Ensures patient visit strings are added to the record to make sure visits are not missed. Assess patients for appropriate placement in Hospice per Medicare guidelines. Make home visits to monitor for signs/symptoms associated with end stage disease and end of life. Implement interventions to palliate symptoms in concert with attending physicians. Make home visits to assess and pronounce death, do post-mortem care, notify County Coroner of patient passing, notify Funeral Home and physician(s). Reinforces diet. Reinforces the CHG bath instructions with patients at Pre-Day call if required. Works with proceduralists, PCP, referring provider in obtaining additional test/forms, teaching on medication, informing patients of lab/test results, assisting patients with ordering additional tests ad deemed by providers for procedural clearance. Performs Post Catheterization Intervention (PCI) patient phone calls to ensure adherence to medication orders.
