What you will do as a Care Coordinator: Manages referrals, hospital discharge coordination, working with providers to complete Care Plans as necessary, creating goals with patients and follow-up with the patients on goal setting, and other duties for care coordination Acts as liaison between clinic and community partners for shared initiatives, and long term collaborations Assists with population health management through working panels and by addressing action items crafted in management meetings from panel review Completes all referrals and referral tracking for patients enrolled in care coordination Partners with patient/care-giver/management in care/decision making Individualizes care/service plan to meet patient/customer needs Modifies interventions/service based on population served Provides patient/employee education based on assessment of learning needs of learner What you will need: High School Diploma or equivalent Basic health care insurance coverage knowledge 2+ years experience in a healthcare setting Some knowledge of EMR software Location: M Health Fairview Clinic and Surgery Center - 909 Fulton St. SE Minneapolis, MN 55455 Hours: 40 hours/week Benefits: This is a fully benefit eligible position: Competitive wages, Healthcare (including vision & dental), 401K, parking & tuition assistance and more!
