As a Home Health Physical Therapist, here's a big-picture view of what you'll do:
Provide home-based physical therapy to patients who have lost mobility, strength and functionality due to an illness or injury
Work as part of a home health team which may include an RN, LPN, CNA, HHA, OT, COTA, PTA and SLP, focused on the patient's plan of care and goals
Observe movements, conduct assessments, document progress and modify care plan as needed
Assist patient with exercises and hands-on therapy to manage pain and increase mobility
Educate patients and family on their plan of care, therapy goals, home exercises, and self-care
A few must-haves for Home Health Physical Therapists:
Graduate of an accredited Physical Therapist Program and active PT license in Colorado
In the last five (5) years, two (2) years of experience as a physical therapist that is relevant to the patient population assigned preferred.
CPR Certification (demonstration course required)
Knowledge of state and federal home health regulations

Requirements
Role Overview:
We are seeking motived RN's with experience in Pediatric Home Health to join our team in Colorado Springs & Surrounding areas.
Key Responsibilities:
Provide private duty nursing care to pediatric patients in their homes.
Administer medication, treatments, and therapies as prescribed by the physician.
Monitor patient's condition and report any changes to the healthcare team.
Assist with activities of daily living and provide emotional support to patients and their families.
Collaborate with other healthcare professionals to ensure optimal patient care.
Maintain accurate and complete medical records using web-based electronic medical records system.
Qualifications:
Active RN License.
Current BLS Certification.
Valid Driver's License and Vehicle Insurance.
Strong time management and prioritization skills.
Excellent assessment skills and attention to detail.

Role Overview
We are looking for compassionate Family Provider CNAs to join our team. This role is ideal for CNAs caring for a family member who want the support, structure, and benefits of working with a trusted home health agency.
As a Family Provider CNA, you'll provide 1:1 care to your loved one, supporting their independence, comfort, and safety while receiving competitive pay, benefits, and agency support.
Key Responsibilities
