, training, and policies that support business units and HR objectives. Additionally, they offer guidance and support to managers on performance management and coaching.
Managing employee relations is a key aspect of the role, involving investigation and developing solutions for fair and timely conflict resolution. The HR Business Partner supports talent management initiatives, including workforce planning, succession planning, and leadership development. They collaborate with office leadership teams to identify employee development and training needs, creating and implementing plans to address those needs. Furthermore, they partner with the business to routinely analyze compensation and provide guidance to maintain or support external competitiveness and internal equity.
Ensuring compliance with HR-related legal requirements, including labor laws and regulations, is another critical responsibility. The HR Business Partner analyzes HR data and metrics to identify trends and opportunities for improvement, regularly evaluating data and providing analytics to assigned groups. They conduct exit interviews, analyze turnover trends, and work with leadership to develop strategies to increase engagement and retention. Participation in the development and implementation of HR programs and initiatives is also part of the role. Regular status updates are provided to HR leadership on the HR Business Partner's progress and impact on the business units. Additionally, they perform other duties as assigned.
Duties/Responsibilities:
* Conducts frequent meetings with respective business partners.
* Consults with leaders and managers, providing HR guidance when appropriate.
* Analyzes trends and metrics in partnership with the HR group to develop solutions, programs and policies.
* Manages and resolves complex employee relations issues. Conducts effective, thorough and objective investigations.
* Maintains in-depth knowledge of legal requirements related to day-to-day management of employees, reducing legal risks and ensuring regulatory compliance. Partners with the legal department as needed/required.
* Provides day-to-day performance management guidance to management (e.g., coaching, counseling, career development, disciplinary actions).
* Works closely with management and employees to improve work relationships, build morale, and increase productivity and retention.
* Provides HR policy guidance and interpretation.
* Provides guidance and input on business restructures, workforce planning and succession planning.
* Identifies training needs for business units and individual executive coaching needs.
* Participates in evaluation and monitoring of training programs to ensure success. Follows up to ensure training objectives are met.
* Performs other related duties as assigned.

Summary:
Responsible for the planning, provision, and coordination of all aspects of direct patient care in accordance with established clinic and departmental policies and procedures, maintaining the highest degree of quality care in a safe environment. Essential Duties and Responsibilities:

* Performs patient evaluation, summarizes objective clinical findings, and develops an accurate individualized plan of care. Ensures that goals are feasible, based upon findings and patient expectations.
* Provides treatment in accordance with physician's orders and established plan of care.
* Evaluates effectiveness of treatment plan and revises plan of care as indicated.
* Communicates/collaborates with patient, family, caregivers, significant other and other members of the healthcare team to promote maximum benefit of care.
* Communicates/collaborates with the patient's referral source such as the physician, NCM, ADJ, etc. regarding the patient's care and progress to promote maximum benefit of care.
* Assess educational needs of the patient, family, caregiver, or significant other and other members of healthcare team to promote maximum benefit of care.
* Demonstrates appropriate knowledge and correct application of clinical techniques, procedures and programs for clinical setting and patient/client populations served.
* Plans and makes appropriate discharge plans.
* Assesses the risks for safety and implements appropriate precautions.
* Complies with appropriate and approved security and safety standards.
* Utilizes appropriate leadership skills in delegating, organizing, and educating coworkers and staff.
* Participates in and contributes to quality improvement process.
* Makes effective use of patient schedule and time by achieving various efficiency standards such as skilled units per visit, visits per day, and evaluations plus two-skilled units.
* Completes timely, competent, and compliant documentation using AgileRPM.

Qualifications:

• Coordinates and facilitates efficient patient flow for the unit. Ensures timely and efficient pull of patients being admitted to the unit as well as timeliness of discharging patients from the unit.
• Assists the manager in achieving and sustaining department and organizational objectives including but not limited to patient experience, length of stay, and unit/patient specific quality measures.
• Participates in department specific initiatives and process improvement projects.
• Participates in peer interviews and provide feedback for employee evaluations as requested by manager.
• Assists with occurrence investigations.

PURPOSE AND SCOPE:
The professional registered nurse Outpatient RN CAP 2 may be an entry level designation into the Clinical Advancement Program (CAP) for new employees who meet the RN CAP 2 criteria or attained through advancement from RN CAP 1. This position is accountable and responsible for the provision and coordination of clinically competent care including assessment, planning, intervention, and evaluation for an assigned group of patients. This may include delegation of appropriate tasks to direct patient care staff including but not limited to RNs, LVN/LPNs, and Patient Care Technicians. As a member of the End Stage Kidney Disease (ESKD) health care team, this position participates in decision-making, teaching, leadership functions, and quality improvement activities that enhance patient care outcomes and facility operations.
PRINCIPAL DUTIES AND RESPONSIBILITIES:
All duties and responsibilities are expected to be performed in accordance with Fresenius Kidney Care policy, procedures, standards of nursing practice, state, and federal regulations.
Performs all essential functions under the direction of the Supervisor and with guidance from the Clinical Educator, Preceptor or in collaboration with other Registered Nurses.
Performs ongoing, systematic collection and analysis of patient data pre - during - post hemodialysis treatment for assigned patients and documents in the patient medical record, makes adjustments or modifications to treatment plan as indicated and notifies Team Leader, Charge Nurse, Supervisor or Physician as needed.
Assesses, collaborates, and documents patient/family's basic learning needs to provide initial and ongoing education to patients and family.
Directs and provides, in collaboration with direct and ancillary patient care staff, all aspects of the daily provision of safe and effective delivery of hemodialysis therapy to assigned patients.
Administers medications as prescribed or in accordance with approved algorithm(s), and documents appropriate medical justification and effectiveness.
Initiates or assists with emergency response measures.
Serves as a resource, leader, coach, mentor and role model for new and incumbent employees by setting examples of appropriate behavior, work habits and attitudes towards patients, co-workers, supervisors and the company at the facility, and area level.
Ensures correct laboratory collection, processing and shipping procedures are performed and reschedules missed or insufficient laboratory collections.
Identifies expected outcomes, documents, and updates the nursing assessment and plan of care for assigned patients through collaboration with the Interdisciplinary Team.
Ensures patient awareness related to transplant and treatment modality options.
Participates in education and quality improvement projects at the facility and area level as directed by Supervisor.
May serve as a Preceptor to new employees.
Required to complete CAP requirements to maintain or advance.
Performs all other duties as assigned by Supervisor.
PHYSICAL DEMANDS AND WORKING CONDITIONS:

SUPERVISION:

EDUCATION and LICENSURE:

EXPERIENCE AND REQUIRED SKILLS:
Entry level for RNs with minimum of 2 years of Nephrology Nursing experience
